mm: introduce helper function set_max_mapnr()

Introduce a helper function set_max_mapnr() to set global variable
max_mapnr.

Also unify condition compilation for max_mapnr with
CONFIG_NEED_MULTIPLE_NODES instead of CONFIG_DISCONTIGMEM.
