The PCIE capability offset is saved during PCI bus walking. It will
remove an unnecessary search in the PCI configuration space if this
value is referenced instead of reacquiring it.

T3 hardware doorbell FIFO overflows can cause application stalls due
to lost doorbell ring events. This has been seen when running large
NP IMB alltoall MPI jobs. The T3 hardware supports an xon/xoff-type
flow control mechanism to help avoid overflowing the HW doorbell FIFO.
This patch uses these interrupts to disable RDMA QP doorbell rings
when we near an overflow condition, and then turn them back on (and
ring all the active QP doorbells) when when the doorbell FIFO empties
out. In addition if an doorbell ring is dropped by the hardware, the
code will now recover.
Design:
cxgb3:
- enable these DB interrupts
- in the interrupt handler, schedule work tasks to call the ULPs event
handlers with the new events.
- ring all the qset txqs when an overflow is detected.
iw_cxgb3:
- disable db ringing on all active qps when we get the DB_FULL event
- enable db ringing on all active qps and ring all active dbs when we get
the DB_EMPTY event
- On DB_DROP event:
- disable db rings in the event handler
- delay-schedule a work task which rings and enables the dbs on
all active qps.
- in post_send and post_recv logic, don't ring the db if it's disabled.

The driver is expected to report that the link is up
when the phy Rx signal is established and the mac
has not detected a link fault.
The code is however broken, the driver does not check the link fault
status when the phy link status changes.
The link fault status being checked within a short period of time,
it leads to link up/link down events.

Override the mac index computation for the gen2 adapter,
as each port is expected to use index 0.
introduces a regression on 2 port 1G adapter
as its xauicfg vpd value is null.

Do not call t3_link_fault() under spinlock, as it calls msleep().
Besides, only the access to pi->link_fault needs to be serialized.
Also initialize local variables before checking the link status,
link state fields might otherwise end up containing garbage.

The driver currently ignores the local or remote link faults
raised at the mac layer. This patch fixes it.
Our mac however only advertizes link events, so wait for the
phy to stabilize the link, then enable mac link events interrupts.

Under RX pressure, The HW might generate a high load of interrupts
to signal mac fifo or free lists overflow.
Disable the interrupts, and poll the relevant status bits
to maintain stats.

The NIC driver can work with mutliple versions of the FW.
Let the driver load when the embedded FW does not match,
and the FW update mechanism failed.
The iWARP module will make its own loading decision.

Do not require PHY interrupts to be connected to GPIs in ascending order.
Base interrupt availability both on PHYs supporting them and on GPIs being
hooked up. Allows boards to specify interrupt GPIs though the PHYs don't
use them.
Remove spurious PHY interrupts due to clearing T3DBG interrupts before
setting their polarity.

When PCI error recovery was added to cxgb3, a function t3_io_slot_reset()
was added. This function can call back into t3_prep_adapter() at any
time, so t3_prep_adapter() can no longer be marked __devinit.
This patch removes the __devinit annotation from t3_prep_adapter() and
all the functions that it calls, which fixes
