Alan Cox 934e6ebf96 tty: Redo current tty locking
Currently it is sometimes locked by the tty mutex and sometimes by the
sighand lock. The latter is in fact correct and now we can hand back referenced
objects we can fix this up without problems around sleeping functions.

Alan Cox 452a00d2ee tty: Make get_current_tty use a kref
We now return a kref covered tty reference. That ensures the tty structure
doesn't go away when you have a return from get_current_tty. This is not
enough to protect you from most of the resources being freed behind your
back - yet.

 an s390 leak found while debugging the former]

Al Viro 77e69dac3c [PATCH] fix races and leaks in vfs_quota_on() users
* new helper: vfs_quota_on_path(); equivalent of vfs_quota_on() sans the
  pathname resolution.
* callers of vfs_quota_on() that do their own pathname resolution and
  checks based on it are switched to vfs_quota_on_path(); that way we
  avoid the races.
* reiserfs leaked dentry/vfsmount references on several failure exits.

Jan Kara 657d3bfa98 quota: implement sending information via netlink about user below quota
Sometimes it may be useful for userspace to know (e.g.  for some hosting
guys) that some user stopped exceeding his hardlimit or softlimit in
quotas.  Implement sending of such events to userspace via quota netlink
protocol so that they don't have to poll for such events.  Based on idea
and initial implementation by Vladislav Bogdanov.

Jan Kara b48d380541 quota: fix possible infinite loop in quota code
When quota structure is going to be dropped and it is dirty, quota code tries
to write it.  If the write fails for some reason (e.  g.  transaction cannot
be started because the journal is aborted), we try writing again and again and
again...  Fix the problem by clearing the dirty bit even if the write failed.

Jan Kara 9377abd026 quota: don't call sync_fs() from vfs_quota_off() when there's no quota turn off
Sometimes, vfs_quota_off() is called on a partially set up super block (for
example when fill_super() fails for some reason).  In such cases we cannot
call ->sync_fs() because it can Oops because of not properly filled in super
block.  So in case we find there's not quota to turn off, we just skip
everything and return which fixes the above problem.

Jan Kara 0ff5af8340 quota: quota core changes for quotaon on remount
Currently, we just turn quotas off on remount of filesystem to read-only
state.  The patch below adds necessary framework so that we can turn quotas
off on remount RO but we are able to automatically reenable them again when
filesystem is remounted to RW state.  All we need to do is to keep references
to inodes of quota files when remounting RO and using these references to
reenable quotas when remounting RW.

Jan Blunck 4ac9137858 Embed a struct path into struct nameidata instead of nd->{dentry,mnt}
This is the central patch of a cleanup series. In most cases there is no good
reason why someone would want to use a dentry for itself. This series reflects
that fact and embeds a struct path into nameidata.

Together with the other patches of this series
- it enforced the correct order of getting/releasing the reference count on
  <dentry,vfsmount> pairs
- it prepares the VFS for stacking support since it is essential to have a
  struct path in every place where the stack can be traversed
- it reduces the overall code size:

Jan Kara 941d2380e9 quota: improve inode list scanning in add_dquot_ref()
We restarted scan of sb->s_inodes list whenever we had to drop inode_lock
in add_dquot_ref().  This leads to overall quadratic running time and thus
add_dquot_ref() can take several minutes when called on a life filesystem.
We fix the problem by using the fact that inode cannot be removed from
s_inodes list while we hold a reference to it and thus we can safely
restart the scan if we don't drop the reference.  Here we use the fact that
inodes freshly added to s_inodes list are already guaranteed to have quotas
properly initialized and the ordering of inodes on s_inodes list does not
change so we cannot skip any inode.

Jan Kara c525460e27 Don't send quota messages repeatedly when hardlimit reached
We should send quota message to netlink only once when hardlimit is
reached.  Otherwise user could easily make the system busy by trying to
exceed the hardlimit (and also the messages could be anoying if you cannot
stop writing just now).

Jan Kara 8e8934695d quota: send messages via netlink
Implement sending of quota messages via netlink interface.  The advantage
is that in userspace we can better decide what to do with the message - for
example display a dialogue in your X session or just write the message to
the console.  As a bonus, we can get rid of problems with console locking
deep inside filesystem code once we remove the old printing mechanism.

Paul Mundt 20c2df83d2 mm: Remove slab destructors from kmem_cache_create().
Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.

This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

Rusty Russell 8e1f936b73 mm: clean up and kernelify shrinker registration
I can never remember what the function to register to receive VM pressure
is called.  I have to trace down from __alloc_pages() to find it.

It's called "set_shrinker()", and it needs Your Help.

1) Don't hide struct shrinker.  It contains no magic.
2) Don't allocate "struct shrinker".  It's not helpful.
3) Call them "register_shrinker" and "unregister_shrinker".
4) Call the function "shrink" not "shrinker".
5) Reduce the 17 lines of waffly comments to 13, but document it properly.

Jan Kara 7925409e20 circular locking dependency found in QUOTA OFF
i_mutex on quota files is special.  Unlike i_mutexes for other inodes it is
acquired under dqonoff_mutex.  Tell lockdep about this lock ranking.  Also
comment and code in quota_sync_sb() seem to be bogus (as i_mutex for quota
file can be acquired under dqonoff_mutex).  Move truncate_inode_pages()
call under dqonoff_mutex and save some problems with races...

Pavel Emelianov b5e618181a Introduce a handy list_first_entry macro
There are many places in the kernel where the construction like

   foo = list_entry(head->next, struct foo_struct, list);

are used.
The code might look more descriptive and neat if using the macro

   list_first_entry(head, type, member) \
             list_entry((head)->next, type, member)

Here is the macro itself and the examples of its usage in the generic code.
 If it will turn out to be useful, I can prepare the set of patches to
inject in into arch-specific code, drivers, networking, etc.

Eric W. Biederman 0b4d414714 [PATCH] sysctl: remove insert_at_head from register_sysctl
The semantic effect of insert_at_head is that it would allow new registered
sysctl entries to override existing sysctl entries of the same name.  Which is
pain for caching and the proc interface never implemented.

I have done an audit and discovered that none of the current users of
register_sysctl care as (excpet for directories) they do not register
duplicate sysctl entries.

So this patch simply removes the support for overriding existing entries in
the sys_sysctl interface since no one uses it or cares and it makes future
enhancments harder.

Christoph Hellwig d003fb70fd [PATCH] remove sb->s_files and file_list_lock usage in dquot.c
Iterate over sb->s_inodes instead of sb->s_files in add_dquot_ref.  This
reduces list search and lock hold time aswell as getting rid of one of the
few uses of file_list_lock which Ingo identified as a scalability problem.

Previously we called dq_op->initialize for every inode handing of a
writeable file that wasn't initialized before.  Now we're calling it for
every inode that has a non-zero i_writecount, aka a writeable file
descriptor refering to it.

Thanks a lot to Jan Kara for running this patch through his quota test
harness.

This patch changes struct file to use struct path instead of having
independent pointers to struct dentry and struct vfsmount, and converts all
users of f_{dentry,vfsmnt} in fs/ to use f_path.{dentry,mnt}.

Additionally, it adds two #define's to make the transition easier for users of
the f_dentry and f_vfsmnt.

Peter Zijlstra 24ec839c43 [PATCH] tty: ->signal->tty locking
Fix the locking of signal->tty.

Use ->sighand->siglock to protect ->signal->tty; this lock is already used
by most other members of ->signal/->sighand.  And unless we are 'current'
or the tasklist_lock is held we need ->siglock to access ->signal anyway.

(NOTE: sys_unshare() is broken wrt ->sighand locking rules)

Note that tty_mutex is held over tty destruction, so while holding
tty_mutex any tty pointer remains valid.  Otherwise the lifetime of ttys
are governed by their open file handles.  This leaves some holes for tty
access from signal->tty (or any other non file related tty access).

It solves the tty SLAB scribbles we were seeing.

(NOTE: the change from group_send_sig_info to __group_send_sig_info needs to
       be examined by someone familiar with the security framework, I think
       it is safe given the SEND_SIG_PRIV from other __group_send_sig_info
       invocations)

Mark file system inode and similar slab caches subject to SLAB_MEM_SPREAD
memory spreading.

If a slab cache is marked SLAB_MEM_SPREAD, then anytime that a task that's
in a cpuset with the 'memory_spread_slab' option enabled goes to allocate
from such a slab cache, the allocations are spread evenly over all the
memory nodes (task->mems_allowed) allowed to that task, instead of favoring
allocation on the node local to the current cpu.

The choice of which slab caches to so mark was quite simple.  I marked
those already marked SLAB_RECLAIM_ACCOUNT, except for fs/xfs, dentry_cache,
inode_cache, and buffer_head, which were marked in a previous patch.  Even
though SLAB_RECLAIM_ACCOUNT is for a different purpose, it marks the same
potentially large file system i/o related slab caches as we need for memory
spreading.

Given that the rule now becomes "wherever you would have used a
SLAB_RECLAIM_ACCOUNT slab cache flag before (usually the inode cache), use
the SLAB_MEM_SPREAD flag too", this should be easy enough to maintain.
Future file system writers will just copy one of the existing file system
slab cache setups and tend to get it right without thinking.

Jan Kara 6362e4d4ed [PATCH] Fix oops in invalidate_dquots()
When quota is being turned off we assumed that all the references to dquots
were already dropped.  That need not be true as inodes being deleted are
not on superblock's inodes list and hence we need not reach it when
removing quota references from inodes.  So invalidate_dquots() has to wait
for all the users of dquots (as quota is already marked as turned off, no
new references can be acquired and so this is bound to happen rather
early).  When we do this, we can also remove the iprune_sem locking as it
was protecting us against exactly the same problem when freeing inodes
icache memory.

Al Viro 7b7b1ace2d [PATCH] saner handling of auto_acct_off() and DQUOT_OFF() in umount
The way we currently deal with quota and process accounting that might
keep vfsmount busy at umount time is inherently broken; we try to turn
them off just in case (not quite correctly, at that) and

  a) pray umount doesn't fail (otherwise they'll stay turned off)
  b) pray nobody doesn anything funny just as we turn quota off

Moreover, LSM provides hooks for doing the same sort of broken logics.

The proper way to deal with that is to introduce the second kind of
reference to vfsmount.  Semantics:

 - when the last normal reference is dropped, all special ones are
   converted to normal ones and if there had been any, cleanup is done.
 - normal reference can be cloned into a special one
 - special reference can be converted to normal one; that's a no-op if
   we'd already passed the point of no return (i.e.  mntput() had
   converted special references to normal and started cleanup).

The way it works: e.g. starting process accounting converts the vfsmount
reference pinned by the opened file into special one and turns it back
to normal when it gets shut down; acct_auto_close() is done when no
normal references are left.  That way it does *not* obstruct umount(2)
and it silently gets turned off when the last normal reference to
vfsmount is gone.  Which is exactly what we want...

The same should be done by LSM module that holds some internal
references to vfsmount and wants to shut them down on umount - it should
make them special and security_sb_umount_close() will be called exactly
when the last normal reference to vfsmount is gone.

quota handling is even simpler - we don't use normal file IO anymore, so
there's no need to hold vfsmounts at all.  DQUOT_OFF() is done from
deactivate_super(), where it really belongs.

Eric Dumazet 2f51201662 [PATCH] reduce sizeof(struct file)
Now that RCU applied on 'struct file' seems stable, we can place f_rcuhead
in a memory location that is not anymore used at call_rcu(&f->f_rcuhead,
file_free_rcu) time, to reduce the size of this critical kernel object.

The trick I used is to move f_rcuhead and f_list in an union called f_u

The callers are changed so that f_rcuhead becomes f_u.fu_rcuhead and f_list
becomes f_u.f_list

Al Viro 27496a8c67 [PATCH] gfp_t: fs/*
- ->releasepage() annotated (s/int/gfp_t), instances updated
 - missing gfp_t in fs/* added
 - fixed misannotation from the original sweep caught by bitwise checks:
   XFS used __nocast both for gfp_t and for flags used by XFS allocator.
   The latter left with unsigned int __nocast; we might want to add a
   different type for those but for now let's leave them alone.  That,
   BTW, is a case when __nocast use had been actively confusing - it had
   been used in the same code for two different and similar types, with
   no way to catch misuses.  Switch of gfp_t to bitwise had caught that
   immediately...

One tricky bit is left alone to be dealt with later - mapping->flags is
a mix of gfp_t and error indications.  Left alone for now.
