Heiko Carstens af51160ebd s390/smp: initialize cpu_present_mask in setup_arch
In order to be able to setup the cpu to node mappings early it is a
prerequisite to know which cpus are present. Therefore cpus must be
detected much earlier than before.

For sclp based cpu detection this requires yet another early sclp
call, since the system is not ready to use the regular interrupt and
memory allocations.

Heiko Carstens 307b3114ef s390/numa: always use logical cpu and core ids
The toptree algorithm uses the physical core ids to create a mapping
between cores and nodes (to_node_id array within emu_cores structure).
The core ids are used as an index into an array which size depends on
CONFIG_NR_CPUS. If the physical core ids are larger, this will result
in out-of-bounds write accesses.

Generate logical core ids instead to avoid this.

Martin Schwidefsky 1a36a39e22 s390/dump: rework CPU register dump code
To collect the CPU registers of the crashed system allocated a single
page with memblock_alloc_base and use it as a copy buffer. Replace the
stop-and-store-status sigp with a store-status-at-address sigp in
smp_save_dump_cpus() and smp_store_status(). In both cases the target
CPU is already stopped and store-status-at-address avoids the detour
via the absolute zero page.

For kexec simplify s390_reset_system and call store_status() before
the prefix register of the boot CPU has been set to zero. Use STPX
to store the prefix register and remove dump_prefix_page.

Michael Holzheu 1592a8e456 s390/kdump: fix nosmt kernel parameter
It turned out that SIGP set-multi-threading can only be done once.
Therefore switching to a different MT level after switching to
sclp.mtid_prev in the dump case fails.

As a symptom specifying the "nosmt" parameter currently fails for
the kdump kernel and the kernel starts with multi-threading enabled.

So fix this and issue diag 308 subcode 1 call after collecting the
CPU states for the dump. Also enhance the diag308_reset() function to
be usable also with enabled lowcore protection and prefix register != 0.
After the reset it is possible to switch the MT level again. We have
to do the reset very early in order not to kill the already initialized
console. Therefore instead of kmalloc() the corresponding memblock
functions have to be used. To avoid copying the sclp cpu code into
sclp_early, we now use the simple sigp loop method for CPU detection.

Martin Schwidefsky 10ad34bc76 s390: add SMT support
The multi-threading facility is introduced with the z13 processor family.
This patch adds code to detect the multi-threading facility. With the
facility enabled each core will surface multiple hardware threads to the
system. Each hardware threads looks like a normal CPU to the operating
system with all its registers and properties.

The SCLP interface reports the SMT topology indirectly via the maximum
thread id. Each reported CPU in the result of a read-scp-information
is a core representing a number of hardware threads.

To reflect the reduced CPU capacity if two hardware threads run on a
single core the MT utilization counter set is used to normalize the
raw cputime obtained by the CPU timer deltas. This scaled cputime is
reported via the taskstats interface. The normal /proc/stat numbers
are based on the raw cputime and are not affected by the normalization.

Martin Schwidefsky d59b93da5e s390/rwlock: use directed yield for write-locked rwlocks
Add an owner field to the arch_rwlock_t to be able to pass the timeslice
of a virtual CPU with diagnose 0x9c to the lock owner in case the rwlock
is write-locked. The undirected yield in case the rwlock is acquired
writable but the lock is read-locked is removed.

Michael Holzheu f4192bf2dc s390/smp: Avoid busy loop after halt and "begin" on z/VM
Currently the smp_stop_cpu() function for SMP kernels enters a busy
loop when "begin" is entered on the z/VM console after Linux is halted.
To avoid this behavior, use the non-SMP variant of smp_stop_cpu()
which stops the CPU again after "begin" is entered. As a side
effect we now have consistent behavior for SMP and non-SMP Linux.

smp_stop_cpu() should stop the current cpu even for !CONFIG_SMP.
Otherwise machine_halt() will return and and the machine generates a
panic instread of simply stopping the current cpu:

Heiko Carstens d80512f874 s390/smp: improve setup of possible cpu mask
Since under z/VM we cannot have more than 64 cpus, make sure the
cpu_possible_mask does not contain more bits.
This avoids wasting memory for dynamic per-cpu allocations if
CONFIG_NR_CPUS is larger than 64.

Michael Holzheu 5895294274 s390: Remove zfcpdump NR_CPUS dependency
Currently zfpcdump can only collect registers for up to CONFIG_NR_CPUS
CPUss. This dependency is not necessary. So remove it by dynamically
allocating the save area array.

Heiko Carstens a53c8fab3f s390/comments: unify copyright messages and remove file names
Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.

Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

Martin Schwidefsky 8b646bd759 [S390] rework smp code
Define struct pcpu and merge some of the NR_CPUS arrays into it, including
__cpu_logical_map, current_set and smp_cpu_state. Split smp related
functions to those operating on physical cpus and the functions operating
on a logical cpu number. Make the functions for physical cpus use a
pointer to a struct pcpu. This hides the knowledge about cpu addresses in
smp.c, entry[64].S and swsusp_asm64.S, thus remove the sigp.h header.

The PSW restart mechanism is used to start secondary cpus, calling a
function on an online cpu, calling a function on the ipl cpu, and for
the nmi signal. Replace the different assembler functions with a
single function restart_int_handler. The new entry point calls a function
whose pointer is stored in the lowcore of the target cpu and it can wait
for the source cpu to stop. This covers all existing use cases.

Overall the code is now simpler and there are ~380 lines less code.

Heiko Carstens 83a24e3290 [S390] topology: get rid of ifdefs
Remove all ifdefs from topology code and also only compile it for the
CONFIG_SCHED_BOOK case. The new code selects SCHED_MC if SCHED_BOOK is
selected. SCHED_MC without SCHED_BOOK is not possible anymore.
Furthermore various sysfs attributes are not available anymore for the
!SCHED_BOOK case. In particular all attributes that correspond to
CPU polarization.
But since all real world kernels have SCHED_BOOK selected anyway this
doesn't matter too much.

Michael Holzheu 1943f53c9c [S390] Force PSW restart on online CPU
PSW restart can be triggered on offline CPUs. If this happens, currently
the PSW restart code fails, because functions like smp_processor_id()
do not work on offline CPUs. This patch fixes this as follows:

If PSW restart is triggered on an offline CPU, the PSW restart (sigp restart)
is done a second time on another CPU that is online and the old CPU is
stopped afterwards.

Heiko Carstens da7f51c11d [S390] smp/idle: call init_idle() before starting a new cpu
Call init_idle() which (re-)initializes the idle task structure before
it gets used on a new cpu.
That way we can also get rid of the odd preempt_enable_no_resched()
call we have in the cpu offline path within cpu_idle(). That call
prevented preempt count imbalances between cpu hotplug operations.

Gerald Schaefer 59b6978745 [S390] spinlock: check virtual cpu running status
This patch introduces a new function that checks the running status
of a cpu in a hypervisor. This status is not virtualized, so the check
is only correct if running in an LPAR. On acquiring a spinlock, if the
cpu holding the lock is scheduled by the hypervisor, we do a busy wait
on the lock. If it is not scheduled, we yield over to that cpu.

This is mostly useless since no distro ships 31 bit kernels together
with a 64 bit dumper.
