6ca9f3bae8 cifs: pass instantiated filp back after open call
The current scheme of sticking open files on a list and assuming that
cifs_open will scoop them off of it is broken and leads to "Busy
inodes after umount..." errors at unmount time.

The problem is that there is no guarantee that cifs_open will always
be called after a ->lookup or ->create operation. If there are
permissions or other problems, then it's quite likely that it *won't*
be called.

Fix this by fully instantiating the filp whenever the file is created
and pass that filp back to the VFS. If there is a problem, the VFS
can clean up the references.

2422f676fb cifs: move cifs_new_fileinfo call out of cifs_posix_open
Having cifs_posix_open call cifs_new_fileinfo is problematic and
inconsistent with how "regular" opens work. It's also buggy as
cifs_reopen_file calls this function on a reconnect, which creates a new
struct cifsFileInfo that just gets leaked.

Push it out into the callers. This also allows us to get rid of the
"mnt" arg to cifs_posix_open.

Finally, in the event that a cifsFileInfo isn't or can't be created, we
always want to close the filehandle out on the server as the client
won't have a record of the filehandle and can't actually use it. Make
sure that CIFSSMBClose is called in those cases.

12420ac341 cifs: implement drop_inode superblock op
The standard behavior for drop_inode is to delete the inode when the
last reference to it is put and the nlink count goes to 0. This helps
keep inodes that are still considered "not deleted" in cache as long as
possible even when there aren't dentries attached to them.

When server inode numbers are disabled, it's not possible for cifs_iget
to ever match an existing inode (since inode numbers are generated via
iunique). In this situation, cifs can keep a lot of inodes in cache that
will never be used again.

Implement a drop_inode routine that deletes the inode if server inode
numbers are disabled on the mount. This helps keep the cifs inode
caches down to a more manageable size when server inode numbers are
disabled.

06b43672a9 cifs: fix page refcount leak
Commit 315e995c63 is causing OOM kills
when stress-testing a CIFS filesystem. The VFS readpages operation takes
a page reference. The older code just handed this reference off to the
page cache, but the new code takes an extra one. The simplest fix is to
put the new reference after add_to_page_cache_lru.

84f30c66c3 cifs: don't update uniqueid in cifs_fattr_to_inode
We use this value to find an inode within the hash bucket, so we can't
change this without re-hashing the inode. For now, treat this value
as immutable.

Eventually, we should probably use an inode number change on a path
based operation to indicate that the lookup cache is invalid, but that's
a bit more code to deal with.

3d69438031 cifs: guard against hardlinking directories
When we made serverino the default, we trusted that the field sent by the
server in the "uniqueid" field was actually unique. It turns out that it
isn't reliably so.

Samba, in particular, will just put the st_ino in the uniqueid field when
unix extensions are enabled. When a share spans multiple filesystems, it's
quite possible that there will be collisions. This is a server bug, but
when the inodes in question are a directory (as is often the case) and
there is a collision with the root inode of the mount, the result is a
kernel panic on umount.

Fix this by checking explicitly for directory inodes with the same
uniqueid. If that is the case, then we can assume that using server inode
numbers will be a problem and that they should be disabled.

2c964d1f7c [CIFS] Fix losing locks during fork()
When process does fork() private_data of files with lock list stays the same

6513a81e93 cifs: Fix a kernel BUG with remote OS/2 server (try #3)
While chasing a bug report involving a OS/2 server, I noticed the server sets
pSMBr->CountHigh to a incorrect value even in case of normal writes. This
results in 'nbytes' being computed wrongly and triggers a kernel BUG at
mm/filemap.c.

void iov_iter_advance(struct iov_iter *i, size_t bytes)
{
        BUG_ON(i->count < bytes);    <--- BUG here

Why the server is setting 'CountHigh' is not clear but only does so after
writing 64k bytes. Though this looks like the server bug, the client side
crash may not be acceptable.

The workaround is to mask off high 16 bits if the number of bytes written as
returned by the server is greater than the bytes requested by the client as
suggested by Jeff Layton.

5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

  http://userweb.kernel.org/~tj/misc/slabh-sweep.py

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
   files.

2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

bcd5357f43 cifs: add a CIFSSMBQFileInfo function
...to get inode attributes via filehandle instead of by path.

In some places, we need to revalidate an inode on an open filehandle,
but we can't necessarily guarantee that the dentry associated with it
will still be valid. When we have an open filehandle already, it makes
more sense to do a filehandle based operation anyway.

df2cf170c8 cifs: overhaul cifs_revalidate and rename to cifs_revalidate_dentry
cifs_revalidate is renamed to cifs_revalidate_dentry as a later patch
will add a by-filehandle variant.

Add a new "invalid_mapping" flag to the cifsInodeInfo that indicates
that the pagecache is considered invalid. Add a new routine to check
inode attributes whenever they're updated and set that flag if the inode
has changed on the server.

cifs_revalidate_dentry is then changed to just update the attrcache if
needed and then to zap the pagecache if it's not valid.

There are some other behavior changes in here as well. Open files are
now allowed to have their caches invalidated. I see no reason why we'd
want to keep stale data around just because a file is open. Also,
cifs_revalidate_cache uses the server_eof for revalidating the file
size since that should more closely match the size of the file on the
server.

cifs_from_ucs2 returns the length of the converted name, including the
length of the NULL terminator. We don't want to include the NULL
terminator in the dentry name length however since that'll throw off the
hash calculation for the dentry cache.

I believe that this is the root cause of several problems that have
cropped up recently that seem to be papered over with the "noserverino"
mount option. More confirmation of that would be good, but this is
clearly a bug and it fixes at least one reproducible problem that
was reported.

6b2f3d1f76 vfs: Implement proper O_SYNC semantics
While Linux provided an O_SYNC flag basically since day 1, it took until
Linux 2.4.0-test12pre2 to actually get it implemented for filesystems,
since that day we had generic_osync_around with only minor changes and the
great "For now, when the user asks for O_SYNC, we'll actually give
O_DSYNC" comment. This patch intends to actually give us real O_SYNC
semantics in addition to the O_DSYNC semantics. After Jan's O_SYNC
patches which are required before this patch it's actually surprisingly
simple, we just need to figure out when to set the datasync flag to
vfs_fsync_range and when not.

This patch renames the existing O_SYNC flag to O_DSYNC while keeping it's
numerical value to keep binary compatibility, and adds a new real O_SYNC
flag. To guarantee backwards compatiblity it is defined as expanding to
both the O_DSYNC and the new additional binary flag (__O_SYNC) to make
sure we are backwards-compatible when compiled against the new headers.

This also means that all places that don't care about the differences can
just check O_DSYNC and get the right behaviour for O_SYNC, too - only
places that actuall care need to check __O_SYNC in addition. Drivers and
network filesystems have been updated in a fail safe way to always do the
full sync magic if O_DSYNC is set. The few places setting O_SYNC for
lower layers are kept that way for now to stay failsafe.

We enforce that O_DSYNC is set when __O_SYNC is set early in the open path
to make sure we always get these sane options.

Note that parisc really screwed up their headers as they already define a
O_DSYNC that has always been a no-op. We try to repair it by using it for
the new O_DSYNC and redefinining O_SYNC to send both the traditional
O_SYNC numerical value _and_ the O_DSYNC one.

a2934c7b36 cifs: NULL out tcon, pSesInfo, and srvTcp pointers when chasing DFS referrals
The scenario is this:

The kernel gets EREMOTE and starts chasing a DFS referral at mount time.
The tcon reference is put, which puts the session reference too, but
neither pointer is zeroed out.

The mount gets retried (goto try_mount_again) with new mount info.
Session setup fails fails and rc ends up being non-zero. The code then
falls through to the end and tries to put the previously freed tcon
pointer again. Oops at: cifs_put_smb_ses+0x14/0xd0

Fix this by moving the initialization of the rc variable and the tcon,
pSesInfo and srvTcp pointers below the try_mount_again label. Also, add
a FreeXid() before the goto to prevent xid "leaks".

cea6234395 [CIFS] Duplicate data on appending to some Samba servers
SMB writes are sent with a starting offset and length. When the server
supports the newer SMB trans2 posix open (rather than using the SMB
NTCreateX) a file can be opened with SMB_O_APPEND flag, and for that
case Samba server assumes that the offset sent in SMBWriteX is unneeded
since the write should go to the end of the file - which can cause
problems if the write was cached (since the beginning part of a
page could be written twice by the client mm). Jeff suggested that
masking the flag on posix open on the client is easiest for the time
being. Note that recent Samba server also had an unrelated problem with
SMB NTCreateX and append (see samba bugzilla bug number 6898) which
should not affect current Linux clients (unless cifs Unix Extensions
are disabled).

The cifs client did not send the O_APPEND flag on posix open
before 2.6.29 so the fix is unneeded on early kernels.

In the future, for the non-cached case (O_DIRECT, and forcedirectio mounts)
it would be possible and useful to send O_APPEND on posix open (for Windows
case: FILE_APPEND_DATA but not FILE_WRITE_DATA on SMB NTCreateX) but for
cached writes although the vfs sets the offset to end of file it
may fragment a write across pages - so we can't send O_APPEND on
open (could result in sending part of a page twice).

that translates to EOPNOTSUPP.

Rather than trying to be clever with that call, just have

ec06aedd44 cifs: clean up handling when server doesn't consistently support inode numbers
It's possible that a server will return a valid FileID when we query the
FILE_INTERNAL_INFO for the root inode, but then zeroed out inode numbers
when we do a FindFile with an infolevel of
SMB_FIND_FILE_ID_FULL_DIR_INFO.

In this situation turn off querying for server inode numbers, generate a
warning for the user and just generate an inode number using iunique.
Once we generate any inode number with iunique we can no longer use any
server inode numbers or we risk collisions, so ensure that we don't do
that in cifs_get_inode_info either.

The reason of this bug is that the error handling code of
cifs_get_tcp_session()
calls kfree() when corresponding kmalloc() failed.
(The kmalloc() is called by extract_hostname().)

3321b791b2 cifs: fix locking and list handling code in cifs_open and its helper
The patch to remove cifs_init_private introduced a locking imbalance. It
didn't remove the leftover list addition code and the unlocking in that
function. cifs_new_fileinfo does the list addition now, so there should
be no need to do it outside of that function.

pCifsInode will never be NULL, so we don't need to check for that. This
patch also gets rid of the ugly locking and unlocking across function
calls.

...it does the same thing as cifs_fill_fileinfo, but doesn't handle the
flist ordering correctly. Also rename cifs_fill_fileinfo to a more
descriptive name and have it take an open flags arg instead of just a
write_only flag. That makes the logic in the callers a little simpler.

3bc303c254 cifs: convert oplock breaks to use slow_work facility (try #4)
This is the fourth respin of the patch to convert oplock breaks to
use the slow_work facility.

A customer of ours was testing a backport of one of the earlier
patchsets, and hit a "Busy inodes after umount..." problem. An oplock
break job had raced with a umount, and the superblock got torn down and
its memory reused. When the oplock break job tried to dereference the
inode->i_sb, the kernel oopsed.

This patchset has the oplock break job hold an inode and vfsmount
reference until the oplock break completes. With this, there should be
no need to take a tcon reference (the vfsmount implicitly holds one
already).

Currently, when an oplock break comes in there's a chance that the
oplock break job won't occur if the allocation of the oplock_q_entry
fails. There are also some rather nasty races in the allocation and
handling these structs.

Rather than allocating oplock queue entries when an oplock break comes
in, add a few extra fields to the cifsFileInfo struct. Get rid of the
dedicated cifs_oplock_thread as well and queue the oplock break job to
the slow_work thread pool.

This approach also has the advantage that the oplock break jobs can
potentially run in parallel rather than be serialized like they are
today.
