This applies the second part of GDB's End of Year Procedure, which
updates the copyright year range in all of GDB's files.

Update copyright year range in all GDB files.
Two modifications:
1. The addition of 2013 to the copyright year range for every file;
2. The use of a single year range, instead of potentially multiple
year ranges, as approved by the FSF.
All callers updated.
(amd64_get_insn_details): Handle more 3-byte opcode insns.
(amd64_breakpoint_p): Delete.
(amd64_displaced_step_fixup): When fixing up after stepping an int3,
don't back up pc to the start of the int3.
