This applies the second part of GDB's End of Year Procedure, which
updates the copyright year range in all of GDB's files.
gdb/ChangeLog:
Update copyright year range in all GDB files.
Both of them are used in conversion. We can remove them since the
conversion is done.
There are many architectures only have one breakpoint instruction,
so their gdbarch methods breakpoint_kind_from_pc and
sw_breakpoint_from_kind look very similar. Instead of macro, we
use template "template <size_t, const gdb_byte *> struct bp_manipulation"
for these architectures. In order to use template, I also change
breakpoint instruction of type "static const gdb_byte[]" to
"constexpr gdb_byte[]", and rename them to ARCH_break_insn.

This change updates the mn10300 dwarf register map. It reduces the
failure count when doing simulator testing against the default
multilib from 788 to 99.
gdb/ChangeLog:
* mn10300-tdep.c (mn10300_dwarf2_reg_to_regnum): Rewrite
dwarf2_to_gdb[] table using symbolic constants. Adjust
penultimate entry from number representing the PC register
to symbolic constant representing the MDR register. Add
constant for the PC register to the end of the table.
This removes gdb_string.h. This patch is purely mechanical. I
created it by running the two commands:
git rm common/gdb_string.h
perl -pi -e's/"gdb_string.h"/<string.h>/;' *.[chyl] */*.[chyl]

This adds -Wold-style-declaration to gdb's list of warnings.
It turns out that a few places use "const static" rather than
"static const". The former is deprecated according to the C standard.
Tested by rebuilding with --enable-targets=all on x86-64 Fedora 18.

-Wpointer-sign catches all these cases across the codebase that should
be using gdb_byte for raw target bytes. I think these are all
obvious, hence I've collapsed into a single patch.

gdb_byte should be used for bytes from the program being debugged. We
have many places using char or unsigned char instead all over the
existing ports, and more ends up added over time due to copy/paste as
new code is based on old code.
I've greped the tree for "char buf[", and fixed all I found.
Tested by building with --enable-targets=all.
