Eliminate target_has_exited
Nothing uses this. gdb/ChangeLog: 2018-04-12 Pedro Alves <palves@redhat.com> * target.h (target_ops::to_has_exited): Delete. (target_has_exited): Delete. * target-delegates.c: Regenerate.
