Many users of debugfs copy the implementation of default_open() when
they want to support a custom read/write function op. This leads to a
proliferation of the default_open() implementation across the entire
tree.
Now that the common implementation has been consolidated into libfs we
can replace all the users of this function with simple_open().

There has long been a limitation using software breakpoints with a
kernel compiled with CONFIG_DEBUG_RODATA going back to 2.6.26. For
this particular patch, it will apply cleanly and has been tested all
the way back to 2.6.36.
The kprobes code uses the text_poke() function which accommodates
writing a breakpoint into a read-only page. The x86 kgdb code can
solve the problem similarly by overriding the default breakpoint
set/remove routines and using text_poke() directly.
The x86 kgdb code will first attempt to use the traditional
probe_kernel_write(), and next try using a the text_poke() function.
The break point install method is tracked such that the correct break
point removal routine will get called later on.

The do_fork and sys_open tests have never worked properly on anything
other than a UP configuration with the kgdb test suite. This is
because the test suite did not fully implement the behavior of a real
debugger. A real debugger tracks the state of what thread it asked to
single step and can correctly continue other threads of execution or
conditionally stop while waiting for the original thread single step
request to return.
Below is a simple method to cause a fatal kernel oops with the kgdb
test suite on a 2 processor ARM system:

The kernel will eventually recovers, but the test suite has completely
failed to test anything useful.
This patch implements behavior similar to a real debugger that does
not rely on hardware single stepping by using only software planted
breakpoints.
In order to mimic a real debugger, the kgdb test suite now tracks the
most recent thread that was continued (cont_thread_id), with the
intent to single step just this thread. When the response to the
single step request stops in a different thread that hit the original
break point that thread will now get continued, while the debugger
waits for the thread with the single step pending. Here is a high
level description of the sequence of events.

happens sometimes because breakpoints are used in place of single
stepping and some threads could have been in the debugger exception
handling queue because breakpoints were hit concurrently on
different CPUs. This also means we wait at least one second before
unplumbing the debugger connection at the very end, so as respond
to any debug threads waiting to be serviced.

The warn will turn to a hard kernel crash shortly after that because
the pc will not get properly rewound to the right value after hitting
a breakpoint leading to a hard lockup.
This change is broken up into 2 pieces because archs that have hw
single stepping (2.6.26 and up) need different changes than archs that
do not have hw single stepping (3.0 and up). This change implements
the correct behavior for an arch that supports hw single stepping.
A minor defect was fixed where sys_open should be do_sys_open
for the sys_open break point test. This solves the problem of running
a 64 bit with a 32 bit user space. The sys_open() never gets called
when using the 32 bit file system for the kgdb testsuite because the
32 bit binaries invoke the compat_sys_open() call leading to the test
never completing.

On x86 the kgdb test suite will oops when the kernel is compiled with
CONFIG_DEBUG_RODATA and you run the tests after boot time. This is
regression has existed since 2.6.26 by commit: b33cb815 (kgdbts: Use
HW breakpoints with CONFIG_DEBUG_RODATA).
The test suite can use hw breakpoints for all the tests, but it has to
execute the hardware breakpoint specific tests first in order to
determine that the hw breakpoints actually work. Specifically the
very first test causes an oops:

stuff like switch_to() that might require MMU definitions.

bit more nasty than usual as in touching things outside of
arch/powerpc mostly due to the big iSeriesectomy :-) We finally got
rid of the bugger (legacy iSeries support) which was a PITA to
maintain and that nobody really used anymore.
Here are some of the highlights:
- Legacy iSeries is gone. Thanks Stephen ! There's still some bits
and pieces remaining if you do a grep -ir series arch/powerpc but
they are harmless and will be removed in the next few weeks
hopefully.
- The 'fadump' functionality (Firmware Assisted Dump) replaces the
previous (equivalent) "pHyp assisted dump"... it's a rewrite of a
mechanism to get the hypervisor to do crash dumps on pSeries, the
new implementation hopefully being much more reliable. Thanks

- The "EEH" code (pSeries PCI error handling & recovery) got a big
spring cleaning, motivated by the need to be able to implement a
new backend for it on top of some new different type of firwmare.
The work isn't complete yet, but a good chunk of the cleanups is

All num, magic and owner are set by alloc_tty_driver. No need to
re-set them on each allocation site.
pti driver sets something different to what it passes to
alloc_tty_driver. It is not a bug, since we don't use the lines
parameter in any way. Anyway this is fixed, and now we do the right
thing.

Some SoC have a 32 bit variant of Timer Counter Blocks. We do not
need the chaining of two 16 bit counters anymore for them.
The SoC nature is deduced from the device tree "compatible" string.
For non-device-tree configurations, backward compatibility is maintained
by using the default 16 bit counter configuration.
This patch addresses both the atmel_tclib and its user: tcb_clksrc
clocksource.

Device tree support added to atmel_tclib: the generic Timer Counter
library. This is used by the clocksource/clockevent driver tcb_clksrc.
The current DT enabled platforms are also modified to use it:
- .dtsi files are modified to add Timer Counter Block entries
- alias are created to allow identification of each block
- clkdev lookup tables are added for clocks identification.

Requesting iomem region and ioremaping is now done using
the resource size specified instead of a constant value.
Each <SoC>_device.c file is modified accordingly to reflect
actual user interface size.

When the system is under heavy load, we occasionally saw a problem where
the system would get a legitimate interrupt when they should be
disabled.
This was caused by the data_dma_cb() DMA callback unconditionally
re-enabling FPGA interrupts even when data dumping is disabled. When
data dumping was re-enabled, the irq handler would fire while a DMA was
in progress. The "BUG_ON(priv->inflight != NULL);" during the second
invocation of the DMA callback caused the system to crash.
To fix the issue, the priv->enabled boolean is moved under the
protection of the priv->lock spinlock. The DMA callback checks the
boolean to know whether to re-enable FPGA interrupts before it returns.
Now that it is fixed, the driver keeps FPGA interrupts disabled when it
expects that they are disabled, fixing the bug.

Lockdep occasionally complains with the message:

This is caused by calling videobuf_dma_unmap() under spin_lock_irq(). To
fix the warning, we drop the lock before unmapping and freeing the
buffer.

It's time to remove the int/unsigned int option. For this version
it'll simply give a warning, but it'll break next kernel version.

module_param_array(), unlike its non-array cousins, didn't check the type
of the variable. Fixing this found two bugs.

The 'switch' sysfs attribute is used for
showing and setting the switch state of the device.
However, setting it from the userspace focibly is not appropriate.
And showing the state can be handled from
other places (e.g. external connector) in a unified manner.
This patch removes the sysfs attribute.
Plus, uevent messages for interrupts are also removed.

This moves all the header files related to the abx500 family into
a common include directory below mfd. From now on we place any
subchip header in that directory. Headers previously in e.g.
<linux/mfd/ab8500/gpio.h> get prefixed and are now e.g.
<linux/mfd/abx500/ab8500-gpio.h>. The top-level abstract interface
remains in <linux/mfd/abx500.h>.
