Originally Xen PV drivers only use single-page ring to pass along
information. This might limit the throughput between frontend and
backend.
The patch extends Xenbus driver to support multi-page ring, which in
general should improve throughput if ring is the bottleneck. Changes to
various frontend / backend to adapt to the new interface are also
included.

Instead of calling target_fabric_configfs_init() +
target_fabric_configfs_register() / target_fabric_configfs_deregister()
target_fabric_configfs_free() from every target driver, rewrite the API
so that we have simple register/unregister functions that operate on
a const operations vector.
This patch also fixes a memory leak in several target drivers. Several
target drivers namely called target_fabric_configfs_deregister()
without calling target_fabric_configfs_free().

Another release, another set of regulator updates. Not much of it is
showing up in the code yet but there's been a lot of discussion going on
about how to enhance the regulator API to work better with modern SoCs
which have a microcontroller sitting between Linux and the hardware.
I'm hopeful that'll start to come through into mainline for v4.2 but
it's not quite there for v4.1 - what we do have (along with the usual
small updates is) is:
- Work from Bjorn Andersson on refactoring the configuration of
regulator loading interfaces to be useful for use with
microcontrollers, the existing interfaces were never actually useful
for anything as-is since nobody was willing to put enough data into
public code.

overflow in the calculation of total number of pages in bio_map_user_iov() -
we rely on there being no more than PAGE_SIZE members of sum in the
first loop there. If that sum wraps around, we end up allocating

The current string_get_size() overflows when the device size goes over
2^64 bytes because the string helper routine computes the suffix from
the size in bytes. However, the entirety of Linux thinks in terms of
blocks, not bytes, so this will artificially induce an overflow on very
large devices. Fix this by making the function string_get_size() take
blocks and the block size instead of bytes. This should allow us to
keep working until the current SCSI standard overflows.
Also fix virtio_blk and mmc (both of which were also artificially
multiplying by the block size to pass a byte side to string_get_size()).
The mathematics of this is pretty simple: we're taking a product of
size in blocks (S) and block size (B) and trying to re-express this in
exponential form: S*B = R*N^E (where N, the exponent is either 1000 or
1024) and R < N. Mathematically, S = RS*N^ES and B=RB*N^EB, so if RS*RB
< N it's easy to see that S*B = RS*RB*N^(ES+EB). However, if RS*BS > N,
we can see that this can be re-expressed as RS*BS = R*N (where R =
RS*BS/N < N) so the whole exponent becomes R*N^(ES+EB+1)

Some implementation of UFS host controller HW might have some non-standard
behaviours (quirks) when compared to behaviour specified by UFSHCI
specification. This patch add support to allow specifying all such quirks
to standard UFS host controller driver so standard driver takes them into
account.
In this change a UFSHCD_QUIRK_DELAY_BEFORE_DME_CMDS is introduced,
where a minimum delay of 1ms is required before DME commands for
stability purposes.

Sometimes, specific information about the UFS controller revision is
required in order to determine certain operations or execute
controller dependent quirks.
In order to avoid reading the controller revision multiple times,
we simply read it once and save this information in internal structure.

For FA-WWPN is enabled port, if NPIV created on that port and,
if port link is brought down, then WWPN was restored from flash for both
physical and NPIV port. This will result in NPIV port and physical port
sharing same WWPN. Any application refreshing ports information will
not be able to scan NPIV port because of this behavior. So while restoring WWPN,
only restore physical port WWPN.

On some vendor switches, when switch port is toggled (down /up),
then in some condition driver tries to configure virtual port,
before FW is actually in ready state to process any commands on wire.

Currently, the driver plays off the fact that older sli4 adapters have a
different rpi access pattern that allowed for the rpi reference to be
released earlier in the teardown sequence, allowing the driver to recycle
the rpi value sooner. Newer sli4 adapters have a different access pattern that
requires us to wait for a later mailbox completion. This changes the put
call location on the newer sli4 adapters.
Symptoms of the error are "0110 ELS" and the "0372 iotag" errors.

Fix host reset escalation killing all IOs.
SLI-3 adapters will use a new host template. The template differs
from SLI-4 adapters in that it does not have an eh_host_reset_handler.
Lpfc has traditionally never had a host_reset. The host reset
handler was added when we ran into a stuck hardware condition on a
SLI-4 adapter. The host_reset will reset and reinit the pci function,
clearing the hardware condition.
Unfortunately, the host reset handler uses attach/detach code paths,
which makes scsi_add_host() and scsi_remove_host() calls. Meaning, a
host_reset will completely remove the scsi_host from the system. As a
new call to scsi_add_host() is made, the shost# changes, which results
in completely new scsi_devices and device names. All the older scsi
devices on the old shost# are now orphaned and unrecoverable.
We realize we need to re-implement the host_reset_handler so the scsi_host
stays registered across the host_reset, but that will be a rather
lengthy effort. In the short term, we had an immediate need to restore
the SLI-3 devices to their working behavior, with the easiest path being
to remove their host_reset handler.

Bus reset always fails because aha1542_reset waits for the controller to assert
the INIT bit (Mailbox Initialization Required) which it never does. This bit is
asserted only after host reset.
Remove the requirement for INIT bit (we really need only the IDLE bit).

Remove aha1542_lock and use host_lock instead.
Remove interrupt and queuecommand function wrappers.
Remove locking from lowlevel _out and _in functions, they now can
onle be called (at runtime) with host_lock being held.
Remove ssleep(4) in aha1542_reset as we can't sleep while holding a spinlock.
It's useless anyway as wait_mask will wait until the controller is idle and
kernel waits for 10 seconds (HOST_RESET_SETTLE_TIME) after that.

After getting the platform shutdown command "VM_CloseAll" response from the
firmware, driver was getting configuration IOCTL request from the upper layers
and it sends down to firmware. This causes firmware assert issue.
This patch fixes the firmware assert issue. During the shutdown, if driver
gets commands from the upper layer, driver sends error code to the upper
layers.

This patch fixes the IOP_RESET issue. Sending IOP_RESET command need to wait
for only 10 sec instead of 5 minutes in case of firmware does not response
IOP_RESET command. Disable interrupt before setup interrupt routine to
prevent spurious interrupts.

The Linux aacriad driver fails to detect the case of SG list count=0 on IOCTL
pass-through command and cause intermittent fault. The result is the Linux
aacriad driver send down IOCTL pass-through command with one not initialized
SG list to firmware when receiving SG list count =0 on pass-through command.

Currently when performing a reboot with an ipr adapter,
the adapter gets shutdown completely, flushing all write
cache, as well as performing a full hardware reset of the card
during the shutdown phase of the old kernel. This ensures
the adapter is in a fully quiesced state across the reboot.
There are scenarios, however, such as when performing
kexec, where this full adapter shutdown is not required
and not desired, since it can make the reboot process take
noticeably longer.
This patch adds a module parameter to allow for skipping the
full shutdown during reboot. Rather than performing a full
adapter shutdown and reset, we simply cancel any outstanding
error buffers, place the adapter into a state where it has no
memory of any DMA addresses from the old kernel, then disable
the device. This significantly speeds up kexec boot, particularly
in configurations with multiple ipr adapters.

SCSI transport drivers and SCSI LLDs block a SCSI device if the
transport layer is not operational. This means that in this state
no requests should be processed, even if the REQ_PREEMPT flag has

This patch updates qla2xxx target to add a new fabric_prot_type TPG
attribute, used for controlling LLD level protection into LIO when
the backend device does not support T10-PI.
This is required for qla_target.c to enable WRITE_STRIP + READ_INSERT
hardware offloads.
It's disabled by default and controls which se_sesion->sess_prot_type
are set at tcm_qla2xxx_check_initiator_node_acl() session registration
time.

READ_ONCE() to the socket cached RX route read, whilst in 'net-next'
Eric Dumazet touched the surrounding code dealing with how mini
sockets are handled.

and then I cherry picked it back into net.

ppc has special instruction forms to efficiently load and store values
in non-native endianness. These can be accessed via the arch-specific
{ld,st}_le{16,32}() inlines in arch/powerpc/include/asm/swab.h.
However, gcc is perfectly capable of generating the byte-reversing
load/store instructions when using the normal, generic cpu_to_le*() and
le*_to_cpu() functions eaning the arch-specific functions don't have much
point.
Worse the "le" in the names of the arch specific functions is now
misleading, because they always generate byte-reversing forms, but some
ppc machines can now run a little-endian kernel.
To start getting rid of the arch-specific forms, this patch removes them
from all the old Power Macintosh drivers, replacing them with the
generic byteswappers.

The be_main.c conflict resolution was really tricky. The conflict
hunks generated by GIT were very unhelpful, to say the least. It
split functions in half and moved them around, when the real actual
conflict only existed solely inside of one function, that being
be_map_pci_bars().
So instead, to resolve this, I checked out be_main.c from the top
of net-next, then I applied the be_main.c changes from 'net' since
the last time I merged. And this worked beautifully.
The inet_diag.c and sysctl_net_core.c conflicts were simple
overlapping changes, and were easily to resolve.

This patch fixes the incorrect use of __transport_register_session()
in tcm_qla2xxx_check_initiator_node_acl() code, that does not perform
explicit se_tpg->session_lock when accessing se_tpg->tpg_sess_list
to add new se_sess nodes.
Given that tcm_qla2xxx_check_initiator_node_acl() is not called with
qla_hw->hardware_lock held for all accesses of ->tpg_sess_list, the
code should be using transport_register_session() instead.

SAS controller has its own tag allocation, which doesn't directly match to ATA
tag, so SAS and SATA have different code path for ata tags. Originally we use
port->scsi_host (98bd4be1) to destinguish SAS controller, but libsas set
->scsi_host too, so we can't use it for the destinguish, we add a new flag for
this purpose.
Without this patch, the following oops can happen because scsi-mq uses
a host-wide tag map shared among all devices with some integer tag
values >= ATA_MAX_QUEUE. These unexpectedly high tag values cause
__ata_qc_from_tag() to return NULL, which is then dereferenced in
ata_qc_new_init().

This effectively reverts commits 85b6c7 ("[SCSI] sd: fix cache flushing on
module removal (and individual device removal)" and dc4515ea ("scsi: always
increment reference count").
We now never call scsi_device_get from the shutdown path, and the fact
that we started grabbing reference there in commit 85b6c7 turned out
turned out to create more problems than it solves, and required

Up to now the pvscsi frontend hasn't supported domain suspend and
resume. When a domain with an assigned pvscsi device was suspended
and resumed again, it was not able to use the device any more: trying
to do so resulted in hanging processes.
Support suspend and resume of pvscsi devices.

This may be OK in archs with contiguous CPU numbers and without
hotplug CPUs, but it sets a terrible example.
And open-coding it like drivers/scsi/hpsa.c is just weird.
BTRFS has a weird comparison with num_online_cpus() too, but since
BTRFS just screwed up my test machines' root partition, I'm not
touching it :)

Instances of var * HZ / 1000 are replaced by msecs_to_jiffies(var).
In addition some timing constants that assumed HZ 100 were adjusted
to HZ independent settings based on review comments from Michael Schmitz

This patch exposes a new ../qla2xxx*/$WWPN/$TPGT/dynamic_sessions
attribute to dump the currently active sessions by FC initiator WWPN
that have been created with dynamically generated se_node_acls.
This information is useful so that user-space can optionally perform
dynamic -> explicit NodeACL conversion based on $INITIATOR_WWPN.

With scsi-mq enabled, userspace programs can get unexpected EWOULDBLOCK
(a.k.a. EAGAIN) errors when submitting commands to the SCSI generic
driver. Fix by calling blk_get_request() with GFP_KERNEL instead of
GFP_ATOMIC.
Note: to avoid introducing a potential deadlock, this patch should be
applied after the patch titled "sg: fix unkillable I/O wait deadlock
with scsi-mq".

When using the write()/read() interface for submitting commands, the
SCSI generic driver does not call blk_put_request() on a completed SCSI
command until userspace calls read() to get the command completion.
Since scsi-mq uses a fixed number of preallocated requests, this makes
it possible for userspace to exhaust the entire preallocated supply of
requests. For places in the kernel that call blk_get_request() with
GFP_KERNEL, this can cause the calling process to deadlock in a
permanent unkillable I/O wait in blk_get_request() -> ... -> bt_get().
For places in the kernel that call blk_get_request() with GFP_ATOMIC,
this can cause blk_get_request() always to return -EWOULDBLOCK. Note
that these problems happen only if scsi-mq is enabled. Prevent the
problems by calling blk_put_request() as soon as the SCSI command
completes instead of waiting for userspace to call read().

printk and friends can now format bitmaps using '%*pb[l]'. cpumask
and nodemask also provide cpumask_pr_args() and nodemask_pr_args()
respectively which can be used to generate the two printf arguments
necessary to format the specified cpu/nodemask.
* map_show()'s return value is too high by one and the function could
modify beyond the end of the buffer when the formatted text is long
enough.

The net/core/dev.c conflict was the overlap of one commit marking an
existing function static whilst another was adding a new function.
In the include/linux/if_vlan.h case, the type used for a local
variable was changed in 'net', whereas the function got rewritten
to fix a stacked vlan bug in 'net-next'.
In drivers/vhost/net.c, Al Viro's iov_iter conversions in 'net-next'
overlapped with an endainness fix for VHOST 1.0 in 'net'.
In drivers/net/vxlan.c, vxlan_find_vni() added a 'flags' parameter
in 'net-next' whereas in 'net' there was a bug fix to pass in the
correct network namespace pointer in calls to this function.

This patch set is fixing two serious problems which have turned up late in the
release cycle. The first fixes a problem with 4k sector disks where the
transfer length (amount of data sent to the disk) was getting increased every
time the disk was revalidated leading to potential for overflows. The other
is a regression oops fix for some of our last merge window code.

Make use of a new interface provided by iov_iter, backed by
scatter-gather list of iovec, instead of the old interface based on
sg_iovec. Also use iov_iter_advance() instead of manual iteration.

Detect failues when attempting to change controller to use simple
or performant transport modes (mode change ack) rather than just
proceeding ahead after timeouts.
Return values are added to:
hpsa_put_ctlr_into_performant_mode

and all their callers check/propagate the result.

Shorten the wait for the CISS configuration table doorbell mode
change acknowledgment from 300-600 s to 20 s, which is the value
specified in the CISS specification that should be honored by
all controllers.
Wait using interruptible msleep() rather than uninterruptible
usleep_range(), which triggers rt_sched timeout errors if the
wait is long.

Move the code which sets up the SG descriptor out of hpsa_scatter_gather()
and into a subroutine where it can be reused (in the next patch). The Ext
field is now assigned unconditionally: this makes the refactor much simpler,
but more importantly it removes a conditional operation from inside the
loop. The case for which the conditional formerly tested is now executed
(unconditionally) after the loop is exited.

In the code that translates logical drive LBAs to physical
drive LBAs if we overflow the raid map disk data array we
will get the wrong answers. We do not expect that to happen,
but best to be on the safe side and guard against it anyway.

In set_encrypt_ioaccel2() and in hpsa_scsi_ioaccel_raid_map
there were BUG_ONs that looked like this:

This means changing the allocator to reference count commands.
The reference count is now the authoritative indicator of whether a
command is allocated or not. The h->cmd_pool_bits bitmap is now
only a heuristic hint to speed up the allocation process, it is no
longer the authoritative record of allocated commands.
Since we changed the command allocator to use reference counting
as the authoritative indicator of whether a command is allocated,
fail_all_outstanding_cmds needs to use the reference count not
h->cmd_pool_bits for this purpose.
Fix hpsa_drain_accel_commands to use the reference count as the
authoritative indicator of whether a command is allocated instead of
the h->cmd_pool_bits bitmap.

When using the ioaccel submission methods, requests destined for RAID volumes
are sometimes diverted to physical devices. The OS has no or limited
knowledge of these physical devices, so it is up to the driver to avoid
pushing the device too hard. It is better to honor the physical device queue
limit rather than making the device spew zillions of TASK SET FULL responses.
This is so that hpsa based devices support /sys/block/sdNN/device/queue_type
of simple, which lets the SCSI midlayer automatically adjust the queue_depth
based on TASK SET FULL and GOOD status.
Adjust the queue depth for a new device after it is created based on the
maximum queue depths of the physical devices that constitute the
device. This drops the maximum queue depth from .can_queue of 1024 to
something like 174 for single-drive RAID-0, 348 for two-drive RAID-1, etc.
It also adjusts for the ratio of data to parity drives.

Instead of kicking the commands all the way back to the mid
layer, use a work queue. This enables having a mechanism for
the driver to be able to resubmit the commands down the "normal"
raid path without turning off the ioaccel feature entirely
whenever an error is encountered on the ioaccel path, and
prevent excessive rescanning of devices.

Factor out the bottom part of the queuecommand function
which is the part that builds commands for submitting down
the "normal' RAID stack path of a Smart Array.
Need to factor this out to improve how commands that
were initially sent down one of the "ioaccellerated"
paths but which have some sort of error condition are
retried down the "normal" path.

The original reasoning behind doing this was faulty. An error
of some sort would be encountered, accelerated i/o would be
disabled for that logical drive, the command would be kicked
back out to the SCSI midlayer for a retry, and since i/o accelerator
mode was disabled, it would get retried down the RAID path.
However, something needs to turn ioaccellerator mode back on,
and this rescan request was what did that. However, it was racy,
and extremely bad for performance to rescan all devices, so,
don't do that.

By not doing maintaining a list of queued commands, we can eliminate some spin
locking in the main i/o path and gain significant improvement in IOPS. Remove
the queuing code and the code that calls it; remove now-unused interrupt code;
remove DIRECT_LOOKUP_BIT.
Now that the passthru commands share the same command pool as
the main i/o path, and the total size of the pool is less than
or equal to the number of commands that will fit in the hardware
fifo, there is no need to check to see if we are exceeding the
hardware fifo's depth.

We need to reserve some commands for device rescans,
aborts, and the pass through ioctls, etc. so we cannot
give them all to the scsi mid layer.
This is in preparation for removing cmd_special_alloc and
cmd_special_free so that we can stop queuing commands internally
in the driver so that we can remove the locks thta protect the
queue that we will no longer have.

If hpsa_allocate_cmd_pool failed, we were calling two functions unnecessarily:
hpsa_free_sg_chain_blocks(h);
hpsa_free_cmd_pool(h);
This didn't cause any problem, as those functions can tolerate being called
when what they free hasn't been allocated (relevant pointers would be NULL)
but it is potentially confusing.

If the board ID lookup function fails, return the return
code rather than return -ENODEV.
The only board ID failure reason right now is -ENODEV,
so this just provides more informative prints in kdump
and adapts to future changes.
Tested with error injection while booting with
reset_devices
on the kernel command line:

In MSI and MSI-X mode, where hpsa asks for more than one interrupt,
hpsa_request_irqs forgets if the first request_irq call failed
if later ones succeed.
It needs to exit the loop on any failure rather than continue,
freeing all irqs that were requested until that point.
Also, it needs to clear out the q numbers up to MAX_REPLY_QUEUES.
The same is true for the general hpsa_free_irqs function.
Tested with error injection of -ENOSYS on the 4th call:

HP now uses RAID-6 rather than RAID-ADG (Advanced Data Guarding)
as the marketing name for our implementation of RAID-6.
The driver considers RAID-1 and RAID-1+0 to be the same level, and
considers RAID-1ADM and RAID-1+0ADM to be the same level. Parenthesis
can be used to reflect the optional +0 portion of both those RAID levels.
Rename: RAID-ADG to RAID-6
RAID-1(1+0) to RAID-1(+0)
RAID-1(ADM) to RAID-1(+0)ADM
Also, add another const after the pointer type as suggested
by checkpatch.pl so the array is:

Change how SA controllers are reset by changing PCI power levels.
The hpsa driver was finding the PCI_PM_CTRL_STATE_MASK offset
then reading/writing a bitmask to change the power state. There
are kernel functions that do the same operations. Better to use
the kernel functions.

Sometimes when the card is restarted it may cause -
"irq 16: nobody cared (try booting with the "irqpoll" option)"
that is likely caused so, that the card, after the hard reset
finishes, pulls on the irq. Disabling the ints before or after
the hpsa_kdump_hard_reset_controller fixes it.
At this point we can't know in which state the card is,
so using SA5_INTR_OFF + SA5_REPLY_INTR_MASK_OFFSET defines directly,
instead of the function the drivers provides, seems to be apropriate.

The following patch fixes an issue observed with 4k sector disks
where the max_hw_sectors attribute was getting set too large in
sd_revalidate_disk. Since sdkp->max_xfer_blocks is in units
of SCSI logical blocks and queue_max_hw_sectors is in units of
512 byte blocks, on a 4k sector disk, every time we went through
sd_revalidate_disk, we were taking the current value of
queue_max_hw_sectors and increasing it by a factor of 8. Fix
this by only shifting sdkp->max_xfer_blocks.

This fixes a regression caused by commit 1d5203 ("scsi: handle more device
handler setup/teardown in common code").
The bug is that the alua detach() callout will try to access the
sddev->scsi_dh_data, but we have already set it to NULL. This patch
moves the clearing of that field to after detach() is called.

This patch fix is to use default firmware configuration files
present in the adapter incase if not available in standard
/lib/firmware/* dir. Additional cleanup is done to reuse flash
related defines from cxgb4 header file.

This is the blk-mq part to support tag allocation policy. The default
allocation policy isn't changed (though it's not a strict FIFO). The new
policy is round-robin for libata. But it's a try-best implementation. If
multiple tasks are competing, the tags returned will be mixed (which is
unavoidable even with !mq, as requests from different tasks can be
mixed in queue)

make libata use block generic tag allocation, so let's add a policy to
tag allocation.

This can happen if a multipathed device uses DIX and another path is
added via an adapter that does not support it. Multipath should not
allow this path to be added, but we should not depend upon that to avoid
crashing.
