Jonathan Wakely 6a28a7531b Improvements to the libstdc++ FAQ and manual 
* doc/xml/faq.xml: Update several old entries. Improve
	cross-references.
	* doc/xml/manual/intro.xml: Add anchors to each DR.
	* doc/html/*: Regenerate.

New users may wish to point their web browsers to the file
index.html in the 'doc/html' subdirectory.  It contains brief
