make Coccinelle transform strings, and second to get rid of now
superfluous error_propagate().
We now use or propagate the original error whole instead of just its
message obtained with error_get_pretty(). This avoids suppressing its
hint (see commit 50b7b00), but I can't see how the errors touched in
this commit could come with hints. It also improves the message
printed with &error_abort when we screw up (see commit 1e9b65b).

Replace the generic vhost_call() by specific functions for each
function call to help with type safety and changing arguments.
While doing this, I found that "unsigned long long" and "uint64_t" were
used interchangeably and causing compilation warnings, using uint64_t
instead, as the vhost & protocol specifies.

vhost_virtqueue_mask takes an "absolute" virtqueue index, while the
code looks like it's passing an index that is relative to
s->dev.vq_index. In reality, s->dev.vq_index is always zero, so
this patch does not make any difference, but the code is clearer.

vhost-scsi calls vhost_dev_init() at realize() time
but forgets to call it's counterpart vhost_dev_cleanup()
at unrealize() time.
Calling it should fix leaking of memory table and
mem_sections table in vhost device. And also unregister
vhost's memory listerner to prevent access from
memory core to freed memory.

We use vhostforce to enable vhost even if Guests don't have MSI-X
support and we fall back to QEMU virtio-net.
This gives a very small performance gain, but the disadvantage
is that guest now controls which virtio code is running
(qemu or vhost) so our attack surface is doubled.
This patch will enable vhost unconditionally whenever it's requested.
For compatibility, enable vhost when vhostforce is set, as well.

Make features 64bit wide everywhere.
On migration a full 64bit guest_features field is sent if one of the
high bits is set, in addition to the lower 32bit guest_features field
which must stay for compatibility reasons. That way we send the lower
32 feature bits twice, but the code is simpler because we don't have
to split and compose the 64bit features into two 32bit fields.

monitor_handle_fd_param() is a wrapper around
monitor_handle_fd_param2() that feeds errors to qerror_report_err()
instead of returning them. qerror_report_err() is inappropriate in
many contexts. monitor_handle_fd_param() looks simpler than
monitor_handle_fd_param2(), which tempts use. Remove the temptation:
drop the wrapper and open-code the (trivial) error handling instead.
Replace the open-coded qerror_report_err() by error_report_err() in
places that already use error_report(). Turns out that's everywhere.
While there, rename monitor_handle_fd_param2() to monitor_fd_param().

We get two error messages: one from monitor_handle_fd_param2(), and
another one from vhost_scsi_realize(). The second one gets suppressed
in QMP context.
That's because monitor_handle_fd_param() calls qerror_report_err().
Calling qerror_report_err() is always inappropriate in realize
methods, because it doesn't return the Error object. It either
reports the error to stderr or the human monitor, or it stores it in
the QMP monitor, where it makes the QMP command fail even when the
realize method ignores the error and succeeds. Fortunately,
vhost_scsi_realize() doesn't do that.
Fix by switching to monitor_handle_fd_param2().

As vhost core can use backend_features during init, clear it earlier to
avoid using uninitialized memory.
This use would be harmless since vhost scsi ignores the result
anyway, but initializing earlier will help prevent valgrind errors,
and make scsi and net behave similarly.

Decouple vhost from the Linux kernel by introducing vhost_ops. The
intention is to provide different backends - a 'kernel' backend based on
the ioctl interface, and an 'user' backend based on a UNIX domain socket
and shared memory interface.

vhost_dev_init will replace devfd and devpath with a single opaque argument.
This is initialised with a file descriptor. When TAP is used (through
vhost_net), open /dev/vhost-net and pass the fd as an opaque parameter in
VhostNetOptions. The same applies to vhost-scsi - open /dev/vhost-scsi and
pass the fd.

vhost userspace needn't to handle vq's notification from guest,
so define dummy handle_output callback for all vqs of vhost-scsi.
In some corner cases(such as when handling vq's reset from VM), virtio-pci
still trys to handle pending virtio-scsi events, then object check failure
inside virtio_scsi_handle_event() for vhost-scsi can be triggered.
The issue can be reproduced by 'rmmod virtio-scsi', 'system sleep' or reboot
inside VM.

The ld_raw and st_raw definitions are only needed in code that
must compile for both user-mode and softmmu emulation. Device
models can use the equivalent ld_p/st_p which are simple
pointer accessors.

Coverity picked up a copy-paste bug. In vhost_scsi_start() we check for
!k->set_guest_notifiers and error out. The check probably got copied
but instead of erroring we actually use the function pointer!

The WWPN specified in configfs is passed to "-device vhost-scsi-pci".
The tgpt field of the SET_ENDPOINT ioctl is obsolete now, so it is not
available from the QEMU command-line. Instead, I hardcode it to zero.
