	[__cplusplus >= 201103L](_M_initialize_dispatch): Remove.
	[__cplusplus >= 201103L](_M_assign_dispatch): Remove.
	[__cplusplus >= 201103L](_M_insert_dispatch): Remove.
	* testsuite/23_containers/deque/allocator/default_init.cc: New.

index.html in the 'doc/html' subdirectory.  It contains brief
building instructions and notes on how to configure the library in
