The conflicts were minor:

1) sit.c changes overlap with change to ip_tunnel_xmit() signature.

2) br_multicast.c had an overlap between computing max_delay using
   msecs_to_jiffies and turning MLDV2_MRC() into an inline function
   with a name using lowercase instead of uppercase letters.

3) stmmac had two overlapping changes, one which conditionally allocated
   and hooked up a dma_cfg based upon the presence of the pbl OF property,
   and another one handling store-and-forward DMA made.  The latter of
   which should not go into the new of_find_property() basic block.

Bart Van Assche 7030fd6261 libfc: Do not invoke the response handler after fc_exch_done()
While the FCoE initiator driver invokes fc_exch_done() from inside
the libfc response handler, FCoE target drivers typically invoke
fc_exch_done() from outside the libfc response handler. The object
fc_exch.arg points at may disappear as soon as fc_exch_done() has
finished. So it's important not to invoke the response handler
function after fc_exch_done() has finished. Modify libfc such that
this guarantee is provided if fc_exch_done() is invoked from
outside a response handler. This patch fixes a sporadic crash in
FCoE target implementations after a command has been aborted.

Bart Van Assche cae7b6dd6c libfc: Avoid that sending after an abort triggers a kernel warning
Calling fc_seq_send() after an ABTS message has been received triggers
a kernel warning (WARN_ON(!(ep->esb_stat & ESB_ST_SEQ_INIT))). Avoid
this by returning -ENXIO to the caller if fc_seq_send() is invoked after
an ABTS message has been received.

Bart Van Assche b86788658b libfc: Fix a race in fc_exch_timer_set_locked()
It is allowed to pass a zero timeout value to fc_seq_exch_abort().
Avoid that this can cause the timeout function to drop the exchange
reference before it has been increased by fc_exch_timer_set_locked().
This patch fixes a crash when running FCoE target code with poisoning
enabled in the memory allocator.

Neil Horman 8b61243452 fcoe: make sure fcoe frames are unshared prior to manipulating them
Based on my last patch I noticed that fcoe_rcv has a simmilar problem, in that
it manipulates the passed in skb without checking to see if it has other users.
Making manipulations to a shared skb can result in various corruptions.

Easy fix, just make sure the skb is unshared prior to doing anything with it.

Looks like the root cause is the fact that the packet recieve function
fcoe_ctlr_recv enqueues the skb to a sk_buff_head_list prior to ensuring that
the skb is unshared.  This can happen when multiple packet listeners recieve an
skb, as the deliver_skb function just increments skb->users for each handler.
As a result, having multiple users of a single skb results in multiple
manipulators of its methods, implying list corruption, and the oops recorded
above.

The fix is pretty easy, just make sure that we clone the skb if its got multiple
users with the skb_share_check function, like other protocols do.

Vaughan Cao e32c9e6300 [SCSI] sg: checking sdp->detached isn't protected when open
@detached is set under the protection of sg_index_lock. Without getting the
lock, new sfp will be added during sg removal and there is no chance for it
to be picked out. So check with sg_index_lock held in sg_add_sfp().

Vaughan Cao 15b06f9a02 [SCSI] sg: use rwsem to solve race during exclusive open
A race condition may happen if two threads are both trying to open the same sg
with O_EXCL simultaneously. It's possible that they both find fsds list is
empty and get_exclude(sdp) returns 0, then they both call set_exclude() and
break out from wait_event_interruptible and resume open.

Now use rwsem to protect this process. Exclusive open gets write lock and
others get read lock. The lock will be held until file descriptor is closed.
This also leads 'exclude' only a status rather than a check mark.

Akinobu Mita a027b5b90e [SCSI] scsi_debug: fix logical block provisioning support when unmap_alignment != 0
Commit b90ebc3d5c ("[SCSI] scsi_debug:
fix logical block provisioning support") fixed several issues with
logical block provisioning support, but it still doesn't properly fix
the cases when unmap_alignment > 0.

For example, load scsi_debug module with the following module parameters
and make all blocks mapped by filling the storage with zero.

        # modprobe scsi_debug lbpu=1 unmap_alignment=1 unmap_granularity=4
        # dd if=/dev/zero of=$DEV

Then, try to unmap the first unmappable blocks at lba=1, but GET LBA STATUS
unexpectedly reports that the last UNMAP has done nothing.

        # sg_unmap --lba=1 --num=4 $DEV
        # sg_get_lba_status --lba=1 $DEV
        descriptor LBA: 0x0000000000000001  blocks: 16383  mapped

The problem is in map_index_to_lba(), which should return the first
LBA which is corresponding to a given index of provisioning map
(map_storep).

Akinobu Mita 150c3544b1 [SCSI] scsi_debug: fix endianness bug in sdebug_build_parts()
With module parameter num_parts > 0, partition table is built on the
ramdisk storage when loading the driver.  Unfortunately, there is an
endianness bug in sdebug_build_parts().  So the partition table is not
correctly initialized on big-endian systems.

The patch changes a specifier used to output some variables. Instead of using
stack for each byte the '%*ph[CN]' allows to take a one pointer and prints
entire buffer as a hexadecimal string with the separator ':' or ''.

Sreekanth Reddy 9c5000606d [SCSI] mpt3sas: Added a driver module parameter max_msix_vectors
Added a driver module parameter max_msix_vectors. Using this module parameter
the maximum number of MSI-X vectors could be set.

The number of MSI-X vectors used would be the minimum of MSI-X vectors
supported by the HBA, the number of CPU cores and the value set to
max_msix_vectors module parameter.

The default value of this module parameter is set to 8. The default value of
this parameter is set to 8 inorder to reduce the amount of memory required for
Reply Descriptor Post queue.  This is because with the higher MSI-X vectors,
some times kernel is not able to allocate the requested amount of memory and
crash is observed. To overcome this problem, the default value is set to 8.

Nilesh Javali 7ab284c9fb [SCSI] qla4xxx: Implementation of ACB configuration during Loopback for ISP8042
While loopback diagnostic is in progress, disable the ACB which resets
all the active connections to target. Disable ACB would filter out all
the DHCP multicast and broadcast packets which otherwise cause the
diagnostic test to take longer time to complete or failures in some
other cases.

Sreekanth Reddy 63e359d421 [SCSI] mpt2sas: Fix for kernel panic when driver loads with HBA connected to non LUN 0 configured expander
With some enclosures when LUN 0 is not created but LUN 1 or LUN X is created
then SCSI scan procedure calls target_alloc, slave_alloc call back functions
for LUN 0 and slave_destory() for same LUN 0.

In these kind of cases within slave_destroy, pointer to scsi_target in
_sas_device structure is set to NULL, following which when slave_alloc for
LUN 1 is called then starget would not be set properly for this LUN.
So, scsi_target pointer pointing to NULL value would lead to a crash later
in the discovery procedure.

To solve this issue set the sas_device's scsi_target pointer to scsi_device's
scsi_target if it is NULL earlier in slave_alloc callback function.

Sreekanth Reddy bc6d4c3b78 [SCSI] mpt2sas: when Async scanning is enabled then while scanning, devices are removed but their transport layer entries are not removed
When Async scanning mode is enabled and device scanning is in progress then
devices should not be removed. But in actuality, devices are removed but
their transport layer entries are not removed. This causes error to add
the same device to the transport layer after host reset or diagnostic
reset.

So, in this patch, modified the code in such a way that device is not removed
when Async scanning mode is enabled and device scanning is in progress.

So, Instead of checking for MPI2_IOCSTATUS_CONFIG_INVALID_PAGE value,
in this patch code is modified to check for IOCStatus not equals to
MPI2_IOCSTATUS_SUCCESS to break the while loop.

The intent of this patch is to perform a graceful shutdown of target drives even if
volume doesn't exits. Changes done in this patch

1. Removed the check for the presence of volumes before sending down
   MPI2_RAID_ACTION_SYSTEM_SHUTDOWN_INITIATED. Therefore, this RAID action
   would be sent if the card is IR Firmware.

2. The MPI2_RAID_ACTION_SYSTEM_SHUTDOWN_INITIATED is sent even when the
   system undergoes suspend (in addition to remove/shutdown which was already
   present)

This is caused by
enqueue_cmd_and_start_io()->
        set_performant_mode()->
                smp_processor_id()
Which if you have debugging enabled calls debug_processor_id() and triggers the warning.

The code here is
 c->Header.ReplyQueue = smp_processor_id() % h->nreply_queues;

Since it is not critical that the code complete on the same processor,
but the cpu is a hint used in generating the ReplyQueue and will still work if
the cpu migrates or is preempted, it is safe to use the raw_smp_processor_id()
to surpress the false positve warning.

To run discovery over iSER we need to advertize the CAP_TEXT_NEGO capability
towards user space. Also need to make sure the login RX buffer is posted when
SendTargets TEXT PDUs are sent. For that end, we use a setting of the
ISCSI_PARAM_DISCOVERY_SESS iscsi param as an indication that this is
discovery session.

Yijing Wang e1e819cc96 [SCSI] pm8001: clean up unnecessary MSI/MSI-X capability find
PCI core will initialize device MSI/MSI-X capability in
pci_msi_init_pci_dev(). So device driver should use
pci_dev->msi_cap/msix_cap to determine whether the device
support MSI/MSI-X instead of using
pci_find_capability(pci_dev, PCI_CAP_ID_MSI/MSIX).
Access to PCIe device config space again will consume more time.

      move this list head initialization out of _base_enable_msix to
      _scsih_probe so this will always be safe.

  3 - check that the controller PCI device and its BARs have been
      enabled before disabling them.

specification regarding reporting of the 3F 0E ASC/ASCQ UA.

Hans Verkuil 9504a92392 [SCSI] pm80xx: fix Adaptec 71605H hang
The IO command size is 128 bytes for these new controllers as opposed to 64
for the old 8001 controller.

The Adaptec out-of-tree driver did this correctly. After comparing the two
this turned out to be the crucial difference.

So don't hardcode the IO command size, instead use pm8001_ha->iomb_size as
that is the correct value for both old and new controllers.

Tomas Henzl 2cc5bfaf85 [SCSI] hpsa: fix a race in cmd_free/scsi_done
When the driver calls scsi_done and after that frees it's internal
preallocated memory it can happen that a new job is enqueud before
the memory is freed. The allocation fails and the message
"cmd_alloc returned NULL" is shown.
Patch below fixes it by moving cmd->scsi_done after cmd_free.

When removing the UFS driver, disable_irq() is called and the IRQ is
not enabled again.  Unfortunately, the IRQ is requested with IRQF_SHARED
and it can be shared among several devices.  So disabling the IRQ in
this way is just breaking other devices which are sharing the IRQ.

Akinobu Mita 36f4f3b672 [SCSI] ufshcd-pci: release ioremapped region during removing driver
Before commit 2953f850c3 ("[SCSI] ufs:
use devres functions for ufshcd"), UFSHCI register was ioremapped by
each glue-driver (ufshcd-pltfrm and ufshcd-pci) during probing and it
was iounmapped by core-driver during removing driver.  The commit
converted ufshcd-pltfrm to use devres functions, but it didn't convert
ufshcd-pci.

Therefore, the change causes ufshcd-pci driver not to iounmap UFSHCI
register region during removing driver.  This fixes it by converting
ufshcd-pci to use devres functions.

Sujit Reddy Thumma 66ec6d5940 [SCSI] ufs: Add support for host assisted background operations
Background operations in the UFS device can be disabled by
the host to reduce the response latency of transfer requests.
Add support for enabling/disabling the background operations
during runtime suspend/resume of the device.

If the device is in critical need of BKOPS it will raise an
URGENT_BKOPS exception which should be handled by the host to
make sure the device performs as expected.

During bootup, the BKOPS is enabled in the device by default.
The disable of BKOPS is supported only when the driver supports
runtime suspend/resume operations as the runtime PM framework
provides a way to determine the device idleness and hence BKOPS
can be managed effectively. During runtime resume the BKOPS is
disabled to reduce latency and during runtime suspend the BKOPS
is enabled to allow device to carry out idle time BKOPS.

In some cases where the BKOPS is disabled during runtime resume
and due to continuous data transfers the runtime suspend is not
triggered, the BKOPS is enabled when the device raises a level-2
exception (outstanding operations - performance impact).

Sujit Reddy Thumma 5a0b0cb9be [SCSI] ufs: Add support for sending NOP OUT UPIU
As part of device initialization sequence, sending NOP OUT UPIU and
waiting for NOP IN UPIU response is mandatory. This confirms that the
device UFS Transport (UTP) layer is functional and the host can configure
the device with further commands. Add support for sending NOP OUT UPIU to
check the device connection path and test whether the UTP layer on the
device side is functional during initialization.

A tag is acquired from the SCSI tag map space in order to send the device
management command. When the tag is acquired by internal command the scsi

Aaron Lu f1bc1e4c44 ata: acpi: rework the ata acpi bind support
Binding ACPI handle to SCSI device has several drawbacks, namely:
1 During ATA device initialization time, ACPI handle will be needed
  while SCSI devices are not created yet. So each time ACPI handle is
  needed, instead of retrieving the handle by ACPI_HANDLE macro,
  a namespace scan is performed to find the handle for the corresponding
  ATA device. This is inefficient, and also expose a restriction on
  calling path not holding any lock.
2 The binding to SCSI device tree makes code complex, while at the same
  time doesn't bring us any benefit. All ACPI handlings are still done
  in ATA module, not in SCSI.

Rework the ATA ACPI binding code to bind ACPI handle to ATA transport
devices(ATA port and ATA device). The binding needs to be done only once,
since the ATA transport devices do not go away with hotplug. And due to
this, the flush_work call in hotplug handler for ATA bay is no longer
needed.

Tested on an Intel test platform for binding and runtime power off for
ODD(ZPODD) and hard disk; on an ASUS S400C for binding and normal boot
and S3, where its SATA port node has _SDD and _GTF control methods when
configured as an AHCI controller and its PATA device node has _GTF
control method when configured as an IDE controller. SATA PMP binding
and ATA hotplug is not tested.

Anton Blanchard f5944daa0a [SCSI] lpfc: Don't force CONFIG_GENERIC_CSUM on
We want ppc64 to be able to select between optimised assembly
checksum routines in big endian and the generic lib/checksum.c
routines in little endian.

The lpfc driver is forcing CONFIG_GENERIC_CSUM on which means
we are unable to make the decision to enable it in the arch
Kconfig. If the option exists it is always forced on.

This got introduced in 3.10 via commit 6a7252fdb0 ([SCSI] lpfc:
fix up Kconfig dependencies). I spoke to Randy about it and
the original issue was with CRC_T10DIF not being defined.

As such, remove the select of CONFIG_GENERIC_CSUM.

Michael Chan 9e9402eb45 cnic, bnx2i: Fix bug on some bnx2x devices that don't support iSCSI
On some bnx2x devices, iSCSI is determined to be unsupported only after
firmware is downloaded.  We need to check max_iscsi_conn again after
NETDEV_UP and block iSCSI init operations.  Without this fix, iscsiadm
can hang as the firmware will not respond to the iSCSI init message.

The attempts to make fnic_fip_queue a single instance for the driver

Problem: When Hardware IOMMU is on, megaraid_sas driver initialization fails
in kdump kernel with LSI MegaRAID controller(device id-0x73).

Actually this issue needs fix in firmware, but for firmware running in field,
this driver fix is proposed to resolve the issue.  At firmware initialization
time, if firmware does not come to ready state, driver will reset the adapter
and retry for firmware transition to ready state unconditionally(not only
executed for kdump kernel).

Olaf Hering cfc25993e8 Drivers: hv: remove HV_DRV_VERSION
Remove HV_DRV_VERSION, it has no meaning for upstream drivers.

Initially it was supposed to show the "Linux Integration Services"
version, now it is not in sync anymore with the out-of-tree drivers
available from the MSFT website.

The only place where a version string is still required is the KVP
command "IntegrationServicesVersion" which is handled by
tools/hv/hv_kvp_daemon.c. To satisfy such KVP request from the host pass
the current string to the daemon during KVP userland registration.

David S. Miller 21af8107f2 esp_scsi: Fix tag state corruption when autosensing.
Meelis Roos reports a crash in esp_free_lun_tag() in the presense
of a disk which has died.

The issue is that when we issue an autosense command, we do so by
hijacking the original command that caused the check-condition.

When we do so we clear out the ent->tag[] array when we issue it via
find_and_prep_issuable_command().  This is so that the autosense
command is forced to be issued non-tagged.

That is problematic, because it is the value of ent->tag[] which
determines whether we issued the original scsi command as tagged
vs. non-tagged (see esp_alloc_lun_tag()).

And that, in turn, is what trips up the sanity checks in
esp_free_lun_tag().  That function needs the original ->tag[] values
in order to free up the tag slot properly.

Fix this by remembering the original command's tag values, and
having esp_alloc_lun_tag() and esp_free_lun_tag() use them.

Ewan D. Milne 085b513f97 [SCSI] sd: fix crash when UA received on DIF enabled device
sd_prep_fn will allocate a larger CDB for the command via mempool_alloc
for devices using DIF type 2 protection.  This CDB was being freed
in sd_done, which results in a kernel crash if the command is retried
due to a UNIT ATTENTION.  This change moves the code to free the larger
CDB into sd_unprep_fn instead, which is invoked after the request is
complete.

It is no longer necessary to call scsi_print_command separately for
this case as the ->cmnd will no longer be NULL in the normal code path.

Also removed conditional test for DIF type 2 when freeing the larger
CDB because the protection_type could have been changed via sysfs while
the command was executing.

Jeff Skirvin 96f15f2903 [SCSI] isci: Fix a race condition in the SSP task management path
This commit fixes a race condition in the isci driver abort task and SSP
device task management path.  The race is caused when an I/O termination
in the SCU hardware is necessary because of an SSP target timeout condition,
and the check of the I/O end state races against the HW-termination-driven
end state.  The failure of the race meant that no TMF was sent to the device
to clean-up the pending I/O.

Douglas Gilbert 655ee63cf3 [SCSI] scsi constants: command, sense key + additional sense strings
It has been several years since the SCSI constants.c
file has been updated. The attached is against lk 3.10
and brings the command strings, sense keys and additional
sense code strings into sync with spc4r36g.pdf.

Certain SCSI command names that previously only took the
opcode (i.e. byte 0 of the cdb) into account, have been
split into several command names using the associated
service action field to differentiate. For example,
persistent reservations that previously had 2 commands
(i.e. "in" and "out") have been expanded to 12 commands
(e.g. "Persistent reserve in, read reservation").

Sync SCSI command names, sense key strings and additional
sense code strings with SPC-4 draft revision 36g

Robert Love a2ceb1fbaa libfcoe: Fix meaningless log statement
ctlr_dev was initialized to NULL, and never re-assigned. This
caused the log statement to always report failure. This patch
removes the unused variable and fixes the log statement to always
report 'success', as that is what should be logged if the code
reaches this point.

Yi Zou 418a8cfe69 fcoe: fix the link error status block sparse warnings
Both fcoe_fc_els_lesb and fc_els_lesb are in __be32 already, and both are
exactly the same size in bytes, with somewhat different member names to
reflect the fact the former is for Ethernet media the latter is for Fiber
Channel, so, remove conversion and use __be32 directly. This fixes the warning
from sparse check.

Akinobu Mita a451751172 scsi_debug: fix do_device_access() with wrap around range
do_device_access() is a function that abstracts copying SG list from/to
ramdisk storage (fake_storep).

It must deal with the ranges exceeding actual fake_storep size, because
such ranges are valid if virtual_gb is set greater than zero, and they
should be treated as fake_storep is repeatedly mirrored up to virtual
size.

Unfortunately, it can't deal with the range which wraps around the end of
fake_storep.  A wrap around range is copied by two
sg_copy_{from,to}_buffer() calls, but sg_copy_{from,to}_buffer() can't
copy from/to in the middle of SG list, therefore the second call can't
copy correctly.

This fixes it by using sg_pcopy_{from,to}_buffer() that can copy from/to
the middle of SG list.

This also simplifies the assignment of sdb->resid in
fill_from_dev_buffer().  Because fill_from_dev_buffer() is now only called
once per command execution cycle.  So it is not necessary to take care to
decrease sdb->resid if fill_from_dev_buffer() is called more than once.

Bart Van Assche 0516c08d10 [SCSI] enable destruction of blocked devices which fail LUN scanning
If something goes wrong during LUN scanning, e.g. a transport layer
failure occurs, then __scsi_remove_device() can get invoked by the
LUN scanning code for a SCSI device in state SDEV_CREATED_BLOCK and
before the SCSI device has been added to sysfs (is_visible == 0).
Make sure that even in this case the transition into state SDEV_DEL
occurs. This avoids that __scsi_remove_device() can get invoked a
second time by scsi_forget_host() if this last function is invoked
from another thread than the thread that performs LUN scanning.

scsi_run_queue() examines all SCSI devices that are present on
the starved list. Since scsi_run_queue() unlocks the SCSI host
lock a SCSI device can get removed after it has been removed
from the starved list and before its queue is run. Protect
against that race condition by holding a reference on the
queue while running it.

Akinobu Mita e9926b4376 [SCSI] scsi_debug: invalidate protection info for unmapped region
When UNMAP command is issued with the data integrity support enabled,
the protection info for the unmapped region is remain unchanged.
So READ command for the region later on causes data integrity failure.

This fixes it by invalidating protection info for the unmapped region
by filling with 0xff pattern.

Akinobu Mita 7cb69d0397 [SCSI] scsi_debug: fix NULL pointer dereference with parameters dif=0 dix=1
The protection info dif_storep is allocated only when parameter dif is
not zero.  But it will be accessed when reading or writing to the storage
installed with parameter dix is not zero.

So kernel crashes if scsi_debug module is loaded with parameters dix=1 and
dif=0.

This fixes it by making dif_storep available if parameter dix is not zero
instead of checking if parameter dif is not zero.

("mm: stack based kmap_atomic()").  Because the kmap_atomic slot KM_IRQ0
was used for data page and the slot KM_IRQ1 was used for protection page.

But KM_types are gone and kmap_atomic() is using stack based implementation.
So two different kmap_atomic() usages must be strictly nested now.

This change ensures kmap_atomic() usage is strictly nested.

Akinobu Mita 518d9df871 [SCSI] scsi_debug: fix invalid address passed to kunmap_atomic()
In the function prot_verify_write(), the kmap address 'daddr' is
incremented in the loop for each data page.  Finally 'daddr' reaches
the next page boundary in the end of the loop, and the invalid address
is passed to kunmap_atomic().

Fix the issue by not incrementing 'daddr' in the loop and offsetting it
by the loop counter on demand.

Sreekanth Reddy f5edbe77d9 [SCSI] mpt3sas: when async scanning is enabled then while scanning, devices are removed but their transport layer entries are not removed
When Async scanning mode is enabled and device scanning is in progress,
devices should not be removed. But in actuality, devices are removed but
their transport layer entries are not removed. This causes error to add
the same device to the transport layer after host reset or diagnostic
reset.

So, in this patch, modified the code in such a way that device is not removed
when Async scanning mode is enabled and device scanning is in progress.

Sreekanth Reddy b65cfedf45 [SCSI] mpt3sas: fix for kernel panic when driver loads with HBA conected to non LUN 0 configured expander
With some enclosures when LUN 0 is not created but LUN 1 or LUN X is created
then SCSI scan procedure calls target_alloc, slave_alloc call back functions
for LUN 0 and slave_destory() for same LUN 0.

In these kind of cases within slave_destroy, pointer to scsi_target in
_sas_device structure is set to NULL, following which when slave_alloc for LUN
1 is called then starget would not be set properly for this LUN.  So,
scsi_target pointer pointing to NULL value would lead to a crash later in the
discovery procedure.

To solve this issue set the sas_device's scsi_target pointer to scsi_device's
scsi_target if it is NULL earlier in slave_alloc callback function.

Bart Van Assche 8c0eb596ba [SCSI] qla2xxx: Fix a memory leak in an error path of qla2x00_process_els()
Avoid that the fcport structure gets leaked if
bsg_job->request->msgcode == FC_BSG_HST_ELS_NOLOGIN, the fcport
allocation succeeds and the !vha->flags.online branch is taken.
This was detected by Coverity. However, Coverity does not recognize
that all qla2x00_process_els() callers specify either
FC_BSG_RPT_ELS or FC_BSG_HST_ELS_NOLOGIN in the field
bsg_job->request->msgcode and that the value of that field is not
modified inside that function. This results in a false positive
report about a possible memory leak in an error path for
bsg_job->request->msgcode values other than the two mentioned
values.  Make it easy for Coverity (and for humans) to recognize
that there is no fcport leak in the error path by changing the
bsg_job->request->msgcode == FC_BSG_HST_ELS_NOLOGIN test into
bsg_job->request->msgcode != FC_BSG_RPT_ELS.

Coverity reports "Overrunning struct type ct_sns_req of 1228 bytes
by passing it to a function which accesses it at byte offset 8207"
for each qla2x00_prep_ct_req(), qla2x00_prep_ct_fdmi_req() and
qla24xx_prep_ct_fm_req() call. Help Coverity to recognize that
these calls do not trigger a buffer overflow by making it explicit
that these three functions initializes both the request and reply
structures. This patch does not change any functionality.

When we are intentionally generating a firmware dump by executing the
MBC_GEN_SYSTEM_ERROR command, the command actually times out. The normal
course of action when a mailbox command times out is to take a firmware dump.

MBA_SYSTEM_ERR AEN already generates a firmware dump.

Kees Cook ffc8b30866 block: do not pass disk names as format strings
Disk names may contain arbitrary strings, so they must not be
interpreted as format strings.  It seems that only md allows arbitrary
strings to be used for disk names, but this could allow for a local
memory corruption from uid 0 into ring 0.

  the number of cpufreq commits, but Viresh Kumar (who works on cpufreq)
  remains the most active patch submitter.

  To me, the most significant change is the addition of offline/online
  device operations to the driver core (with the Greg's blessing) and
  the related modifications of the ACPI core hotplug code.  Next are the
  freezer updates from Colin Cross that should make the freezing of
  tasks a bit less heavy weight.

  We also have a couple of regression fixes, a number of fixes for
  issues that have not been identified as regressions, two new drivers

  Highlights:

   - Hotplug changes to support graceful hot-removal failures.

     It sometimes is necessary to fail device hot-removal operations
     gracefully if they cannot be carried out completely.  For example,
     if memory from a memory module being hot-removed has been allocated
     for the kernel's own use and cannot be moved elsewhere, it's
     desirable to fail the hot-removal operation in a graceful way
     rather than to crash the kernel, but currenty a success or a kernel
     crash are the only possible outcomes of an attempted memory
     hot-removal.  Needless to say, that is not a very attractive
     alternative and it had to be addressed.

     However, in order to make it work for memory, I first had to make
     it work for CPUs and for this purpose I needed to modify the ACPI
     processor driver.  It's been split into two parts, a resident one
     handling the low-level initialization/cleanup and a modular one
     playing the actual driver's role (but it binds to the CPU system
     device objects rather than to the ACPI device objects representing
     processors).  That's been sort of like a live brain surgery on a
     patient who's riding a bike.

     So this is a little scary, but since we found and fixed a couple of
     regressions it caused to happen during the early linux-next testing
     (a month ago), nobody has complained.

     As a bonus we remove some duplicated ACPI hotplug code, because the
     ACPI-based CPU hotplug is now going to use the common ACPI hotplug
     code.

   - Lighter weight freezing of tasks.

     These changes from Colin Cross and Mandeep Singh Baines are
     targeted at making the freezing of tasks a bit less heavy weight
     operation.  They reduce the number of tasks woken up every time
     during the freezing, by using the observation that the freezer
     simply doesn't need to wake up some of them and wait for them all
     to call refrigerator().  The time needed for the freezer to decide
     to report a failure is reduced too.

     Also reintroduced is the check causing a lockdep warining to
     trigger when try_to_freeze() is called with locks held (which is
     generally unsafe and shouldn't happen).

   - cpufreq updates

     First off, a commit from Srivatsa S Bhat fixes a resume regression
     introduced during the 3.10 cycle causing some cpufreq sysfs
     attributes to return wrong values to user space after resume.  The
     fix is kind of fresh, but also it's pretty obvious once Srivatsa
     has identified the root cause.

     Second, we have a new freqdomain_cpus sysfs attribute for the
     acpi-cpufreq driver to provide information previously available via
     related_cpus.  From Lan Tianyu.

     Finally, we fix a number of issues, mostly related to the
     CPUFREQ_POSTCHANGE notifier and cpufreq Kconfig options and clean
     up some code.  The majority of changes from Viresh Kumar with bits

   - ACPICA update

     A usual bunch of updates from the ACPICA upstream.

     During the 3.4 cycle we introduced support for ACPI 5 extended

     HW-reduced mode bit is set in the FADT flags and the code attempted
     to use them without checking that bit.  That caused suspend/resume

     those registers to be used only if the HW-reduced mode bit is set.

In UIC error interrupt handling, it checks if UIC data link layer error
code indicates PA_INIT_ERROR in order to determine whether a fatal error
handling is needed or not.

But the code tries to read UIC data link layer error code from wrong
REG_UIC_ERROR_CODE_PHY_ADAPTER_LAYER, it should be
REG_UIC_ERROR_CODE_DATA_LINK_LAYER.

Seungwon Jeon 6ccf44fe4c [SCSI] ufs: rework link start-up process
Link start-up requires long time with multiphase handshakes
between UFS host and device. This affects driver's probe time.
This patch let link start-up run asynchronously. Link start-up
will be executed at the end of prove separately.
Along with this change, the following is worked.

Defined completion time of uic command to avoid a permanent wait.
Added mutex to guarantee of uic command at a time.
Adapted some sequence of controller initialization after link statup
according to HCI standard.

The standard scsi timeout is not appropriate in some of the environments where
Hyper-V is deployed. Set this timeout appropriately for all devices managed
by this driver.

On cloud environments where storage latencies may be unbounded, having the
scsi layer initiating recovery can be problematic since (a) the host is
already implementing a variety of recovery strategies and (b) implementing a
recovery strategy at the VM level may be more appropriate in cases where
storage latencies exceed a certain threshold.
