17d5363b83 scsi: introduce a result field in struct scsi_request
This passes on the scsi_cmnd result field to users of passthrough
requests.  Currently we abuse req->errors for this purpose, but that
field will go away in its current form.

Note that the old IDE code abuses the errors field in very creative
ways and stores all kinds of different values in it.  I didn't dare
to touch this magic, so the abuses are brought forward 1:1.

aebf526b53 block: fold cmd_type into the REQ_OP_ space
Instead of keeping two levels of indirection for requests types, fold it
all into the operations.  The little caveat here is that previously
cmd_type only applied to struct request, while the request and bio op
fields were set to plain REQ_OP_READ/WRITE even for passthrough
operations.

Instead this patch adds new REQ_OP_* for SCSI passthrough and driver
private requests, althought it has to add two for each so that we
can communicate the data in/out nature of the request.

57292b58dd block: introduce blk_rq_is_passthrough
This can be used to check for fs vs non-fs requests and basically
removes all knowledge of BLOCK_PC specific from the block layer,
as well as preparing for removing the cmd_type field in struct request.

82ed4db499 block: split scsi_request out of struct request
And require all drivers that want to support BLOCK_PC to allocate it
as the first thing of their private data.  To support this the legacy
IDE and BSG code is switched to set cmd_size on their queues to let
the block layer allocate the additional space.

a8036dfba9 cciss: print max outstanding commands as a hex value
The max outstanding commands is being printed with a 0x prefix to
suggest it is a hex value, when in fact the integer decimal %d format
specifier is being used and this is a bit confusing. Use %x instead to
match the proceeding 0x prefix.

24cddb83b4 cciss: Use pci_enable_msix_exact() instead of pci_enable_msix()
As result of deprecation of MSI-X/MSI enablement functions
pci_enable_msix() and pci_enable_msi_block() all drivers
using these two interfaces need to be updated to use the
new pci_enable_msi_range()  or pci_enable_msi_exact()
and pci_enable_msix_range() or pci_enable_msix_exact()
interfaces.

371ff93a72 cciss: Fallback to MSI rather than to INTx if MSI-X failed
Currently the driver falls back to INTx mode when MSI-X
initialization failed. This is a suboptimal behaviour
for chips that also support MSI. This update changes that
behaviour and falls back to MSI mode in case MSI-X mode
initialization failed.

e7b18ede44 cciss: set max scatter gather entries to 32 on P600
At one time we used to set the maximum number of scatter gather elements
on all Smart Array controllers to 32.  At some point in time the
firmware began to write the "appropriate" value for each controller into
the config table.  The cciss driver would then read that and set
h->maxsgentries.

        h->maxsgentries = readl(&(h->cfgtable->MaxSGElements);

On the P600 that value is 544.  Under some workloads a significant
performance reduction may result.  This patch forces the P600 to use
only 32 scatter gather elements.  Other controllers are not affected.

If a new logical drive is added and the CCISS_REGNEWD ioctl is invoked
(as is normal with the Array Configuration Utility) the process will
hang as below.  It attempts to acquire the same mutex twice, once in
do_ioctl() and once in cciss_unlocked_open().  The BKL was recursive,
the mutex isn't.

This mutex usage was added into the ioctl path when the big kernel lock
was removed.  As it turns out, these paths are all thread safe anyway
(or can easily be made so) and we don't want ioctl() to be single
threaded in any case.

db2a144bed block_device_operations->release() should return void
The value passed is 0 in all but "it can never happen" cases (and those
only in a couple of drivers) *and* it would've been lost on the way
out anyway, even if something tried to pass something meaningful.
Just don't bother.

0821e90405 cciss: bug fix to prevent cciss from loading in kdump crash kernel
By default the cciss driver supports all "older" HP Smart Array
controllers and hpsa supports all controllers starting with the G6 family.
 There are module parameters that allow a user to override those defaults
and use hpsa for any HP Smart Array controller.

If the user does override the default behavior and uses hpsa for older
controllers it is possible that cciss may try to load in a kdump crash
kernel.  This may happen if cciss is loaded first from the kdump initrd
image.  If cciss does load rather than hpsa and reset_devices is true we
immediately call cciss_hard_reset_controller.  This will result in a
kernel panic and the core file cannot be created.  This patch prevents
cciss from trying to load in this scenario.

The only part of proc_dir_entry the code outside of fs/proc
really cares about is PDE(inode)->data.  Provide a helper
for that; static inline for now, eventually will be moved
to fs/proc, along with the knowledge of struct proc_dir_entry
layout.

8d85fce77e Drivers: block: remove __dev* attributes.
CONFIG_HOTPLUG is going away as an option.  As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

in order to handle some of the coding style issues better, by hand.

We weren't filling in the transfer length of the
flush cache command (it transfers 4 bytes of zeroes).
Firmware didn't seem to be bothered by this, but it

0007a4c90a cciss: auto engage SCSI mid layer at driver load time
A long time ago, probably in 2002, one of the distros, or maybe more than
one, loaded block drivers prior to loading the SCSI mid layer.  This meant
that the cciss driver, being a block driver, could not engage the SCSI mid
layer at init time without panicking, and relied on being poked by a
userland program after the system was up (and the SCSI mid layer was
therefore present) to engage the SCSI mid layer.

This is no longer the case, and cciss can safely rely on the SCSI mid
layer being present at init time and engage the SCSI mid layer straight
away.  This means that users will see their tape drives and medium
changers at driver load time without need for a script in /etc/rc.d that
does this:

done

However, if no tape drives or medium changers are detected, the SCSI mid
layer will not be engaged.  If a tape drive or medium change is later
hot-added to the system it will then be necessary to use the above script
or similar for the device(s) to be acceesible.

8a4ec67bd5 cciss: add cciss_tape_cmds module paramter
This is to allow number of commands reserved for use by SCSI tape drives
and medium changers to be adjusted at driver load time via the kernel
parameter cciss_tape_cmds, with a default value of 6, and a range
of 2 - 16 inclusive.  Previously, the driver limited the number of
commands which could be queued to the SCSI half of the the driver
to only 2.  This is to fix the problem that if you had more than
two tape drives, you couldn't, for example, erase or rewind them all
at the same time.

5afe278114 cciss: do soft reset if hard reset is broken
on driver load, if reset_devices is set, and the hard reset
attempts fail, try to bring up the controller to the point that
a command can be sent, and send it a soft reset command, then
after the reset undo whatever driver initialization was done to get
it to the point to take a command, and re-do it after the reset.

This is to get kdump to work on all the "non-resettable" controllers
(except 64xx controllers which can't be reset due to the potentially
shared cache module.)

bf2e2e6b87 cciss: use new doorbell-bit-5 reset method
The bit-2-doorbell reset method seemed to cause (survivable) NMIs
on some systems and (unsurvivable) IOCK NMIs on some G7 servers.
Firmware guys implemented a new doorbell method to alleviate these
problems triggered by bit 5 of the doorbell register.  We want to
use it if it's available.
