David S. Miller df568d8e52 scsi: Use 'depends' with LIBFC instead of 'select'.
LIBFC depends upon SCSI_FC_ATTRS and select's CRC32C.

The only alternative would be to 'select' CRC32C and all of
SCSI_FC_ATTRS direct and indirect dependencies in the Kconfig section
for every LIBFCOE user which makes little sense.

Subsequently, use 'depends' instead of 'select' for LIBFCOE too.

Chad Dupuis 5565461e30 bnx2fc: fix incorrect DMA memory mapping in bnx2fc_unmap_sg_list()
This patch is based on a problem and solution from Maurizio Lombardi
where bnx2fc isn't consistent in which device struct we using for DMA
map and unmap operations.  Make them consistent by using dma_sg_unmap
in bnx2fc_unmap_sg_list like bnx2fc_map_sg.

Anish Bhatt 23a3c99201 bnx2i/bnx2fc : fix randconfig error in next-20140909
Just like CNIC, tristate of these two modules is also dependent on IPV6.
These need to be handled separately as they select CNIC, which can override
tristate for CNIC from 'm' to 'y', which can cause build failures when ipv6 is
 compiled as a module even if CNIC's Kconfig will only 'm' or 'n' when ipv6 is
compiled as a module.

Hannes Reinecke 9cb78c16f5 scsi: use 64-bit LUNs
The SCSI standard defines 64-bit values for LUNs, and large arrays
employing large or hierarchical LUN numbers become more and more
common.

So update the linux SCSI stack to use 64-bit LUN numbers.

Its due to the fact that we call a potentially sleeping function from the bnx2fc
rcv path with preemption disabled (via the get_cpu call embedded in the per-cpu
variable stats lookup in bnx2fc_l2_rcv_thread.

Easy enough fix, we can just move the stats collection later in the function
where we are sure we won't preempt or sleep.  This also allows us to not have to
enable pre-emption when doing a per-cpu lookup, since we're certain not to get
rescheduled.

Maurizio Lombardi 9172b763a7 bnx2fc: do not scan uninitialized lists in case of error.
In case of of error, the bnx2fc_cmd_mgr_alloc() function will call
the bnx2fc_cmd_mgr_free() to perform the cleanup.
The problem is that in one case the latter may try to scan
some not-yet initialized lists, resulting in a kernel panic.

This patch prevents this from happening by freeing the lists
before calling bnx2fc_cmd_mgr_free().

Maurizio Lombardi 91d9f809da bnx2fc: fix memory leak and potential NULL pointer dereference.
If bnx2fc_allocate_hash_table() for some reasons fails, it is possible that the
hash_tbl_segments or the hash_tbl_pbl pointers are NULL.
In this case bnx2fc_free_hash_table() will panic the system.

this patch also fixes a memory leak, the hash_tbl_segments pointer was never
freed.

Srivatsa S. Bhat 7229b6d0b2 scsi, bnx2fc: Fix CPU hotplug callback registration
Subsystems that want to register CPU hotplug callbacks, as well as perform
initialization for the CPUs that are already online, often do it as shown
below:

	get_online_cpus();

	for_each_online_cpu(cpu)
		init_cpu(cpu);

	register_cpu_notifier(&foobar_cpu_notifier);

	put_online_cpus();

This is wrong, since it is prone to ABBA deadlocks involving the
cpu_add_remove_lock and the cpu_hotplug.lock (when running concurrently
with CPU hotplug operations).

Instead, the correct and race-free way of performing the callback
registration is:

	cpu_notifier_register_begin();

	for_each_online_cpu(cpu)
		init_cpu(cpu);

	/* Note the use of the double underscored version of the API */
	__register_cpu_notifier(&foobar_cpu_notifier);

	cpu_notifier_register_done();

Fix the bnx2fc code in scsi by using this latter form of callback
registration.

Michael Chan be1fefc214 cnic,bnx2i,bnx2fc: Fix inconsistent use of page size
The bnx2/bnx2x rings are made up of linked pages.  However there is an
upper limit on the page size as some the page size settings are 16-bit
in the hardware/firmware interface.  In the current code, some parts
use BNX2_PAGE_SIZE which has a 16K upper limit and some parts use
PAGE_SIZE.  On archs with >= 64K PAGE_SIZE, it generates some compile
warnings.  Define a new CNIC_PAGE_SZIE which has an upper limit of
16K and use it consistently in all relevant parts.

Eddie Wai 245a575424 [SCSI] bnx2fc: Fixed the handling for the SCSI retry delay
SCSI retry delay upon SAM_STAT_BUSY/_SET_FULL was not being handled
in bnx2fc.  This patch adds such handling by returning TARGET_BUSY
to the SCSI ML for the corresponding LUN until the retry timer expires.

Eddie Wai 06c4f20d28 [SCSI] bnx2fc: Fixed scsi_remove_target soft lockup when rmmod bnx2x
The problem has been identified to be a change in the scsi_remove_device
path where a call to the pm_runtime_set_memalloc_noio was added when
del_gendisk is called in this path.  Note that the new pm routine
attempts to cycle through all parent devices from the FC target device
to set the memalloc_noio flag.  Because of this new change, a dependency
was created between the FC target device and the parent netdev device
in the destroy path.

In order to synchronized the destroy paths, bnx2fc has been modified
to flush all destroy workqueues in the NETDEV_UNREGISTER return path.

Eddie Wai 0680810c11 [SCSI] BNX2FC: hung task timeout warning observed when rmmod bnx2x with active FCoE targets
[v2] - removed the interface->enabled flag setting which prevented the
       fcoe ctlr link from being brought back up after a MTU change

A rtnl_lock deadlock was observed from the rmmod thread where it
tries to unregister the fcoe_ctlr device.  This unregistration
triggered a flush of the sysfs queue of the associated ctlr and led to
a call to the set_fcoe_ctlr_enabled routine.  This will eventually propagate
down to call the bnx2fc_disable routine and contented for the rtnl_lock
in the same context.

This patch creates a subset of the bnx2fc_enable/disable routine which
removes the unnecesary rtnl_lock and the bnx2fc_dev_lock acquisition from
the set_fcoe_ctlr_enabled path.

Eddie Wai 2183789609 [SCSI] bnx2fc: Fixed a SCSI CMD cmpl race condition between ABTS and CLEANUP
In the case when a SCSI_CMD times out, bnx2fc will initiate the sending of the
ABTS.  However, if the SCSI layer's SCSI command timer also times out, it'll
instantiate a task abort of the same xid.

The race condition this patch tries to fix is as follows:

SCSI_CMD timeout (20s)
thread 1                   thread 2
send ABTS
rx ABTS cmpl
                           task abort_eh
                           explicit LOGO since ABTS was engaged
                           CLEANUP cmpl
SCSI_CMD cmpl (ABTS cmpl)
instantiate RRQ
wait 10s
attempt to send RRQ (because of LOGO, it wouldn't continue)

Note that there is no call to scsi_done for this SCSI_CMD cmpletion
in this path.

The patch changes the path of execution to call scsi_done immediately
instead of instantiating the RRQ.

Robert Love 9d34876f82 libfcoe: Make fcoe_sysfs optional / fix fnic NULL exception
fnic doesn't use any of the create/destroy/enable/disable interfaces
either from the (legacy) module paramaters or the (new) fcoe_sysfs
interfaces. When fcoe_sysfs was introduced fnic wasn't changed since
it wasn't using the interfaces. libfcoe incorrectly assumed that that
all of its users were using fcoe_sysfs and when adding and deleting
FCFs would assume the existance of a fcoe_ctlr_device. fnic was not
allocating this structure because it doesn't care about the standard
user interfaces (fnic starts on link only). If/When libfcoe tried to use
the fcoe_ctlr_device's lock for the first time a NULL pointer exception
would be triggered.

Since fnic doesn't care about sysfs or user interfaces, the solution
is to drop libfcoe's assumption that all drivers are using fcoe_sysfs.

This patch accomplishes this by changing some of the structure
relationships.

We need a way to determine when a LLD is using fcoe_sysfs or not and
we can do that by checking for the existance of the fcoe_ctlr_device.
Prior to this patch, it was assumed that the fcoe_ctlr structure was
allocated with the fcoe_ctlr_device and immediately followed it in
memory. To reach the fcoe_ctlr_device we would simply go back in memory
from the fcoe_ctlr to get the fcoe_ctlr_device.

Since fnic doesn't allocate the fcoe_ctlr_device, we cannot keep that
assumption. This patch adds a pointer from the fcoe_ctlr to the
fcoe_ctlr_device. For bnx2fc and fcoe we will continue to allocate the
two structures together, but then we'll set the ctlr->cdev pointer
to point at the fcoe_ctlr_device. fnic will not change and will continue
to allocate the fcoe_ctlr itself, and ctlr->cdev will remain NULL.

When libfcoe adds fcoe_fcf's to the fcoe_ctlr it will check if ctlr->cdev
is set and only if so will it continue to interact with fcoe_sysfs.

changed the configuration of the doorbell HW and it broke iSCSI and FCoE.
We fix this by making compatible changes to the doorbell address in bnx2i
and bnx2fc.  For the userspace driver, we need to pass a modified CID
so that the existing userspace driver will calculate the correct doorbell
address and continue to work.

incorrectly assumes that abort failed and performes recovery by issuing

The firmware supports a maximum of 4K FCoE exchanges. In 4-port devices,
or when working in multi-function mode, this resource needs to be distributed
between the various possible FCoE functions.

This information needs to be calculated by bnx2x and propagated into bnx2fc
via cnic. bnx2fc can then use this value to calculate corresponding xid
resources instead of using global constants.

and therefore fcoemon does not create the fcoe interface
for bnx2fc.

This patch sets up the correct linkage for bnx2fc such that

Bhanu Prakash Gollapudi e7f4fed5fa [SCSI] bnx2fc: Map the doorbell register between offload and enable requests
We used to map doorbell register after FW enable request is complete.
This causes a race condition when unsolicited event is received, and FW
sends a CQE for it. Since the doorbell is not mapped, driver does not
arm CQ, which means FW will not notify the driver for further CQ
completions.  To resolve this, map the doorbell between offload and
enable, so that driver is ready to receive the unsolicited packets and
arm the CQ as soon as FW enable is performed.

Bhanu Prakash Gollapudi 50b7186f64 [SCSI] bnx2fc: support software fcoe target
Software FCoE target always advertises RETRY bit even when there are no
tape LUNs behind the target. This causes the driver to enable FW support
for sequence level error recovery and perform REC/SRR. This patch
arrests the behavior by not enabling SLER feature for this target.

Robert Love 6e89ea3f10 bnx2fc: Use the fcoe_sysfs control interface
This patch adds support for the new fcoe_sysfs
control interface to bnx2fc.ko. It keeps the deprecated
interface in tact and therefore either the legacy
or the new control interfaces can be used. A mixed mode
is not supported. A user must either use the new
interfaces or the old ones, but not both.

The fcoe_ctlr's link state is now driven by both the
netdev link state as well as the fcoe_ctlr_device's
enabled attribute. The link must be up and the
fcoe_ctlr_device must be enabled before the FCoE
Controller starts discovery or login.

Robert Love 6a891b071b libfcoe, fcoe, bnx2fc: Add new fcoe control interface
This patch does a few things.

1) Makes /sys/bus/fcoe/ctlr_{create,destroy} interfaces.
   These interfaces take an <ifname> and will either
   create an FCoE Controller or destroy an FCoE
   Controller depending on which file is written to.

   The new FCoE Controller will start in a DISABLED
   state and will not do discovery or login until it
   is ENABLED. This pause will allow us to configure
   the FCoE Controller before enabling it.

2) Makes the 'mode' attribute of a fcoe_ctlr_device
   writale. This allows the user to configure the mode
   in which the FCoE Controller will start in when it
   is ENABLED.

   Possible modes are 'Fabric', or 'VN2VN'.

   The default mode for a fcoe_ctlr{,_device} is 'Fabric'.
   Drivers must implement the set_fcoe_ctlr_mode routine
   to support this feature.

   libfcoe offers an exported routine to set a FCoE
   Controller's mode. The mode can only be changed
   when the FCoE Controller is DISABLED.

   This patch also removes the get_fcoe_ctlr_mode pointer
   in the fcoe_sysfs function template, the code in
   fcoe_ctlr.c to get the mode and the assignment of
   the fcoe_sysfs function pointer to the fcoe_ctlr.c
   implementation (in fcoe and bnx2fc). fcoe_sysfs can
   return that value for the mode without consulting the
   LLD.

3) Make a 'enabled' attribute of a fcoe_ctlr_device. On a
   read, fcoe_sysfs will return the attribute's value. On
   a write, fcoe_sysfs will call the LLD (if there is a
   callback) to notifiy that the enalbed state has changed.

This patch maintains the old FCoE control interfaces as
module parameters, but it adds comments pointing out that
the old interfaces are deprecated.

Bhanu Prakash Gollapudi 5c17ae217a [SCSI] bnx2fc: Improve error recovery by handling parity errors
During parity errors, the ramrods are not issued to FW. bnx2fc waits for the
timeout value, and proceeds with cleaning up the IOs. Since we are already
out-of-sync with FW, cleanup commands timeout too, and do not get the
completion.  This operation takes 36 secs for each session to upload causing
huge delays. To fix this, bnx2fc now gets a PARITY_ERROR from cnic driver, and
upon failure, the driver does not issue any commands to the FW and finishes the
upload process sooner.

Bhanu Prakash Gollapudi 7adc5a3746 [SCSI] bnx2fc: Support interface creation on non-VLAN interface also.
bnx2fc had an assumption that the fcoe interface will always start on the vlan
dev. However, some switch implementations (Eg., HP virtual connect FlexFabric)
expects the fcoe interface to be started on physical interface. Do not error
out if the netdev is not a vlan dev.

This patch has the SW FCoE driver and the bnx2fc
driver make use of the new fcoe_sysfs API added
earlier in this patch series.

After this patch a fcoe_ctlr_device is allocated with
private data in this order.

libfcoe also takes part in this new model since it
discovers and manages fcoe_fcf instances. The memory
allocation is different for FCFs. I didn't want to
impact libfcoe's fcoe_fcf processing, so this patch
creates fcoe_fcf_device instances for each discovered
fcoe_fcf. The two are paired using a (void * priv)
member of the fcoe_ctlr_device. This allows libfcoe
to continue maintaining its list of fcoe_fcf instances
and simply attaches and detaches them from existing
or new fcoe_fcf_device instances.

Robert Love fd8f89027d [SCSI] bnx2fc: Allocate fcoe_ctlr with bnx2fc_interface, not as a member
Currently the fcoe_ctlr associated with an interface is allocated
    as a member of struct bnx2fc_interface. This causes problems when
    when later patches attempt to use the new fcoe_sysfs APIs which
    allow us to allocate the bnx2fc_interface as private data to a
    fcoe_ctlr_device instance. The problem is that libfcoe wants to
    be able use pointer math to find a fcoe_ctlr's fcoe_ctlr_device
    as well as finding a fcoe_ctlr_device's assocated fcoe_ctlr. To
    do this we need to allocate the fcoe_ctlr_device, with private
    data for the LLD. The private data will contain the fcoe_ctlr
    and its private data will be the bnx2fc_interface.

Michael Chan 8ec3e70207 cnic: Fix parity error code conflict
The recently added parity error handling used an error code that was
already defined for a different error.  This could lead to bnx2x
firmware assert.  We need to fix this with new error codes that are

When there are 255 NPIV ports, and the interface is brought down & up, both
physical and NPIV ports are logged off and never logged back in. Since
discovery happens on single CPU, XID resources on that CPU will be limited,
which when exhausted the discovery fails. Increase the XID resource range to
ensure that the discovery completes successfully. Also ensure that
fc_exch_mgr_alloc() doesn't fail on the system that has lower number of CPUs.

Bhanu Prakash Gollapudi 32c3045450 [SCSI] bnx2fc: Handle SRR LS_ACC drop scenario
When SRR LS_ACC is dropped, the driver was not issuing ABTS for SRR when it
times out. Since the target received SRR, it was able to send the XFER_RDY and
the the original IO request completed successfully. In this condition ABTS was
not sent during bnx2fc_srr_compl(). Fix this by first checking for ELS timeout
and issue ABTS before checking if original IO request is complete.

If the IO and the corresponding ABTS are not responded by a target, cleanup the
IO and issue explicit logout when ulp timer expires while waiting for ABTS to
complete. Wait for the session to be ready before returning to the SCSI layer.
If the session is not ready let the SCSI-ml escalate the error recovery.

Link may not be up when the driver receives ulp_start event, and hence
fcoe_ctlr_link_up is not called. Call fcoe_ctlr_link_up during
indicate_netevent only when the interface is enabled. (It has to be called when
enabled because that is an indication that the vlan discovery is completed).

deadlock as bnx2fc_if_destroy calls fc_remove_host holding rtnl_lock.

Link up event is generated to the driver even before vlan discovery has
started. Because of this driver can send discovery solicitation on a stale
vlan. Call fcoe_ctlr_link_up() only when the driver is in enabled state, which
implies the vlan discovery is complete before sending solicitation.

Jiri Pirko 4bc71cb983 net: consolidate and fix ethtool_ops->get_settings calling
This patch does several things:
- introduces __ethtool_get_settings which is called from ethtool code and
  from drivers as well. Put ASSERT_RTNL there.
- dev_ethtool_get_settings() is replaced by __ethtool_get_settings()
- changes calling in drivers so rtnl locking is respected. In
  iboe_get_rate was previously ->get_settings() called unlocked. This
  fixes it. Also prb_calc_retire_blk_tmo() in af_packet.c had the same
  problem. Also fixed by calling __dev_get_by_index() instead of
  dev_get_by_index() and holding rtnl_lock for both calls.
- introduces rtnl_lock in bnx2fc_vport_create() and fcoe_vport_create()
  so bnx2fc_if_create() and fcoe_if_create() are called locked as they
  are from other places.
- use __ethtool_get_settings() in bonding code

Also, do not count uncolicited CQ completions for free_sqes.

During the unsolicited LOGO processing, the session is uploaded and offloaded
after the relogin is complete. In between any new IOs are errored back as the

When NPIV ports are created/deleted rapidly there is a race condition between
bnx2fc_vport_destroy() from sysfs and bnx2fc_flogi_resp(), which could try to

NPIV ports to find a match, and only when it exists remove it.

For the devices that support sequence level error recovery, based on the REC
response, the firmware has to be informed about the offset from which the
retransmission should happen. Driver initiates sequence cleanup task to
firmware so that the firmware can program the task. Upon the sequence cleanup
completion, SRR is issued to retransmit the sequence.

Based on PRLI response, identify if the target is FCP-2 (seq level error
recovery) capable, and appropriately set the corresponding CONF, REC flags when
offloading the session.

Bhanu Prakash Gollapudi aea71a0249 [SCSI] bnx2fc: Introduce interface structure for each vlan interface
Currently, bnx2fc has a hba structure that can work with only a single vlan
interface.  When there is a change in vlan id, it does not have the capability
to switch to different vlan interface. To solve this problem, a new structure
called 'interface' has been introduced, and each hba can now have multiple
interfaces, one per vlan id.

Most of the patch is a moving the interface specific fields from hba to the
interface structure, and appropriately modifying the dereferences. A list of
interfaces (if_list) is maintained along with adapter list. During a create
call, the interface structure is allocated and added to if_list and deleted &
freed on a destroy call.  Link events are propagated to all interfaces
belonging to the hba.

Bhanu Prakash Gollapudi d36b3279e1 [SCSI] bnx2fc: Fix kernel panic when deleting NPIV ports
Deleting NPIV port causes a kernel panic when the NPIV port is in the same zone
as the physical port and shares the same LUN. This happens due to the fact that
vport destroy and unsolicited ELS are scheduled to run on the same workqueue,
and vport destroy destroys the lport and the unsolicited ELS tries to access
the invalid lport.  This patch fixes this issue by maintaining a list of valid
lports and verifying if the lport is valid or not before accessing it.

New FW/HSI (7.0):
 - Added support to 578xx chips
 - Improved HSI - much less driver's direct access to the FW internal
   memory needed.

New implementation of the HSI handling layer in the bnx2x (bnx2x_sp.c):
 - Introduced chip dependent objects that have chip independent interfaces
   for configuration of MACs, multicast addresses, Rx mode, indirection table,
   fast path queues and function initialization/cleanup.
 - Objects functionality is based on the private function pointers, which
   allows not only a per-chip but also PF/VF differentiation while still
   preserving the same interface towards the driver.
 - Objects interface is not influenced by the HSI changes which do not require
   providing new parameters keeping the code outside the bnx2x_sp.c invariant
   with regard to such HSI chnages.

Changes in a CNIC, bnx2fc and bnx2i modules due to the new HSI.
