This commit applies all changes made after running the gdb/copyright.py
script.
Note that one file was flagged by the script, due to an invalid
copyright header
(gdb/unittests/basic_string_view/element_access/char/empty.cc).
As the file was copied from GCC's libstdc++-v3 testsuite, this commit
leaves this file untouched for the time being; a patch to fix the header
was sent to gcc-patches first.

For gdb_bfd_ref_ptr, gdb already had a convenience function like the
new gdb_ref_ptr::new_reference -- called new_bfd_ref. This patch
removes it in favor of the new common function.
While doing this I also noticed that the comment for gdb_bfd_open was
incorrect (in a way related to reference counting), so this patch
updates the comment as well.

This converts observers from using a special source-generating script
to be plain C++. This version of the patch takes advantage of C++11
by using std::function and variadic templates; incorporates Pedro's
patches; and renames the header file to "observable.h" (this change
eliminates the need for a clean rebuild).
Note that Pedro's patches used a template lambda in tui-hooks.c, but
this failed to compile on some buildbot instances (presumably due to
differing C++ versions); I replaced this with an ordinary template
function.
Regression tested on the buildbot.

This patch gets rid of catch_exceptions / catch_exceptions_with_msg.
The latter is done mostly by getting rid of the three remaining
vestigial libgdb wrapper functions, which are really pointless
nowadays. This results in a good number of simplifications.
(I checked that Insight doesn't use those functions.)
The gdb.mi/mi-pthreads.exp change is necessary because this actually
fixes a bug, IMO -- the patch stops MI's -thread-select causing output
on the CLI stream.

* gdb.mi/mi-pthreads.exp (check_mi_thread_command_set): Don't
expect CLI output.
This introduces a new specialization of gdb::ref_ptr that can be used
to manage BFD reference counts. Then it changes most places in gdb to
use this new class, rather than explicit reference-counting or
cleanups. This patch removes make_cleanup_bfd_unref.
If you look you will see a couple of spots using "release" where a use
of gdb_bfd_ref_ptr would be cleaner. These will be fixed in the next
patch.
I think this patch fixes some latent bugs. For example, it seems to
me that previously objfpy_add_separate_debug_file leaked a BFD.
I'm not 100% certain that the macho_symfile_read_all_oso change is
correct. The existing code here is hard for me to follow. One goal
of this sort of automated reference counting, though, is to make it
more difficult to make logic errors; so hopefully the code is clear
now.

We have a case in solib-svr4.c where we could reuse symfile-mem.c's
vDSO range lookup. Since symfile-mem.c is not present in all
configurations solib-svr4.c is, move that lookup to a gdbarch hook.
This has the minor (good) side effect that we stop even trying the
target_auxv_search lookup against targets that don't have a concept of
a vDSO, in case symfile-mem.c happens to be linked in the build
(--enable-targets=all).
Tested on x86_64 Fedora 20.

Target sections added by the add-symbol-file-from-memory command are not
removed when the process exits. In fact, they are not removed, at all.
This causes GDB to crash in gdb.base/break-interp.exp.
Change the owner of those target sections to the object file generated in
symbol_file_add_from_memory and generalize the free_objfile observer in
symfile.c to remove target sections of any freed object file.
When loading symbols for the vdso, also add its sections to target_sections.
This fixes an issue with record btrace where vdso instructions could not be
disassembled during replay.

When a VDSO gets large enough that it doesn't entirely fit in one page,
but not so large that the part described by the program header exceeds
one page, then gdb/BFD doesn't read the section headers and symbol
table information. This patch cures that by passing the size of the
vdso to BFD, and fixes a number of other issues in the BFD code.
