All Alchemy chips have coherent DMA, but for example the USB or AC97
peripherals on the Au1000/1500/1100 are not.
This patch uses DMA_MAYBE_COHERENT on Alchemy and sets coherentio based
on CPU type.

Commit 7517de3486 ("MIPS: Alchemy: Redo
PCI as platform driver") added a reference to CONFIG_DEBUG_PCI. Change
it to CONFIG_PCI_DEBUG, as that is a valid Kconfig macro.
Also add a newline to a debugging printk that this fix enables.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

Move add_wired_entry to its own header file from where it will be
always included. Patch up other users of add_wired_entry to also include
the header as needed.

Move PCI Controller PM to syscore_ops since the platform_driver PM methods
are called way too late on resume and far too early on suspend (after and
before PCI device resume/suspend).
This also allows to simplify wired entry management a bit.
