	Include cpphash.h.

	* cpplib.c (do_define): Generalise to handle poisoned definitions,
	redefining poisoned identifiers, etc.
	(do_undef): Don't allow poisoned identifiers to be undefined.
	(do_pragma): Add #pragma poison.
	(do_xifdef): Handle `#ifdef xxx' for poisoned xxx.
