* gdb.texinfo (Top): Add GDB/MI to the main menu and @include
gdbmi.texinfo.
(Mode Options): Add xref to GDB/MI docs and remove a FIXME
comment.
* gdbmi.texinfo: Lots of changes, to include this document as part
