d07f4a8200 usb: phy: mv-usb: remove incorrect __exit_p annotation
When platform_driver_probe() is not used, bind/unbind via sysfs
is enabled.  Thus, __exit_p annotation should be removed from
remove().

Also, mv_otg_remove() is staticized, because this function is
used only in this file. Fix the following sparse warning:

drivers/usb/phy/phy-mv-usb.c:656:5: warning: symbol 'mv_otg_remove' was not declared. Should it be static?

19f9e188de usb: phy: use dev_get_platdata()

c21b7eeaee usb: phy: mv-usb: Remove redundant platform_set_drvdata()
Commit 0998d06310 (device-core: Ensure drvdata = NULL when no
driver is bound) removes the need to set driver data field to
NULL.

df18fedae5 usb: otg: mv_otg: remove unused clock
The origianl understanding of clock is wrong. The OTG controller
only have one clock input.
Passing clock name by pdata is wrong. The clock is defined by device
iteself.

94ae98433a usb: phy: rename all phy drivers to phy-$name-usb.c
this will make sure that we have sensible names
for all phy drivers. Current situation was already
quite bad with too generic names being used.
