eepro100: address pci todo's, use pci_set_xx

eepro100 uses macros which rely on a specific
local variable name (pci_conf) which is scary.
Some of the uses are wrong or unnecessary,
remove them. The rest are small in number, open-code
them using pci_set_xx functions.
