Chris Wright 5d990b6275 PCI: add pci_request_acs
Commit ae21ee65e8 "PCI: acs p2p upsteram
forwarding enabling" doesn't actually enable ACS.

Add a function to pci core to allow an IOMMU to request that ACS
be enabled.  The existing mechanism of using iommu_found() in the pci
core to know when ACS should be enabled doesn't actually work due to
initialization order;  iommu has only been detected not initialized.

Have Intel and AMD IOMMUs request ACS, and Xen does as well during early
init of dom0.

Kenji Kaneshige b26a34aa47 PCI: fix BUG_ON triggered by logical PCIe root port removal
This problem happened when removing PCIe root port using PCI logical
hotplug operation.

The immediate cause of this problem is that the pointer to invalid
data structure is passed to pcie_update_aspm_capable() by
pcie_aspm_exit_link_state(). When pcie_aspm_exit_link_state() received
a pointer to root port link, it unconfigures the root port link and
frees its data structure at first. At this point, there are not links
to configure under the root port and the data structure for root port
link is already freed. So pcie_aspm_exit_link_state() must not call
pcie_update_aspm_capable() and pcie_config_aspm_path().

This patch fixes the problem by changing pcie_aspm_exit_link_state()
not to call pcie_update_aspm_capable() and pcie_config_aspm_path() if
the specified link is root port link.

Andrew Patterson 6cdfd995a6 PCI: unconditionally clear AER uncorr status register during cleanup
The current implementation of pci_cleanup_aer_uncorrect_error_status
only clears either fatal or non-fatal error status bits depending
on the state of the I/O channel. This implementation will then often
leave some bits set after PCI error recovery completes.  The uncleared bit
settings will then be falsely reported the next time an AER interrupt is
generated for that hierarchy. An easy way to illustrate this issue is to
use the aer-inject module to simultaneously inject both an uncorrectable
non-fatal and uncorrectable fatal error.  One of the errors will not be
cleared.

This patch resolves this issue by unconditionally clearing all bits in
the AER uncorrectable status register. All settings and corrective action
strategies are saved and determined before
pci_cleanup_aer_uncorrect_error_status is called, so this change should not
affect errory handling functionality.

Kenji Kaneshige 694f88ef7a PCI: portdrv: remove unnecessary struct pcie_port_data
Remove 'port_type' field in struct pcie_port_data(), because we can
get port type information from struct pci_dev. With this change, this
patch also does followings:

 - Remove struct pcie_port_data because it no longer has any field.
 - Remove portdrv private definitions about port type (PCIE_RC_PORT,
   PCIE_SW_UPSTREAM_PORT and PCIE_SW_DOWNSTREAM_PORT), and use generic
   definitions instead.

Kenji Kaneshige dc5351784e PCI: portdrv: cleanup service irqs initialization
This patch cleans up the service irqs initialization as follows:

 - Remove 'irq_mode' field in pcie_port_data and related definitions,
   which is not needed because we can get the same information from
   'is_msix', 'is_msi' and 'pin' fields in struct pci_dev.

 - Change the name of 'vectors' argument of assign_interrupt_mode() to
   'irqs' because it holds irq numbers actually. People might confuse
   it with CPU vector or MSI/MSI-X vector.

 - Change function name assign_interrupt_mode() to init_service_irqs()
   becasuse we no longer have 'irq_mode' data structure, and new name
   is more straightforward (IMO).

Alex Williamson 59353ea30e PCI: Always set prefetchable base/limit upper32 registers
Prior to 1f82de10 we always initialized the upper 32bits of the
prefetchable memory window, regardless of the address range used.
Now we only touch it for a >32bit address, which means the upper32
registers remain whatever the BIOS initialized them too.

It's valid for the BIOS to set the upper32 base/limit to
0xffffffff/0x00000000, which makes us program prefetchable ranges
like 0xffffffffabc00000 - 0x00000000abc00000

Revert the chunk of 1f82de10 that made this conditional so we always
write the upper32 registers and remove now unused pref_mem64 variable.

Shmulik Ravid 04b55c4732 PCI: read-modify-write the pcie device control register when initiating pcie flr
The pcie_flr routine writes the device control register with the FLR bit
set clearing all other fields for the FLR duration. Among other fields,
the Max_Payload_Size is also cleared which can cause errors if there are
transactions lurking in the HW pipeline. The patch replaces the blank
write with read-modify-write of the control register keeping the other
fields intact.

Lin Ming 2263576cfc ACPICA: Add post-order callback to acpi_walk_namespace
The existing interface only has a pre-order callback. This change
adds an additional parameter for a post-order callback which will
be more useful for bus scans. ACPICA BZ 779.

Also update the external calls to acpi_walk_namespace.

Kenji Kaneshige 5651c48cfa PCI pciehp: fix power fault interrupt storm problem
Enabling power fault detected event notification in current pciehp
might cause power fault interrupt storm on some machines. On those
machines. On those machines, power fault detected bit in the slot
status register was set again immediately when it is cleared in the
interrupt service routine, and next power fault detected interrupt was
notified again. Therefore, disable power fault detected event
notification for now.

This patch also removes unnecessary handling for power fault cleared
event because this event is not supported by PCIe spec.

David Woodhouse 5854d9c8d1 Fix handling of the HP/Acer 'DMAR at zero' BIOS error for machines with <4GiB RAM.
Commit 86cf898e1d ("intel-iommu: Check for
'DMAR at zero' BIOS error earlier.") was supposed to work by pretending
not to detect an IOMMU if it was actually being reported by the BIOS at
physical address zero.

However, the intel_iommu_init() function is called unconditionally, as
are the corresponding functions for other IOMMU hardware.

So the patch only worked if you have RAM above the 4GiB boundary. It
caused swiotlb to be initialised when no IOMMU was detected during early
boot, and thus the later IOMMU init would refuse to run.

But if you have less RAM than that, swiotlb wouldn't get set up and the
IOMMU _would_ still end up being initialised, even though we never
claimed to detect it.

This patch also sets the dmar_disabled flag when the error is detected
during the initial detection phase -- so that the later call to
intel_iommu_init() will return without doing anything, regardless of
whether swiotlb is used or not.

Fenghua Yu 99dcadede4 intel-iommu: Support PCIe hot-plug
To support PCIe hot plug in IOMMU, we register a notifier to respond to device
change action.

When the notifier gets BUS_NOTIFY_UNBOUND_DRIVER, it removes the device
from its DMAR domain.

A hot added device will be added into an IOMMU domain when it first does IOMMU
op. So there is no need to add more code for hot add.

Without the patch, after a hot-remove, a hot-added device on the same
slot will not work.

Alex Williamson e8bb910d1b intel-iommu: Obey coherent_dma_mask for alloc_coherent on passthrough
The model for IOMMU passthrough is that decent devices that can cope
with DMA to all of memory get passthrough; crappy devices with a limited
dma_mask don't -- they get to use the IOMMU anyway.

This is done on the basis that IOMMU passthrough is usually wanted for
performance reasons, and it's only the decent PCI devices that you
really care about performance for, while the crappy 32-bit ones like
your USB controller can just use the IOMMU and you won't really care.

Unfortunately, the check for this was only looking at dev->dma_mask, not
at dev->coherent_dma_mask. And some devices have a 32-bit
coherent_dma_mask even though they have a full 64-bit dma_mask.

Even more unfortunately, fixing that simple oversight would upset
certain broken HP devices. Not only do they have a 32-bit
coherent_dma_mask, but they also have a tendency to do stray DMA to
unmapped addresses. And then they die when they take the DMA fault they
so richly deserve.

So if we do the 'correct' fix, it'll mean that affected users have to
disable IOMMU support completely on "a large percentage of servers from
a major vendor."

Personally, I have little sympathy -- given that this is the _same_
'major vendor' who is shipping machines which claim to have IOMMU
support but have obviously never _once_ booted a VT-d capable OS to do
any form of QA. But strictly speaking, it _would_ be a regression even
though it only ever worked by fluke.

For 2.6.33, we'll come up with a quirk which gives swiotlb support
for this particular device, and other devices with an inadequate
coherent_dma_mask will just get normal IOMMU mapping.

The simplest fix for 2.6.32, though, is just to jump through some hoops
to try to allocate coherent DMA memory for such devices in a place that
they can reach. We'd use dma_generic_alloc_coherent() for this if it
existed on IA64.

Linus Torvalds 8c8def26bf PCI: allow matching of prefetchable resources to non-prefetchable windows
I'm not entirely sure it needs to go into 32, but it's probably the right
thing to do. Another way of explaining the patch is:

 - we currently pick the _first_ exactly matching bus resource entry, but
   the _last_ inexactly matching one. Normally first/last shouldn't
   matter, but bus resource entries aren't actually all created equal: in
   a transparent bus, the last resources will be the parent resources,
   which we should generally try to avoid unless we have no choice. So
   "first matching" is the thing we should always aim for.

 - the patch is a bit bigger than it needs to be, because I simplified the
   logic at the same time. It used to be a fairly incomprehensible

	if ((res->flags & IORESOURCE_PREFETCH) && !(r->flags & IORESOURCE_PREFETCH))
		best = r;       /* Approximating prefetchable by non-prefetchable */

   and technically, all the patch did was to make that complex choice be
   even more complex (it basically added a "&& !best" to say that if we
   already gound a non-prefetchable window for the prefetchable resource,
   then we won't override an earlier one with that later one: remember
   "first matching").

 - So instead of that complex one with three separate conditionals in one,
   I split it up a bit, and am taking advantage of the fact that we
   already handled the exact case, so if 'res->flags' has the PREFETCH
   bit, then we already know that 'r->flags' will _not_ have it. So the
   simplified code drops the redundant test, and does the new '!best' test
   separately. It also uses 'continue' as a way to ignore the bus
   resource we know doesn't work (ie a prefetchable bus resource is _not_
   acceptable for anything but an exact match), so it turns into:

	/* We can't insert a non-prefetch resource inside a prefetchable parent .. */
	if (r->flags & IORESOURCE_PREFETCH)
		continue;
	/* .. but we can put a prefetchable resource inside a non-prefetchable one */
	if (!best)
		best = r;

   instead. With the comments, it's now six lines instead of two, but it's
   conceptually simpler, and I _could_ have written it as two lines:

	if ((res->flags & IORESOURCE_PREFETCH) && !best)
		best = r;	/* Approximating prefetchable by non-prefetchable */

   but I thought that was too damn subtle.

FUJITA Tomonori 75f1cdf1dd x86: Handle HW IOMMU initialization failure gracefully
If HW IOMMU initialization fails (Intel VT-d often does this,
typically due to BIOS bugs), we fall back to nommu. It doesn't
work for the majority since nowadays we have more than 4GB
memory so we must use swiotlb instead of nommu.

The problem is that it's too late to initialize swiotlb when HW
IOMMU initialization fails. We need to allocate swiotlb memory
earlier from bootmem allocator. Chris explained the issue in
detail:

The current x86 IOMMU initialization sequence is too complicated
and handling the above issue makes it more hacky.

This patch changes x86 IOMMU initialization sequence to handle
the above issue cleanly.

The new x86 IOMMU initialization sequence are:

1. we initialize the swiotlb (and setting swiotlb to 1) in the case
   of (max_pfn > MAX_DMA32_PFN && !no_iommu). dma_ops is set to
   swiotlb_dma_ops or nommu_dma_ops. if swiotlb usage is forced by
   the boot option, we finish here.

2. we call the detection functions of all the IOMMUs

3. the detection function sets x86_init.iommu.iommu_init to the
   IOMMU initialization function (so we can avoid calling the
   initialization functions of all the IOMMUs needlessly).

4. if the IOMMU initialization function doesn't need to swiotlb
   then sets swiotlb to zero (e.g. the initialization is
   sucessful).

5. if we find that swiotlb is set to zero, we free swiotlb
   resource.

David Woodhouse 86cf898e1d intel-iommu: Check for 'DMAR at zero' BIOS error earlier.
Chris Wright has some patches which let us fall back to swiotlb nicely
if IOMMU initialisation fails. But those are a bit much for 2.6.32.

Instead, let's shift the check for the biggest problem, the HP and Acer
BIOS bug which reports a DMAR at physical address zero. That one can
actually be checked much earlier -- before we even admit to having
detected an IOMMU in the first place. So the swiotlb init goes ahead as
we want.

Andreas Herrmann e0cd516034 PCI: derive nearby CPUs from device's instead of bus' NUMA information
In case of AMD CPU northbridge functions this NUMA information might
differ.  Here is an example from a 4-socket system.

Currently Linux shows

  root@hagen:/sys/devices/pci0000:00/0000:00:1a.4# cat local_cpu*

which is not correct for northbridge functions as the local CPUs
are those of the same socket.

With this patch and a quirk for AMD CPU NB functions Linux can
do better and correctly show

Kenji Kaneshige 761434a318 PCI ASPM: fix oops on root port removal
Fix the following BUG_ON() problem reported by Alex Chiang.

This problem happened when removing PCIe root port using PCI logical
hotplug operation.

The immediate cause of this problem is that the pointer to invalid
data structure is passed to pcie_update_aspm_capable() by
pcie_aspm_exit_link_state(). When pcie_aspm_exit_link_state() received
a pointer to root port link, it unconfigures the root port link and
frees its data structure at first. At this point, there are not links
to configure under the root port and the data structure for root port
link is already freed. So pcie_aspm_exit_link_state() must not call
pcie_update_aspm_capable() and pcie_config_aspm_path().

This patch fixes the problem by changing pcie_aspm_exit_link_state()
not to call pcie_update_aspm_capable() and pcie_config_aspm_path() if
the specified link is root port link.

Kenji Kaneshige 0efea00063 PCI: cache PCIe capability offset
There are a lot of codes that searches PCI express capability offset
in the PCI configuration space using pci_find_capability(). Caching it
in the struct pci_dev will reduce unncecessary search. This patch adds
an additional 'pcie_cap' fields into struct pci_dev, which is
initialized at pci device scan time (in set_pcie_port_type()).

Bjorn Helgaas 204d49a561 PCI hotplug: move IOAPIC support from acpiphp to ioapic driver
This patch moves PCI I/O APIC support from acpiphp to a separate driver.

Like pciehp and shpchp, acpiphp handles PCI hotplug, i.e., addition and
removal of PCI adapters.  But in addition, acpiphp handles some ACPI
hotplug, such as the addition of new host bridges, and the I/O APIC
support was tangled up with that.

I don't think the I/O APIC support needs to be in acpiphp; PCI I/O APICs
usually appear as a function on a PCI host bridge, and we'll enumerate the
APIC before any of the devices behind the bridge that use it.

As far as I know, nobody actually uses I/O APIC hotplug.  It depends on
acpi_register_ioapic(), which is only implemented for ia64, and I don't
think any vendors have supported I/O chassis hotplug yet.

Andrew Patterson 3c299dc226 PCI: add pci_get_domain_bus_and_slot function
Added the pci_get_domain_and_slot_function which is analogous to
pci_get_bus_and_slot. It returns a pci_dev given a domain (segment) number,
bus number, and devnr. Like pci_get_bus_and_slot,
pci_get_domain_bus_and_slot holds a reference to the returned pci_dev.

Converted pci_get_bus_and_slot to a wrapper that calls
pci_get_domain_bus_and_slot with the domain hard-coded to 0.

This routine was patterned off code suggested by Bjorn Helgaas.

Feedback from Hidetoshi Seto and Kenji Kaneshige incorporated.  This
correctly handles PCI-X bridges, PCIe root ports and endpoints, and
prints debug messages when invalid/reserved types are found in the
HEST.  PCI devices not in domain/segment 0 are not represented in
HEST, thus will be ignored.

Today, the PCIe Advanced Error Reporting (AER) driver attaches itself
to every PCIe root port for which BIOS reports it should, via ACPI
_OSC.

However, _OSC alone is insufficient for newer BIOSes.  Part of ACPI
4.0 is the new APEI (ACPI Platform Error Interfaces) which is a way
for OS and BIOS to handshake over which errors for which components
each will handle.  One table in ACPI 4.0 is the Hardware Error Source
Table (HEST), where BIOS can define that errors for certain PCIe
devices (or all devices), should be handled by BIOS ("Firmware First
mode"), rather than be handled by the OS.

Dell PowerEdge 11G server BIOS defines Firmware First mode in HEST, so
that it may manage such errors, log them to the System Event Log, and
possibly take other actions.  The aer driver should honor this, and
not attach itself to devices noted as such.

Furthermore, Kenji Kaneshige reminded us to disallow changing the AER
registers when respecting Firmware First mode.  Platform firmware is
expected to manage these, and if changes to them are allowed, it could
break that firmware's behavior.

The HEST parsing code may be replaced in the future by a more
feature-rich implementation.  This patch provides the minimum needed
to prevent breakage until that implementation is available.

When probing for ROM BAR size, we should not change bits 1:10 in this
BAR, because these bits are marked as "reserved for future use" in PCI

Allen Kay ae21ee65e8 PCI: acs p2p upsteram forwarding enabling
Note: dom0 checking in v4 has been separated out into 2/2.

This patch enables P2P upstream forwarding in ACS capable PCIe switches.
It solves two potential problems in virtualization environment where a PCIe
device is assigned to a guest domain using a HW iommu such as VT-d:

1) Unintentional failure caused by guest physical address programmed
   into the device's DMA that happens to match the memory address range
   of other downstream ports in the same PCIe switch.  This causes the PCI
   transaction to go to the matching downstream port instead of go to the
   root complex to get translated by VT-d as it should be.

2) Malicious guest software intentionally attacks another downstream
   PCIe device by programming the DMA address into the assigned device
   that matches memory address range of the downstream PCIe port.

We are in process of implementing device filtering software in KVM/XEN
management software to allow device assignment of PCIe devices behind a PCIe
switch only if it has ACS capability and with the P2P upstream forwarding bits
enabled.  This patch is intended to work for both KVM and Xen environments.

Matthew Garrett 3368dd2958 PCI hotplug: acpiphp should be linked after vendor drivers
As a followup to 71a082efc9, it's conceivable
that some vendors may expose PCI hotplug functionality through both vendor
mechanisms and ACPI. The native mechanism will generally be a superset of
any functionality provided via ACPI, so the acpiphp driver should always
be initialised after any others. Change the link order such that acpiphp
will not be initialised until any other statically linked drivers have had
an opportunity to claim the hardware.

Tejun Heo 15ea76d407 pccard: configure CLS on attach
For non hotplug PCI devices, the system firmware usually configures
CLS correctly.  For pccard devices system firmware can't do it and
Linux PCI layer doesn't do it either.  Unfortunately this leads to
poor performance for certain devices (sata_sil).  Unless MWI, which
requires separate configuration, is to be used, CLS doesn't affect
correctness, so the configuration should be harmless.

This patch makes pci_set_cacheline_size() always built and export it
and make pccard call it during attach.

Please note that some other PCI hotplug drivers (shpchp and pciehp)
also configure CLS on hotplug.

Jesse Barnes ac1aa47b13 PCI: determine CLS more intelligently
Till now, CLS has been determined either by arch code or as
L1_CACHE_BYTES.  Only x86 and ia64 set CLS explicitly and x86 doesn't
always get it right.  On most configurations, the chance is that
firmware configures the correct value during boot.

This patch makes pci_init() determine CLS by looking at what firmware
has configured.  It scans all devices and if all non-zero values
agree, the value is used.  If none is configured or there is a
disagreement, pci_dfl_cache_line_size is used.  arch can set the dfl
value (via PCI_CACHE_LINE_BYTES or pci_dfl_cache_line_size) or
override the actual one.

ia64, x86 and sparc64 updated to set the default cls instead of the
actual one.

While at it, declare pci_cache_line_size and pci_dfl_cache_line_size
in pci.h and drop private declarations from arch code.

David Woodhouse cf6f3bf7e5 Run pci_apply_final_quirks() sooner.
Having this as a device_initcall() means that some real device drivers
can actually initialise _before_ the quirks are run, which is wrong.

We want it to run _before_ device_initcall(), but _after_ fs_initcall(),
since some arch-specific PCI initialisation like pcibios_assign_resources()
is done at fs_initcall().

We could use rootfs_initcall() but I actually want to use that for the
IOMMU initialisation, which has to come after the quirks, but still
before the real devices. So use fs_initcall_sync() instead -- since this
is entirely synchronous, it doesn't hurt that it'll escape the
synchronisation.

Kenji Kaneshige 30fc24b5cb PCI: Prevent AER driver from being loaded on non-root port PCIE devices
A bug was seen on boards using a PLX 8518 switch device which advertises
AER on each of it's transparent bridges. The AER driver was loaded for
each bridge and this driver tried to access the AER source ID register
whenever an interrupt occured on the shared PCI INTX lines. The source
ID register does not exist on non root port PCIE device's  which
advertise AER and trying to access this register causes a unsupported
request error on the bridge. Thus, when the next interrupt occurs,
another error is found and the non existent source ID register is
accessed again, and so it goes on.

The result is a spammed dmesg with unsupported request PCI express
errors on the bridge device that the AER driver is loaded against.

Rafael J. Wysocki e13cdbd71f PCI PM: Read device power state from register after updating it
After attempting to change the power state of a PCI device
pci_raw_set_power_state() doesn't check if the value it wrote into
the device's PCI_PM_CTRL register has been stored in there, but
unconditionally modifies the device's current_state field to reflect
the change.  This may cause problems to happen if the power state of
the device hasn't been changed in fact, because it will make the PCI
PM core make a wrong assumption.

To prevent such situations from happening modify
pci_raw_set_power_state() so that it reads the device's PCI_PM_CTRL
register after writing into it and uses the value read from the
register to update the device's current_state field.  Also make it
print a message saying that the device refused to change its power
state as requested (returning an error code in such cases would cause
suspend regressions to appear on some systems, where device drivers'
suspend routines return error codes if pci_set_power_state() fails).

Asus decided to ship a BIOS which configures sound DMA to go via the
dedicated IOMMU unit, but assigns precisely zero TLB entries to that
unit. Which causes the whole thing to deadlock, including the DMA
traffic on the _other_ IOMMU units. Nice one.

Bjorn Helgaas b24715027a ACPICA: fixup after acpi_get_object_info() change
Commit 15b8dd53f5 changed info->hardware_id from a static array to
a pointer.  If hardware_id is non-NULL, it points to a NULL-terminated
string, so we don't need to terminate it explicitly.  However, it may
be NULL; in that case, we *can't* add a NULL terminator.

This causes a NULL pointer dereference oops for devices without _HID.

Roland Dreier 17b6097753 intel-iommu: Decode (and ignore) RHSA entries
I recently got a system where the DMAR table included a couple of RHSA
(remapping hardware static affinity) entries.  Rather than printing a
message about an "Unknown DMAR structure," it would probably be more
useful to dump the RHSA structure (as other DMAR structures are dumped).

Roland Dreier 4de75cf939 intel-iommu: Make "Unknown DMAR structure" message more informative
We might as well print the type of the DMAR structure we don't know how
to handle when skipping it.  Then someone getting this message has a
chance of telling whether the structure is just bogus, or if there
really is something valid that the kernel doesn't know how to handle.

If end_pfn is equal to (unsigned long)-1, then the loop will never end.

Seen on 32-bit kernel, but could have happened on 64-bit too once we get
hardware that supports 64-bit guest addresses.

Change both functions to a 'do {} while' loop with the test at the end,
and check for the PFN having wrapper round to zero.

Register had been changed by the "ASPM optionality ECN" as follows:

	00b	No ASPM Support
	01b	L0s Supported
	10b	L1 Supported
	11b	L0s and L1 Supported

Current linux ASPM driver doesn't enable ASPM if the support field is
00b or 10b. So there is no impact about 00b. But current linux ASPM
driver doesn't enable L1 if the support field is 10b. With this patch,
10b (L1 support) is handled properly.

Bjorn Helgaas 8838400db5 PCI hotplug: add pci_configure_slot()
This patch adds a new pci_configure_slot() function that programs the
PCI bus characteristics for a newly-added device.  This is based on
code in pciehp_pci.c, but should be generic enough to be used by pciehp,
shpchp, and acpiphp.

The hotplug_params struct and the program_hpp_typeX() functions are based
on the ACPI definitions, but they aren't really ACPI-specific, and there's
no alternate implementation, so I don't see the need to abstract them yet.

Bjorn Helgaas fca6825ad7 PCI hotplug: acpiphp: don't cache hotplug_params in acpiphp_bridge
We always look up hotplug_params with decode_hpp() immediately before using
them, so we don't need to save them in the acpiphp_bridge struct.  This
patch follows the example of program_fw_provided_values() in pciehp_pci.c
and shpchp_pci.c by just keeping the params on the stack while we need them.

Bjorn Helgaas dffb4bb560 PCI hotplug: acpiphp: remove superfluous _HPP/_HPX evaluation
decode_hpp() looks up hotplug PCI parameters from ACPI and saves them
in the acpiphp_bridge structure.  These parameters (in bridge->hpp) are
only used by the acpiphp_set_hpp_values() -> program_hpp() path.  In
that path, we always call decode_hpp() before program_hpp(), so there's
no need to do it in init_bridge_misc().

Rafael J. Wysocki 4b77b0a2ba PCI: Clear saved_state after the state has been restored
Some PCI devices fail if their standard configuration registers are
restored twice in a row.  Prevent this from happening by making
pci_restore_state() clear the saved_state flag of the device right
after the device's standard configuration registers have been
populated with the previously saved values.

Simplify PCI PM callbacks by removing the direct clearing of
state_saved from them, as it shouldn't be necessary any more (except
in pci_pm_thaw(), where it has to be cleared, so that the values saved
during the "freeze" phase of hibernation are not used later by mistake).

Youquan Song 074835f014 intel-iommu: Fix kernel hang if interrupt remapping disabled in BIOS
BIOS clear DMAR table INTR_REMAP flag to disable interrupt remapping. Current
kernel only check interrupt remapping(IR) flag in DRHD's extended capability
register to decide interrupt remapping support or not. But IR flag will not
change when BIOS disable/enable interrupt remapping.

When user disable interrupt remapping in BIOS or BIOS often defaultly disable
interrupt remapping feature when BIOS is not mature.Though BIOS disable
interrupt remapping but intr_remapping_supported function will always report
to OS support interrupt remapping if VT-d2 chipset populated. On this
cases, kernel will continue enable interrupt remapping and result kernel panic.
This bug exist on almost all platforms with interrupt remapping support.

This patch add DMAR table INTR_REMAP flag check before enable interrupt
remapping.

Current kernel enable interrupt remapping only when all the vt-d unit support
interrupt remapping. So it is reasonable we should also disallow enabling
intr-remapping if there any io-apics that are not listed under vt-d units.

Rafael J. Wysocki 0baed8da1e PCI / ACPI PM: Propagate wake-up enable for devices w/o ACPI support
Some PCI devices (not PCI Express), like PCI add-on cards, can
generate PME#, but they don't have any special platform wake-up
support.  For this reason, even if they generate PME# to wake up the
system from a sleep state, wake-up events are not generated by the
platform.

It turns out that, at least on some systems, PCI bridges and the PCI
host bridge have ACPI GPEs associated with them that, if enabled to
generate wake-up events, allow the system to wake up if one of the
add-on devices asserts PME# while the system is in a sleep state.
Following this observation, if a PCI device without direct ACPI
wake-up support is prepared to wake up the system during a transition
into a sleep state (eg. suspend to RAM), try to configure the bridges
on the path from the device to the root bridge to wake-up the system.

In general a BIOS may goof or we may hotplug in a hotplug controller.
In either case the kernel needs to reserve resources for plugging
in more devices in the future instead of creating a minimal resource
assignment.

We already do this for cardbus bridges I am just adding a variant
for pcie bridges.

v2: Make testing for pcie hotplug bridges based on a flag.

    So far we only set the flag for pcie but a header_quirk
    could easily be added for the non-standard pci hotplug
    bridges.

Eric W. Biederman 0ba379ec0f PCI: Simplify hotplug mch quirk.
There is a very old quirk for the intel E7502 E7320 and E7525 memory
controller hubs that disables usage of msi interrupts on pcie hotplug
bridges of those devices, and disables changing the affinity of irqs.

Today all we have to do to disable msi on a specific device is to set
dev->no_msi, which is much more straightforward than the previous
logic.

The re-running of this fixup after pci hotplug happens below these
devices is totally bogus.  All of the state we change is pure software
state and we don't change the hardware at all.  Which means hotplug on
the lower devices doesn't have a chance to change this state.  So we
can safely remove the special case from the pciehp driver and the pcie
portdriver.

I suspect the special case was someone's expermental debug code that
slipped in. Certainly it isn't mentioned in commit
6fb8880a61510295aece04a542767161f624dffe aka BKrev:
41966101LJ_ogfOU0m2aE6teZfQnuQ where the code first appears.

Multiple bits might be set in the Uncorrectable Error Status
register.  But aer_print_error_source() only report a error of
the lowest bit set in the error status register.

So print strings for all bits unmasked and set.

And check First Error Pointer to mark the error occured first.
This FEP is not valid when the corresponing bit of the Uncorrectable
Error Status register is not set, or unimplemented or undefined.

Hidetoshi Seto 0d90c3ac0b PCI: pcie, aer: refer mask state in mask register properly
ERR_{,UN}CORRECTABLE_ERROR_MASK are set of error bits which linux know,
set of PCI_ERR_COR_* and PCI_ERR_UNC_* defined in linux/pci_regs.h.
This masks make aerdrv not to report errors of unknown bit, while aerdrv
have ability to report such undefined errors as "Unknown Error Bit %2d".

OTOH aerdrv_errprint does not have any check of setting in mask register.
So it could report masked wrong error by finding bit in status without
knowing that the bit is masked in the mask register.

This patch changes aerdrv to use mask state in mask register propely
instead of defined/hardcoded ERR_{,UN}CORRECTABLE_ERROR_MASK.
This change prevents aerdrv from reporting masked error, and also enable
reporting unknown errors.

The flag AER_MULTI_ERROR_VALID_FLAG in info->flag does mean that the
root port receives multiple error messages.  Error messages can be
posted from different devices, so it does not mean that each reported
device has multiple errors.

If there are multiple error devices and the root port has valid error
source ID, it would be nice to report which device is the error source
reported first.

In case of multiple errors, struct aer_err_info would be reused among
all reported devices.  So the info->status should be initialized before

Hidetoshi Seto f158575696 PCI: pcie, aer: rework MASK macros in aerdrv_errprint.c
Definitions of MASK macros in aerdrv_errprint.c are tricky and unsafe.

For example, AER_AGENT_TRANSMITTER_MASK(_sev, _stat) does work like:

      return (_stat & (PCI_ERR_COR_REP_ROLL|PCI_ERR_COR_REP_TIMER));
    else
      return (_stat & PCI_ERR_COR_REP_ROLL);
  }
In case of else path here, for uncorrectable errors, testing bits in
_stat by PCI_ERR_COR_* does not make sense because _stat should have only
PCI_ERR_UNC_* bits originated in uncorrectable error status register.
But at this time this is safe because uncorrectable error using bit
position same to PCI_ERR_COR_REP_ROLL(= bit position 8) is not defined.
Likewise, AER_AGENT_COMPLETER_MASK is always PCI_ERR_UNC_COMP_ABORT but
it works because bit 15 of correctable error status is not defined.

It means that these MASK macros will turn to be wrong once if new error
is defined. (In fact, bit 15 of correctable is now defined in PCIe 2.1)

This patch changes these MASK macros to be more strict, not to return
PCI_ERR_COR_* bits for uncorrectable error status and vise versa.

Tejun Heo b439b1d4e3 PCI: pci-stub: add pci_stub.ids parameter
Add ids module parameter which allows specifying initial IDs for the
pci-stub driver.  When built into the kernel, pci-stub is linked
before any real pci drivers and by setting up IDs from initialization
it can prevent built-in drivers from attaching to specific devices.

While at it, make pci_stub_probe() print out about devices it grabbed
to weed out "but my controller isn't being probed" bug reports.

Tejun Heo 9dba910e9d PCI: separate out pci_add_dynid()
Separate out pci_add_dynid() from store_new_id() and export it so that
in-kernel code can add PCI IDs dynamically.  As the function will be
available regardless of HOTPLUG, put it and pull pci_free_dynids()
outside of CONFIG_HOTPLUG.

This will be used by pci-stub to initialize initial IDs via module
param.

While at it, remove bogus get_driver() failure check.

Kenji Kaneshige ac18018a41 PCI ASPM: support per direction l0s management
The L0s state can be managed separately for each direction (upstream
direction and downstream direction) of the link. But in the current
implementation, those are mixed up. With this patch, L0s for each
direction are managed separately.

To maintain three states (upstream direction L0s, downstream L0s and
L1), 'aspm_support', 'aspm_enabled', 'aspm_capable', 'aspm_disable'
and 'aspm_default' fields in struct pcie_link_state are changed to
3-bit from 2-bit. The 'latency' field is separated to two 'latency_up'
and 'latency_dw' fields to maintain exit latencies for each direction
of the link. For L0, 'latency_up.l0' and 'latency_dw.l0' are used to
configure upstream direction L0s and downstream direction L0s
respectively. For L1, larger value of 'latency_up.l1' and
'latency_dw.l1' is considered as L1 exit latency.

Kenji Kaneshige b7206cbf02 PCI ASPM: support partial aspm enablement
In the current implementation, ASPM L0s/L1 is disabled for all links
in the hierarchy if one of the link doesn't meet latency requirement.
But we can partially enable ASPM L0s/L1 on sub-tree in the hierarchy.
This patch allows partial L0s/L1 enablement in the hierarchy. And it
also reduce the calculation cost of ASPM configuration very much.

In the previous implementation, all links were enabled with the same
state. With this patch, enabled state for each link is determined
simply as follows (the 'requested' is from policy_to_aspm_state()).

    enabled = requested & (link->aspm_capable & link->aspm_disable)

Kenji Kaneshige 07d92760d2 PCI ASPM: introduce capable flag
Introduce 'aspm_capable' field to maintain the capable ASPM setting of
the link. By the 'aspm_capable', we don't need to recheck latency
every time ASPM policy is changed.

Each bit in 'aspm_capable' is associated to ASPM state (L0S/L1). The
bit is set if the associated ASPM state is supported by the link and
it satisfies the latency requirement (i.e. exit latency < endpoint
acceptable latency). The 'aspm_capable' is updated when

  - an endpoint device is added (boot time or hot-plug time)
  - an endpoint device is removed (hot-unplug time)
  - PCI power state is changed.

Background:
Graphic devices are accessed through ranges in I/O or memory space. While most
modern devices allow relocation of such ranges, some "Legacy" VGA devices
implemented on PCI will typically have the same "hard-decoded" addresses as
they did on ISA. For more details see "PCI Bus Binding to IEEE Std 1275-1994
Standard for Boot (Initialization Configuration) Firmware Revision 2.1"
Section 7, Legacy Devices.

The Resource Access Control (RAC) module inside the X server currently does
the task of arbitration when more than one legacy device co-exists on the same
machine. But the problem happens when these devices are trying to be accessed
by different userspace clients (e.g. two server in parallel). Their address
assignments conflict. Therefore an arbitration scheme _outside_ of the X
server is needed to control the sharing of these resources. This document
introduces the operation of the VGA arbiter implemented for Linux kernel.

Michael S. Tsirkin 711d57796f PCI: expose function reset capability in sysfs
Some devices allow an individual function to be reset without affecting
other functions in the same device: that's what pci_reset_function does.
For devices that have this support, expose reset attribite in sysfs.

This is useful e.g. for virtualization, where a qemu userspace
process wants to reset the device when the guest is reset,
to emulate machine reboot as closely as possible.

Alex Chiang 5228a828ee PCI Hotplug: acpiphp: get pci_bus from acpi handle correctly
We cannot simply call acpi_get_pci_dev() on any random ACPI handle
and hope that it works, because a PCI root bridge may not have
an associated struct pci_dev.

This is allowed per the PCI specification, and is referred to as a
non-materialized bridge.

So, depending on the type of PCI bridge that the handle points to,
use the appropriate interface to return the struct pci_bus correctly.

DMAR faults are recorded into a ring of "fault recording registers".
fault_index is a 0-based index into the ring. The code allows the
0-based fault_index to be equal to the total number of fault registers
available from the cap_num_fault_regs() macro, which causes access
beyond the last available register.

Chris Wright 6faf17f6f1 PCI SR-IOV: correct broken resource alignment calculations
An SR-IOV capable device includes an SR-IOV PCIe capability which
describes the Virtual Function (VF) BAR requirements.  A typical SR-IOV
device can support multiple VFs whose BARs must be in a contiguous region,
effectively an array of VF BARs.  The BAR reports the size requirement
for a single VF.  We calculate the full range needed by simply multiplying
the VF BAR size with the number of possible VFs and create a resource
spanning the full range.

This all seems sane enough except it artificially inflates the alignment
requirement for the VF BAR.  The VF BAR need only be aligned to the size
of a single BAR not the contiguous range of VF BARs.  This can cause us
to fail to allocate resources for the BAR despite the fact that we
actually have enough space.

This patch adds a thin PCI specific layer over the generic
resource_alignment() function which is aware of the special nature of
VF BARs and does sorting and allocation based on the smaller alignment
requirement.

I recognize that while resource_alignment is generic, it's basically a
PCI helper.  An alternative to this patch is to add PCI VF BAR specific
information to struct resource.  I opted for the extra layer rather than
adding such PCI specific information to struct resource.  This does
have the slight downside that we don't cache the BAR size and re-read
for each alignment query (happens a small handful of times during boot
for each VF BAR).

Completed a major update for the acpi_get_object_info external interface.
Changes include:
 - Support for variable, unlimited length HID, UID, and CID strings
 - Support Processor objects the same as Devices (HID,UID,CID,ADR,STA, etc.)
 - Call the _SxW power methods on behalf of a device object
 - Determine if a device is a PCI root bridge
 - Change the ACPI_BUFFER parameter to ACPI_DEVICE_INFO.
These changes will require an update to all callers of this interface.

Donald Dutile 94a91b5051 intel-iommu: iommu init error path bug fixes
The kcalloc() failure path in iommu_init_domains() calls
free_dmar_iommu(), which assumes that ->domains, ->domain_ids,
and ->lock have been properly initialized.

Add checks in free_[dmar]_iommu to not use ->domains,->domain_ids
if not alloced. Move the lock init to prior to the kcalloc()'s,
so it is valid in free_context_table() when free_dmar_iommu() invokes
it at the end.

Sheng Yang c5b1525533 intel-iommu: Fix enabling snooping feature by mistake
Two defects work together result in KVM device passthrough randomly can't
work:
1. iommu_snooping is not initialized to zero when vm_iommu_init() called.
So it is possible to get a random value.
2. One line added by commit 2c2e2c38("IOMMU Identity Mapping Support")
change the code path, let it bypass domain_update_iommu_cap(), as well as
missing the increment of domain iommu reference count.

The latter is also likely to cause a leak of domains on repeated VMM 
assignment and deassignment.

Fenghua Yu 33041ec049 intel-iommu: Mask physical address to correct page size in intel_map_single()

Fenghua Yu f532959b77 intel-iommu: Correct sglist size calculation.
In domain_sg_mapping(), use aligned_nrpages() instead of hand-coded
rounding code for calculating the size of each sg elem. This means that
on IA64 we correctly round up to the MM page size, not just to the VT-d
page size.

Also remove the incorrect mm_to_dma_pfn() when intel_map_sg() calls
domain_sg_mapping() -- the 'size' variable is in VT-d pages already.

David Woodhouse 19943b0e30 intel-iommu: Unify hardware and software passthrough support
This makes the hardware passthrough mode work a lot more like the
software version, so that the behaviour of a kernel with 'iommu=pt'
is the same whether the hardware supports passthrough or not.

In particular:
 - We use a single si_domain for the pass-through devices.
 - 32-bit devices can be taken out of the pass-through domain so that
   they don't have to use swiotlb.
 - Devices will work again after being removed from a KVM guest.
 - A potential oops on OOM (in init_context_pass_through()) is fixed.

Linus Torvalds 79896cf42f Make pci_claim_resource() use request_resource() rather than insert_resource()
This function has traditionally used "insert_resource()", because before
commit cebd78a8c5 ("Fix pci_claim_resource") it used to just insert the
resource into whatever root resource tree that was indicated by
"pcibios_select_root()".

So there Matthew fixed it to actually look up the proper parent
resource, which means that now it's actively wrong to then traverse the
resource tree any more: we already know exactly where the new resource
should go.

And when we then did commit a76117dfd6 ("x86: Use pci_claim_resource"),
which changed the x86 PCI code from the open-coded

	pr = pci_find_parent_resource(dev, r);
	if (!pr || request_resource(pr, r) < 0) {

to using

	if (pci_claim_resource(dev, idx) < 0) {

that "insert_resource()" now suddenly became a problem, and causes a
regression covered by

Joseph Cihula a59b50e995 intel_txt: Force IOMMU on for Intel TXT launch
The tboot module will DMA protect all of memory in order to ensure the that
kernel will be able to initialize without compromise (from DMA).  Consequently,
the kernel must enable Intel Virtualization Technology for Directed I/O
(VT-d or Intel IOMMU) in order to replace this broad protection with the
appropriate page-granular protection.  Otherwise DMA devices will be unable
to read or write from memory and the kernel will eventually panic.

Because runtime IOMMU support is configurable by command line options, this
patch will force it to be enabled regardless of the options specified, and will
log a message if it was required to force it on.

Sheng Yang 4b99d35270 intel-iommu: Fix intel_iommu_unmap_range() with size 0
After some API change, intel_iommu_unmap_range() introduced a assumption that
parameter size != 0, otherwise the dma_pte_clean_range() would have a
overflowed argument. But the user like KVM don't have this assumption before,
then some BUG() triggered.

Fix it by ignoring size = 0.

David Woodhouse 147202aa77 intel-iommu: Speed up map routines by using cached domain ASAP
We did before, in the end -- but it was at the bottom of a long stack of
functions. Add an inline wrapper get_valid_domain_for_dev() which will
use the cached one _first_ and only make the out-of-line call if it's
not already set.

This takes the average time taken for a 1-page intel_map_sg() from 5961
cycles to 4812 cycles on my Lenovo x200s test box -- a modest 20%.

David Woodhouse 3dfc813d94 intel-iommu: Don't use identity mapping for PCI devices behind bridges
Our current strategy for pass-through mode is to put all devices into
the 1:1 domain at startup (which is before we know what their dma_mask
will be), and only _later_ take them out of that domain, if it turns out
that they really can't address all of memory.

However, when there are a bunch of PCI devices behind a bridge, they all
end up with the same source-id on their DMA transactions, and hence in
the same IOMMU domain. This means that we _can't_ easily move them from
the 1:1 domain into their own domain at runtime, because there might be DMA
in-flight from their siblings.

So we have to adjust our pass-through strategy: For PCI devices not on
the root bus, and for the bridges which will take responsibility for
their transactions, we have to start up _out_ of the 1:1 domain, just in
case.

This fixes the BUG() we see when we have 32-bit-capable devices behind a
PCI-PCI bridge, and use the software identity mapping.

It does mean that we might end up using 'normal' mapping mode for some
devices which could actually live with the faster 1:1 mapping -- but
this is only for PCI devices behind bridges, which presumably aren't the
devices for which people are most concerned about performance.

When we reattach a device to the si_domain (because it's been removed
from a VM), we weren't calling domain_context_mapping() to actually tell
the hardware about that.

We should really put the call to domain_context_mapping() into
domain_add_dev_info() -- we never call the latter without also doing the

The aligned_nrpages() function rounds up to the next VM page, but
returns its result as a number of DMA pages.

Purely theoretical except on IA64, which doesn't boot with VT-d right
now anyway.

Kill this, in favour of just looking to see if the incremented pte
pointer has 'wrapped' onto the next page. Which means we have to check
it _after_ incrementing it, not before.

Matthew Wilcox 46b952a3c3 PCI: Fix IRQ swizzling for ARI-enabled devices
For many purposes, including interrupt-swizzling, devices with ARI
enabled behave as if they have one device (number 0) and 256 functions.
This probably hasn't bitten us in practice because all ARI devices I've
seen are also IOV devices, and IOV devices are required to use MSI.
This isn't guaranteed, and there are legitimate reasons to use ARI
without IOV, and hence potentially use pin-based interrupts.

Since we're using cmpxchg64() anyway (because that's the only way to do
an atomic 64-bit store on i386), we might as well ditch the extra
locking and just use cmpxchg64() to ensure that we don't add the page
twice.
