block/loop: implement REQ_FLUSH/FUA support

Deprecate REQ_HARDBARRIER and implement REQ_FLUSH/FUA instead.  Also,
instead of checking file->f_op->fsync() directly, look at the value of
vfs_fsync() and ignore -EINVAL return.
