	* include/debug/set.h, unordered_map, multiset.h, forward_list,
	unordered_set, vector, deque, string, list, multimap.h: Remove base
	class default constructor calls.
	* include/debug/map.h: Likewise and cleanup several redefinition of
	base iterator typedef.
