armv7m: implement CCR, CFSR, HFSR, DFSR, BFAR, and MMFAR

Implement the v7M system registers CCR, CFSR, HFSR, DFSR, BFAR and
MMFAR.  For the moment these simply read as written (with some basic
handling of RAZ/WI bits and W1C semantics).
