Tom Tromey 2e78302469 Use ui_out_emit_tuple 
This patch changes various places to use ui_out_emit_tuple,
eliminating a number of cleanups.  This patch only tackles "easy"
cases, which are ones where the cleanups in question were
block-structured and did not involve any changes other than the
obvious replacement.

Tom Tromey 192b62ce0b Use class to manage BFD reference counts 
This introduces a new specialization of gdb::ref_ptr that can be used
to manage BFD reference counts.  Then it changes most places in gdb to
use this new class, rather than explicit reference-counting or
cleanups.  This patch removes make_cleanup_bfd_unref.

If you look you will see a couple of spots using "release" where a use
of gdb_bfd_ref_ptr would be cleaner.  These will be fixed in the next
patch.

I think this patch fixes some latent bugs.  For example, it seems to
me that previously objfpy_add_separate_debug_file leaked a BFD.

I'm not 100% certain that the macho_symfile_read_all_oso change is
correct.  The existing code here is hard for me to follow.  One goal
of this sort of automated reference counting, though, is to make it
more difficult to make logic errors; so hopefully the code is clear
now.

Simon Marchi 112e8700a6 Class-ify ui_out 
This patch finalizes the C++ conversion of the ui-out subsystem, by
turning the ui_out and ui_out_impl structures into a single class
hierarchy.  ui_out functions are turned into virtual methods of that new
class, so as a result there are a lot of call sites to update.

In the previous version of the patchset, there were separate ui_out and
ui_out_impl classes, but it wasn't really useful and added boilerplate.
In this version there is simply an ui_out base class that is
extended for CLI, TUI and MI.

It's a bit hard to maintain a ChangeLog for such a big patch, I did my
best but I'm sure there are some missing or outdated info in there...

Simon Marchi 9a3c826307 Add some more casts (1/2) 
Note: I needed to split this patch in two, otherwise it's too big for
the mailing list.

This patch adds explicit casts to situations where a void pointer is
assigned to a pointer to the "real" type.  Building in C++ mode requires
those assignments to use an explicit cast.  This includes, for example:

 - callback arguments (cleanups, comparison functions, ...)
 - data attached to some object (objfile, program space, etc) in the form
   of a void pointer
 - "user data" passed to some function

This patch comes from the commit "(mostly) auto-generated patch to insert
casts needed for C++", taken from Pedro's C++ branch.

Only files built on x86 with --enable-targets=all are modified, so the
native files for other arches will need to be dealt with separately.

I built-tested this with --enable-targets=all and reg-tested.  To my
surprise, a test case (selftest.exp) had to be adjusted.

Here's the ChangeLog entry.  Again, this was relatively quick to make
despite the length, thanks to David Malcom's script, although I don't
believe it's very useful information in that particular case...

Simon Marchi 224c3ddb89 Add casts to memory allocation related calls 
Most allocation functions (if not all) return a void* pointing to the
allocated memory.  In C++, we need to add an explicit cast when
assigning the result to a pointer to another type (which is the case
more often than not).

The content of this patch is taken from Pedro's branch, from commit
"(mostly) auto-generated patch to insert casts needed for C++".  I
validated that the changes make sense and manually reflowed the code to
make it respect the coding style.  I also found multiple places where I
could use XNEW/XNEWVEC/XRESIZEVEC/etc.

Thanks a lot to whoever did that automated script to insert casts, doing
it completely by hand would have taken a ridiculous amount of time.

Only files built on x86 with --enable-targets=all are modified.  This
means that all other -nat.c files are untouched and will have to be
dealt with later by using appropiate compilers.  Or maybe we can try to
build them with a regular g++ just to know where to add casts, I don't
know.

I built-tested this with --enable-targets=all and reg-tested.

Gary Benson 4313b8c0ed Warn when accessing binaries from remote targets 
GDB provides no indicator of progress during file operations, and can
appear to have locked up during slow remote transfers.  This commit
updates GDB to print a warning each time a file is accessed over RSP.
An additional message detailing how to avoid remote transfers is
printed for the first transfer only.

Andrew Burgess c04fe68f6b gdb: Improve cache matching criteria for the bfd cache. 
Within gdb open bfd objects are reused where possible if an attempt is
made to reopen a file that is already being debugged.  To spot if the on
disc file has changed gdb currently examines the mtime of the file and
compares it to the mtime of the open bfd in the cache.

A problem exists when the on disc file is being rapidly regenerated, as
happens, for example, with automated testing.  In some cases the file is
generated so quickly that the mtime appears not to change, while the on
disc file has changed.

This patch extends the bfd cache to also hold the file size of the file,
the inode of the file, and the device id of the file; gdb can then
compare filename, file size, mtime, inode, and device id to determine if
an existing bfd object can be reused.

Gary Benson e3dd7556ad Move vgdb special case into remote_filesystem_is_local 
Valgrind GDB (vgdb) presents itself as a remote target but works on
the local filesystem.  gdb_bfd_open contained a special case to make
vgdb work with "target:" sysroots, but the implementation meant that
GDB would fall back to the local filesystem if *any* to_fileio_open
method failed with ENOSYS for *any* reason.  This commit moves the
vgdb special case to remote_filesystem_is_local to allow the fallback
to be restricted only to the specific case that remote file transfer
is unsupported.  This commit also adds a warning which is displayed
the first time the fallback is used.

Gary Benson f08e97fed1 Make gdb_bfd_open able to open BFDs using target fileio 
This commit updates gdb_bfd_open to access files using target
fileio functions if the supplied path starts with "target:"
and if the local and target filesystems are not the same.
This allows users to specify "set sysroot target:" and have
GDB access files locally or from the remote as appropriate.

The new functions in gdb_bfd.c are copies of functions from
remote.c. This duplication is intentional and will be removed
by the next commit in this series.

Indexes returned for special sections are off by one, i.e. with N+4
sections last one has index N+4 returned which is outside allocated
obstack (at the same time index N is not used at all).

In worst case, if sections obstack is allocated up to end of chunk,
writing last section data will cause buffer overrun and some data
corruption.

Tom Tromey 1da77581c0 don't share per-BFD data if relocations are needed 
Right now we always share per-BFD data across objfiles, if there is a
BFD.  This works fine.  However, we're going to start sharing more
data, and sometimes this data will come directly from sections of the
BFD.  If such a section has SEC_RELOC set, then the data coming from
that section will not be truly sharable -- the section will be
program-space-dependent, and re-read by gdb for each objfile.

This patch disallows per-BFD sharing in this case.  This is a bit
"heavy" in that we could in theory examine each bit of shared data for
suitability.  However, that is more complicated, and SEC_RELOC is rare
enough that I think we needn't bother.

Note that the "no sharing" case is equivalent to "gdb works as it
historically did".  That is, the sharing is a new(-ish) optimization.

Built and regtested on x86-64 Fedora 18.

Joel Brobecker 28e7fd6234 Update years in copyright notice for the GDB files. 
Two modifications:
  1. The addition of 2013 to the copyright year range for every file;
  2. The use of a single year range, instead of potentially multiple
