Markus Armbruster	121d07125b	Clean up header guards that don't match their file name ... Header guard symbols should match their file name to make guard collisions less likely. Offenders found with scripts/clean-header-guards.pl -vn. Cleaned up with scripts/clean-header-guards.pl, followed by some renaming of new guard symbols picked by the script to better ones. Paolo Bonzini	bd3f16ac30	s390x: reorganize CSS bits between cpu.h and other headers ... Move cpu_inject_* to the only C file where they are used. Move ioinst.h declarations that need S390CPU to cpu.h, to make ioinst.h independent of cpu.h. Move channel declarations that only need SubchDev from cpu.h to css.h, to make more channel users independent of cpu.h.
