arm: Add ARMv6-M programmer's model support

Forbid stack alignment change. (CCR)
Reserve FAULTMASK, BASEPRI registers.
Report any fault as a HardFault. Disable MemManage, BusFault and
UsageFault, so they always escalated to HardFault. (SHCSR)
