This patch converts the drivers in drivers/hwmon/* to use the
module_platform_driver() macro which makes the code smaller and a bit
simpler.

channel configuration name changed.

Avoid registering channels that have zero divider settings in them, as this
will only lead to kernel OOPS from divide-by-zero when the sysfs entry is
read.
