1) moved author email addresses and manual version info back onto copyright

page (had crept onto front of title page w/ other stuff migrating between
gdb.texinfo and gdb.top-m4).

2) gdb.top-m4: added Detailed Node Listing to main menu.
               made $Id: for fragment into texinfo comment.
