The copyright header in most of GDB files were changed from mail address
to the URL in the conversion to GPLv3 in Aug 2007. However, some files
still use mail address instead of the URL. This patch fixes them.

Remove code relevant for printing C/C++ Integer values in a
Fortran specific file to unify printing of Fortran values.
This does not change the output.
Printing the prefix "PTR TO -> (" resp. "REF TO ->(" ignored the active
indentation level. This caused inconsistent appearance of user-defined
Fortran types containing pointers. Fix by using "fprintfi_filtered" with the
current indentation level for outputting the prefix string. Add test case
ptr-indentation.

This applies the second part of GDB's End of Year Procedure, which
updates the copyright year range in all of GDB's files.

Using the default lookup for the symbol "this" might lead to segmentation
fault in GDB.
Some languages, e.g. Fortran, use as default lookup routine the C++
routines.
For those languages "this" can be the instance of a class or even the
definition of a class.
When an instance of a class having the name "this" is evaluated
in GDB a segmentation fault was observed.
As example of the issue take into consideration the Fortran code:

type(foo) :: this
Issue appears when evaluating the variable "this" in GDB.
Within the language definition structure there is a field that represents
the name of the special symbol used for the C++ "this" for the language
being described.
The fix presented here takes into account the aforementioned field. In the
case the aforementioned field is NULL "this" is not represented in the
language described and the lookup should return a null_block_symbol.
Tests: Performed tests with gfortran and ifort.

Instead of pre-computing indices into a fortran array re-use
the value_* interfaces to subscript a fortran array.
The benefit of using the new interface is that it takes care of
dynamic types and resolve them when needed.

When running tests in parallel, each test puts its generated files in a
different directory, under "outputs". I think it would be nice if it
was always the case, as it would isolate the test cases a bit more. An
artifact created by a test wouldn't get overwritten by another test.
Also, it makes it easier to clean up. A lot of executables are left all
over the place because their names do not appear in gdb.*/Makefile. If
everything is in "outputs", then we just have to delete that directory
(which we already do).
At the same time it makes the gdb.foo directories and their Makefiles
useless in the build directory, since they are pretty much only used for
cleaning.

In vla.f90, this single line of source is compiled to many instructions,
vla2(:, :, :) = 1311 ! vla2-allocated
it is quite slow (about several minutes in my testing) to step over this
source line without range stepping. This patch is to increase the timeout
value by 15 times, which is a magic number to make sure timeout disappears
in my testing with a slow arm-linux board.

or associated with a variable under explicit program control.
The purpose of this commit is:

in case the type is allocated or associated (pointer to
dynamic array).

This fixes two FAIL results on this testcase which were caused by a
misplaced "continue" command. This testcase used to end inferior's
execution too soon, causing the following tests to fail. Now we break
right after inferior's loop and perform the rest of the tests there.

This commit fixes the PR mentioned in $subject. It is about a set but
unused variable that refers to the output format of integer values
printed in Fortran.
This was probably a thinko (like most set-but-unused-vars), but it
could cause an internal error depending on the scenario. I am sending
a testcase which triggers this error as well.

* f-exp.y (yylex): Add domain array to enable lookup
in multiple domains. Loop over lookup domains and try
to find requested symbol. Add STRUCT_DOMAIN to lookup
domains to be able to query for user defined types.
testsuite/
* gdb.fortran/type.f90: New file.
* gdb.fortran/whatis_type.f90: New file.
Two modifications:
1. The addition of 2013 to the copyright year range for every file;
2. The use of a single year range, instead of potentially multiple
year ranges, as approved by the FSF.
Update some files in GDB that were accidently left with a GPL v2
copyright header.
Update some files where the copyright notice still provides the
old contact info, using the approach for providing the FSF's contact
info.
