The current code was adjusting guest time in interrupt context; do this
in process context since we may have to initiate cross-processor
interrupts as part of setting time.

Individual drivers should never be touching the 'struct device' field,
so if that is a requirement to pass to the vmbus core, you know
something is wrong.
This patch fixes that all up, and resolves the problem where the module
reference counting was not happening properly for the individual drivers
as well. Overall, it reduces the lines of code the individual drivers
have to have, which tells you that this is the correct thing to do.
Also, somehow the _GPL marking for the functions got removed on an older
patch. As the name of the function was changing, properly change the
_GPL marking as well at the same time.

The util module expects that the util channels are fully initialized
when the module loads. To deal with the race condition which can result
in a NULL pointer dereferencing if the util module were to load before
all the util channels are fully initialized, in commit:

code was introduced in the vmbus driver to ensure that all the
util channels were fully initialized before returning from the load
of the vmbus driver. This solution has several problems: if for whatever
reason, any util channel were to fail to initialize, vmbus driver would
wait indefinitely. We deal with this synchronization issue very differently
in this patch.

The OSD layer was a wrapper around native interfaces
adding little value and was infact buggy -
refer to the osd_wait.patch for details.
This patch gets rid of the OSD abstraction.
