event_notifier_get_fd(const EventNotifier *e) always returns
EventNotifier's read file descriptor (rfd). This is not a problem when
the EventNotifier is backed by a an eventfd, as a single file
descriptor is used both for reading and triggering events (rfd ==
wfd).
But, when EventNotifier is backed by a pipe pair, we have two file
descriptors, one that can only be used for reads (rfd), and the other
only for writes (wfd).
There's, at least, one known situation in which we need to obtain wfd
instead of rfd, which is when setting up the file that's going to be
sent to the peer in vhost's SET_VRING_CALL.
Add a new event_notifier_get_wfd(const EventNotifier *e) that can be
used to obtain wfd where needed.

Otherwise the call to event_notifier_set() is a nop, which causes
the SLOF firmware on POWER to hang when booting from a virtio-scsi
device:
virtio_scsi_dataplane_start()
virtio_scsi_vring_init()
virtio_bus_set_host_notifier() <- assign == true
event_notifier_init() <- active == 1
event_notifier_set() <- fails right away if !e->initialized

Add 'initialized' field and use it to avoid touching event notifiers which are
either not initialized or if their initialization failed.
This is somewhat a hack, but it seems the less intrusive way to make
virtio code deal with event notifiers that failed initialization.

If rfd is equal to wfd the file descriptor is closed but
rfd will still have the closed value.
The EventNotifier structure should not be used again after calling
event_notifier_cleanup or should be initialized again but make
sure to not have dandling file descriptors around.

Let's set the handles to the underlying facilities to their extremal
value so no accidental misuse can happen, and to make it obvious that the
notifier is dysfunctional. E.g. if we just close an fd but do not touch
the int holding the fd eventually a read/write could succeed again when
the fd gets reused, and corrupt the file addressed by the fd.

Remove the useless is_external argument. Since the iohandler
AioContext is never used for block devices, aio_disable_external
is never called on it. This lets us remove stubs/iohandler.c.

All callers pass "false" keeping the old semantics. The windows
implementation doesn't distinguish the flag yet. On posix, it is passed
down to the underlying aio context.

Event notifiers are designed for eventfd(2). They can fall back to
pipes, but according to Paolo, event_notifier_init_fd() really
requires the real thing, and should therefore be under #ifdef
CONFIG_EVENTFD. Do that.
Its only user is ivshmem, which is currently CONFIG_POSIX. Narrow it
to CONFIG_EVENTFD.
