hw/arm/virt: Reject instantiation of multiple IOMMUs

We do not support instantiating multiple IOMMUs. Before adding a
virtio-iommu, check that no other IOMMU is present. This will detect
both "iommu=smmuv3" machine parameter and another virtio-iommu instance.
