Matthew Wilcox 353dfe1290 [PARISC] Fix PCI bus numbering in the presence of Cardbus bridges
Firmware ignores Cardbus bridges, so when Linux assigns bus numbers,
it must override firmware's notion of what the busses are numbered.
This is not sufficient to support the Cardbus bridge as there is also
no interrupt routing table entry for them.

David Howells 7d12e780e0 IRQ: Maintain regs pointer globally rather than passing to IRQ handlers
Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.

The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around.  On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).

Where appropriate, an arch may override the generic storage facility and do
something different with the variable.  On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.

Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions.  Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller.  A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.

I've build this code with allyesconfig for x86_64 and i386.  I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.

This will affect all archs.  Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

	struct pt_regs *old_regs = set_irq_regs(regs);

And put the old one back at the end:

	set_irq_regs(old_regs);

Don't pass regs through to generic_handle_irq() or __do_IRQ().

In timer_interrupt(), this sort of change will be necessary:

	-	update_process_times(user_mode(regs));
	-	profile_tick(CPU_PROFILING, regs);
	+	update_process_times(user_mode(get_irq_regs()));
	+	profile_tick(CPU_PROFILING);

I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().

Some notes on the interrupt handling in the drivers:

 (*) input_dev() is now gone entirely.  The regs pointer is no longer stored in
     the input_dev struct.

 (*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking.  It does
     something different depending on whether it's been supplied with a regs
     pointer or not.

 (*) Various IRQ handler function pointers have been moved to type
     irq_handler_t.

James Bottomley 7085689ed1 [PARISC] Allow nested interrupts
Our prior mode of operation didn't allow nested interrupts
because it makes the interrupt code much simpler.  However,
nested interrupts are better for latency.

This code uses the EIEM register to simulate level interrupts
and thus achieve nesting.

Cedric Le Goater 9ec52099e4 [PATCH] replace cad_pid by a struct pid
There are a few places in the kernel where the init task is signaled.  The
ctrl+alt+del sequence is one them.  It kills a task, usually init, using a
cached pid (cad_pid).

This patch replaces the pid_t by a struct pid to avoid pid wrap around
problem.  The struct pid is initialized at boot time in init() and can be
modified through systctl with

	/proc/sys/kernel/cad_pid

[ I haven't found any distro using it ? ]

It also introduces a small helper routine kill_cad_pid() which is used
where it seemed ok to use cad_pid instead of pid 1.

Serge E. Hallyn 96b644bdec [PATCH] namespaces: utsname: use init_utsname when appropriate
In some places, particularly drivers and __init code, the init utsns is the
appropriate one to use.  This patch replaces those with a the init_utsname
helper.

Changes: Removed several uses of init_utsname().  Hope I picked all the
	right ones in net/ipv4/ipconfig.c.  These are now changed to
	utsname() (the per-process namespace utsname) in the previous
	patch (2/7)

Christoph Lameter f8891e5e1f [PATCH] Light weight event counters
The remaining counters in page_state after the zoned VM counter patches
have been applied are all just for show in /proc/vmstat.  They have no
essential function for the VM.

We use a simple increment of per cpu variables.  In order to avoid the most
severe races we disable preempt.  Preempt does not prevent the race between
an increment and an interrupt handler incrementing the same statistics
counter.  However, that race is exceedingly rare, we may only loose one
increment or so and there is no requirement (at least not in kernel) that
the vm event counters have to be accurate.

In the non preempt case this results in a simple increment for each
counter.  For many architectures this will be reduced by the compiler to a
single instruction.  This single instruction is atomic for i386 and x86_64.
 And therefore even the rare race condition in an interrupt is avoided for
both architectures in most cases.

The patchset also adds an off switch for embedded systems that allows a
building of linux kernels without these counters.

The implementation of these counters is through inline code that hopefully
results in only a single instruction increment instruction being emitted
(i386, x86_64) or in the increment being hidden though instruction
concurrency (EPIC architectures such as ia64 can get that done).

Benefits:
- VM event counter operations usually reduce to a single inline instruction
  on i386 and x86_64.
- No interrupt disable, only preempt disable for the preempt case.
  Preempt disable can also be avoided by moving the counter into a spinlock.
- Handling is similar to zoned VM counters.
- Simple and easily extendable.
- Can be omitted to reduce memory use for embedded use.

Ingo Molnar d1bef4ed5f [PATCH] genirq: rename desc->handler to desc->chip
This patch-queue improves the generic IRQ layer to be truly generic, by adding
various abstractions and features to it, without impacting existing
functionality.

While the queue can be best described as "fix and improve everything in the
generic IRQ layer that we could think of", and thus it consists of many
smaller features and lots of cleanups, the one feature that stands out most is
the new 'irq chip' abstraction.

The irq-chip abstraction is about describing and coding and IRQ controller
driver by mapping its raw hardware capabilities [and quirks, if needed] in a
straightforward way, without having to think about "IRQ flow"
(level/edge/etc.) type of details.

This stands in contrast with the current 'irq-type' model of genirq
architectures, which 'mixes' raw hardware capabilities with 'flow' details.
The patchset supports both types of irq controller designs at once, and
converts i386 and x86_64 to the new irq-chip design.

As a bonus side-effect of the irq-chip approach, chained interrupt controllers
(master/slave PIC constructs, etc.) are now supported by design as well.

The end result of this patchset intends to be simpler architecture-level code
and more consolidation between architectures.

We reused many bits of code and many concepts from Russell King's ARM IRQ
layer, the merging of which was one of the motivations for this patchset.

This patch:

rename desc->handler to desc->chip.

Originally i did not want to do this, because it's a big patch.  But having
both "desc->handler", "desc->handle_irq" and "action->handler" caused a
large degree of confusion and made the code appear alot less clean than it
truly is.

I have also attempted a dual approach as well by introducing a
desc->chip alias - but that just wasnt robust enough and broke
frequently.

So lets get over with this quickly.  The conversion was done automatically
via scripts and converts all the code in the kernel.

This renaming patch is the first one amongst the patches, so that the
remaining patches can stay flexible and can be merged and split up
without having some big monolithic patch act as a merge barrier.

Thibaut Varene 3f9edb53f7 [PARISC] pdc_stable version 0.30
pdc_stable v0.30:

This patch introduces 3 more files to the /sys/firmware/stable tree:
- diagnostic, which contains a cryptic hex string
- osdep1, a 16 bytes os-dependent storage area always available
- osdep2, another os-dependent storage area which existence/size depends
on hversion.

This patch also adds code to setup the "Linux" signature in stable
storage. That is to say that starting with this patch, the kernel will
now sign its OSID (0x0006, thx LaMont) in Stable Storage upon boot,
whether pdc_stable is enabled or not.

Thibaut Varene 8ffaeaf42e [PARISC] PDC_CHASSIS is implemented on all machines
This patch removes a limitation of the original code, so that CHASSIS
codes can be sent to all machines. On machines with a LCD panel, this
code displays "INI" during bootup, "RUN" when the system is booted and
running, "FLT" when a panic occurs, etc.

This part of the code can be enabled/disabled through CONFIG_PDC_CHASSIS

This patch also adds minimalistic support for Chassis warnings, through
a proc entry '/proc/chassis', which will reflect the warnings status (PSU
or fans failure when they happen, NVRAM battery level and temperature
thresholds overflows).

This part of the code can be enabled/disabled through CONFIG_PDC_CHASSIS_WARN

Grant Grundler b312c33e36 [PARISC] Document that we tolerate "Relaxed Ordering"
This means "DMA Read returns" can bypass "MMIO Writes".
Violating the PCI specs in this case improves outbound DMA "flows"
and is currently not required by any drivers.

This is NOT a new behavior. Previous chipsets did this
already and I believe ZX1 PDC was already setting this
for hpux. I just want to further document the behavior.

Alan Stern e041c68341 [PATCH] Notifier chain update: API changes
The kernel's implementation of notifier chains is unsafe.  There is no
protection against entries being added to or removed from a chain while the
chain is in use.  The issues were discussed in this thread:

We noticed that notifier chains in the kernel fall into two basic usage
classes:

	"Blocking" chains are always called from a process context
	and the callout routines are allowed to sleep;

	"Atomic" chains can be called from an atomic context and
	the callout routines are not allowed to sleep.

We decided to codify this distinction and make it part of the API.  Therefore
this set of patches introduces three new, parallel APIs: one for blocking
notifiers, one for atomic notifiers, and one for "raw" notifiers (which is
really just the old API under a new name).  New kinds of data structures are
used for the heads of the chains, and new routines are defined for
registration, unregistration, and calling a chain.  The three APIs are
explained in include/linux/notifier.h and their implementation is in
kernel/sys.c.

With atomic and blocking chains, the implementation guarantees that the chain
links will not be corrupted and that chain callers will not get messed up by
entries being added or removed.  For raw chains the implementation provides no
guarantees at all; users of this API must provide their own protections.  (The
idea was that situations may come up where the assumptions of the atomic and
blocking APIs are not appropriate, so it should be possible for users to
handle these things in their own way.)

There are some limitations, which should not be too hard to live with.  For
atomic/blocking chains, registration and unregistration must always be done in
a process context since the chain is protected by a mutex/rwsem.  Also, a
callout routine for a non-raw chain must not try to register or unregister
entries on its own chain.  (This did happen in a couple of places and the code
had to be changed to avoid it.)

Since atomic chains may be called from within an NMI handler, they cannot use
spinlocks for synchronization.  Instead we use RCU.  The overhead falls almost
entirely in the unregister routine, which is okay since unregistration is much
less frequent that calling a chain.

Here is the list of chains that we adjusted and their classifications.  None
of them use the raw API, so for the moment it is only a placeholder.

It's possible that some of these classifications are wrong.  If they are,
please let us know or submit a patch to fix them.  Note that any chain that
gets called very frequently should be atomic, because the rwsem read-locking
used for blocking chains is very likely to incur cache misses on SMP systems.
(However, if the chain's callout routines may sleep then the chain cannot be
atomic.)

The patch set was written by Alan Stern and Chandra Seetharaman, incorporating
material written by Keith Owens and suggestions from Paul McKenney and Andrew
Morton.

Kyle McMartin 85509c0007 [PARISC] Add chassis_power_off routine
Define a chassis_power_off routine that machines which have a way
to turn off the power supply can hook into. Formerly they were
using pm_power_off, which is now being used by generic code. Make
lasi.c use chassis_power_off instead of pm_power_off.

Note, all machines need to call machine_power_off so that the
switch can power off the machine, though halt -p may not necessarily
be able to work properly on the machine.

James Bottomley c2ab64d098 [PARISC] Add IRQ affinities
This really only adds them for the machines I can check SMP on, which
is CPU interrupts and IOSAPIC (so not any of the GSC based machines).

With this patch, irqbalanced can be used to maintain irq balancing.
Unfortunately, irqbalanced is a bit x86 centric, so it doesn't do an
incredibly good job, but it does work.

Matthew Wilcox 92b919fe46 [PARISC] Update dino from parisc tree
Fix card-mode Dino crashes on 725 (and probably other Snake) systems.
Dino was coming up in fatal mode after a warm reboot.  Resetting Dino
brings it out of fatal mode, so do that if the status register indicates
we're in fatal mode.  Since this was never observed on any later systems,
I presume firmware does this for us on those.

2.6.14-rc2-pa2 - make SBA more anal about invalidating pdir entries

Previous code cleared the valid flag a pdir entry but it did NOT
guarantee this change was visible to the PDIR before writing
the PCOM register. Ie the SBA could pick up a stale entry if
the write happened to hit the SBA before the cacheline was flushed
from the cache.

Long term, I think I want to make this a compile time flag.
Developement tree should enable anal pdir checking by default
and Debian can disable it with either a CONFIG option
or one-line patch. fdc/sync options can only negatively affect
performance though I haven't measure how much yet.
If someone can run netperf TCP_RR across gige and compare
-pa1 and -pa2, that would be sufficient.

Cleaned up the use of "fdc" to make sure it's using "kernel"
space id (specify sr0 but maps to sr4-7). It seems a bit fragile
to assume "sr1" gets loaded with KERNEL_SPACE which is how the
code works today.

Tested on 32 and 64-bit SMP kernels on j6k.

2.6.12-rc4-pa2  fix 32-bit support for Astro platforms
o Since my last SBA code change, SBA could allocate more than 1GB of IOVA
  space on Astro boxes with more than 1GB of RAM when running 32-bit kernel.
  This is bad since IOMMU can only talk to the first 1GB at most.
  Kudos to jejb for quickly spotting that bug.

o jejb also noted SBA should *always* reject DMA masks > 32-bits since
  DMA-mapping.txt indicates caller should try again with 32-bits.

o off-by-one error when comparing the mask to IOVA space size.

Convert pa_dev->hpa from an unsigned long to a struct resource.
Use insert_resource() instead of request_mem_region().
Request resources at bus walk time instead of driver probe time.
Don't release the resources as we don't have any hotplug parisc_device
support yet.
Add parisc_pathname() to conveniently get the textual representation
of the hwpath used in sysfs.
Inline the remnants of claim_device() into its caller.

I noticed that some of the STI regions weren't showing up in iomem.
Reading the STI spec indicated that all STI devices occupy at least 32MB.
So check for STI HPAs and give them 32MB instead of 4kB.

Herbert Xu e5ed639913 [IPV4]: Replace __in_dev_get with __in_dev_get_rcu/rtnl
The following patch renames __in_dev_get() to __in_dev_get_rtnl() and
introduces __in_dev_get_rcu() to cover the second case.

1) RCU with refcnt should use in_dev_get().
2) RCU without refcnt should use __in_dev_get_rcu().
3) All others must hold RTNL and use __in_dev_get_rtnl().

There is one exception in net/ipv4/route.c which is in fact a pre-existing
race condition.  I've marked it as such so that we remember to fix it.

This patch is based on suggestions and prior work by Suzanne Wood and
Paul McKenney.

Rajesh Shah c431ada45d [PATCH] acpi bridge hotadd: ACPI based root bridge hot-add
When you hot-plug a (root) bridge hierarchy, it may have p2p bridges and
devices attached to it that have not been configured by firmware.  In this
case, we need to configure the devices before starting them.  This patch
separates device start from device scan so that we can introduce the
configuration step in the middle.

I kept the existing semantics for pci_scan_bus() since there are a huge number
of callers to that function.

Also, I have no way of testing the changes I made to the parisc files, so this
needs review by those folks.  Sorry for the massive cross-post, this touches
files in many different places.

Linus Torvalds 1da177e4c3 Linux-2.6.12-rc2
Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
