Instead of including the same list of devices for each target,
set CONFIG_PCI to true, and make the devices default to present
whenever PCI is available. However, s390x does not want all the
PCI devices, so there is a separate symbol to enable them.
Done mostly with the following script:

Devices that are mostly used for testing purposes (for example in
endianness-test) will be moved under a new symbol CONFIG_TEST_DEVICES
that can be disabled in the default-configs file. This makes
it easier to drop this code from QEMU if desirable.

The make_device_config.sh script is replaced by minikconf, which
is modified to support the same command line as its predecessor.
The roots of the parsing are default-configs/*.mak, Kconfig.host and
hw/Kconfig. One difference with make_device_config.sh is that all symbols
have to be defined in a Kconfig file, including those coming from the
configure script. This is the reason for the Kconfig.host file introduced
in the previous patch. Whenever a file in default-configs/*.mak used
$(...) to refer to a config-host.mak symbol, this is replaced by a
Kconfig dependency; this part must be done already in this patch
for bisectability.

With certain USB devices passed through via usb-host, a guest attempting to
reset a usb-host device can trigger a reset loop that renders the USB device
unusable. In my use case, the device was an iPhone XR that was passed through to
a Mac OS X Mojave guest. Upon connecting the device, the following happens:
1) Guest recognizes new device, sends reset to emulated USB host
2) QEMU's USB host sends reset to host kernel
3) Host kernel resets device
4) After reset, host kernel determines that some part of the device descriptor
has changed ("device firmware changed" in dmesg), so host kernel decides to
re-enumerate the device.
5) Re-enumeration causes QEMU to disconnect and reconnect the device in the
guest.

The result of this is that the device remains permanently unusable in the guest.

This problem can be elegantly solved by interrupting step 2) above. Instead of
passing through the reset, QEMU simply ignores it. To allow this to be
configured on a per-device level, a new parameter "no_guest_reset" is
introduced for the usb-host device. I can confirm that the configuration
described above (iPhone XS + Mojave guest) works flawlessly with
no_guest_reset=True specified.
Working command line for my scenario:

it will allow to return another hotplug handler than the default
one for a specific bus based device type. Which is needed to handle
non trivial plug/unplug sequences that need the access to resources
configured outside of bus where device is attached.
That will allow for returned hotplug handler to orchestrate wiring
in arbitrary order, by chaining other hotplug handlers when
it's needed.
PS:
It could be used for hybrid virtio-mem and virtio-pmem devices
where it will return machine as hotplug handler which will do
necessary wiring at machine level and then pass control down
the chain to bus specific hotplug handler.

When unplugging a device, at one point the device will be destroyed
via object_unparent(). This will, one the one hand, unrealize the
removed device hierarchy, and on the other hand, destroy/free the
device hierarchy.
When chaining hotplug handlers, we want to overwrite a bus hotplug
handler by the machine hotplug handler, to be able to perform
some part of the plug/unplug and to forward the calls to the bus hotplug
handler.
For now, the bus hotplug handler would trigger an object_unparent(), not
allowing us to perform some unplug action on a device after we forwarded
the call to the bus hotplug handler. The device would be gone at that
point.

So move the object_unparent() to the original caller of the unplug. For
now, keep the unrealize() at the original places of the
object_unparent(). For implicitly chained hotplug handlers (e.g. pc
code calling acpi hotplug handlers), the object_unparent() has to be
done by the outermost caller. So when calling hotplug_handler_unplug()
from inside an unplug handler, nothing is to be done.

The qbus_is_full(BusState *bus) function (qdev_monitor.c) compares the max_index
value of the BusState structure with the max_dev value of the BusClass structure
to determine whether the maximum number of children has been reached for the
bus. The problem is, the max_index field of the BusState structure does not
necessarily reflect the number of devices that have been plugged into
the bus.
Whenever a child device is plugged into the bus, the bus's max_index value is
assigned to the child device and then incremented. If the child is subsequently
unplugged, the value of the max_index does not change and no longer reflects the
number of children.
When the bus's max_index value reaches the maximum number of devices
allowed for the bus (i.e., the max_dev field in the BusClass structure),
attempts to plug another device will be rejected claiming that the bus is
full -- even if the bus is actually empty.
To resolve the problem, a new 'num_children' field is being added to the
BusState structure to keep track of the number of children plugged into the
bus. It will be incremented when a child is plugged, and decremented when a
child is unplugged.

The new feature enables the virtio-balloon device to receive hints of
guest free pages from the free page vq.
A notifier is registered to the migration precopy notifier chain. The
notifier calls free_page_start after the migration thread syncs the dirty
bitmap, so that the free page optimization starts to clear bits of free
pages from the bitmap. It calls the free_page_stop before the migration
thread syncs the bitmap, which is the end of the current round of ram
save. The free_page_stop is also called to stop the optimization in the
case when there is an error occurred in the process of ram saving.
Note: balloon will report pages which were free at the time of this call.
As the reporting happens asynchronously, dirty bit logging must be
enabled before this free_page_start call is made. Guest reporting must be
disabled before the migration dirty bitmap is synchronized.

We are about to allow the memory map to grow beyond 1TB and
potentially overshoot the VCPU AA64MMFR0.PARANGE.
In aarch64 mode and when highmem is set, let's check the VCPU
PA range is sufficient to address the highest GPA of the memory
map.

This patch implements the machine class kvm_type() callback.
It returns the number of bits requested to implement the whole GPA
range including the RAM and IO regions located beyond.
The returned value is passed though the KVM_CREATE_VM ioctl and
this allows KVM to set the stage2 tables dynamically.
To compute the highest GPA used in the memory map, kvm_type()
must freeze the memory map by calling virt_set_memmap().

Up to now the memory map has been static and the high IO region
base has always been 256GiB.
This patch modifies the virt_set_memmap() function, which freezes
the memory map, so that the high IO range base becomes floating,
located after the initial RAM and the device memory.
The function computes
- the base of the device memory,
- the size of the device memory,
- the high IO region base
- the highest GPA used in the memory map.
Entries of the high IO region are assigned a base address. The
device memory is initialized.
The highest GPA used in the memory map will be used at VM creation
to choose the requested IPA size.
Setting all the existing highmem IO regions beyond the RAM
allows to have a single contiguous RAM region (initial RAM and
possible hotpluggable device memory). That way we do not need
to do invasive changes in the EDK2 FW to support a dynamic
RAM base.
Still the user cannot request an initial RAM size greater than 255GB.

On ARM, the kvm_type will be resolved by querying the KVMState.
Let's add the MachineState handle to the callback so that we
can retrieve the KVMState handle. in kvm_init, when the callback
is called, the kvm_state variable is not yet set.

In the prospect to introduce an extended memory map supporting more
RAM, let's split the memory map array into two parts:
- the former a15memmap, renamed base_memmap, contains regions below
and including the RAM. MemMapEntries initialized in this array
have a static size and base address.
- extended_memmap, only initialized with entries located after the
RAM. MemMapEntries initialized in this array only get their size
initialized. Their base address is dynamically computed depending
on the the top of the RAM, with same alignment as their size.
Eventually base_memmap entries are copied into the extended_memmap
array. Using two separate arrays however clarifies which entries
are statically allocated and those which are dynamically allocated.
This new split will allow to grow the RAM size without changing the
description of the high IO entries.
We introduce a new virt_set_memmap() helper function which
"freezes" the memory map. We call it in machvirt_init as
memory attributes of the machine are not yet set when
virt_instance_init() gets called.
The memory map is unchanged (the top of the initial RAM still is
256GiB). Then come the high IO regions with same layout as before.

Expose the virtio-net self announcement capability and allow
qemu_announce_self() to call it.
These announces are caused by something external (i.e. the
announce-self command); they won't trigger if the migration
counter is triggering announces at the same time.

Switch virtio's self announcement to use the AnnounceTimer.
It keeps it's own AnnounceTimer (per device), and starts running it
using a migration post-load and a virtual clock; that way the
announce happens once the guest is actually running.
The timer uses the migration parameters to set the timing of
the repeats.

Introduces hot plug/unplug support for the vfio-ap device.
To hot plug a vfio-ap device using the QEMU device_add command:
(qemu) device_add vfio-ap,sysfsdev=$path-to-mdev
Where $path-to-mdev is the absolute path to the mediated matrix device
to which AP resources to be used by the guest have been assigned.
A vfio-ap device can be hot plugged only if:
1. A vfio-ap device has not been attached to the virtual machine's ap-bus
via the QEMU command line or a prior hot plug action.
2. The guest was started with the CPU model feature for AP enabled
(e.g., -cpu host,ap=on)

because the AP matrix configuration for the guest is performed by the
kernel device driver when the vfio-ap device is realized. The vfio-ap device
represents a VFIO mediated device created in the host sysfs for use by a guest.
The mdev device is configured with an AP matrix (i.e., adapters and domains) via
its sysfs attribute interfaces prior to starting the guest or plugging a vfio-ap
device in. When the device is realized, a file descriptor is opened on the mdev
device which results in a callback to the vfio_ap kernel device driver. The
device driver then configures the AP matrix in the guest's SIE state description
from the AP matrix assigned via the mdev device's sysfs interfaces. The AP
devices will be created for the guest when the AP bus running on the guest
subsequently performs its periodic scan for AP devices.
The qdev_simple_device_unplug_cb() callback function is used for the same
reaons; namely, the vfio_ap kernel device driver will perform the AP resource
de-configuration for the guest when the vfio-ap device is unplugged. When the
vfio-ap device is unrealized, the mdev device file descriptor is closed which
results in a callback to the vfio_ap kernel device driver. The device driver
then clears the AP matrix configuration in the guest's SIE state description
and resets all of the affected queues. The AP devices created for the guest
will be removed when the AP bus running on the guest subsequently performs
its periodic scan and finds there are no longer any AP resources assigned to the
guest.

The locally allocated QDict-s need to be freed. ('file_layer' will be
freed implicitly since it is added as an object to 'driver_layer').

While in the neighbourhood free 'driver' and 'filename' as soon as they are
added to the QDicts. Freeing after the 'done' label doesn't make that much
sense as, if the error path jumps to that label, the values would be NULL
anyway.
This patch also makes that more obvious by taking the error path if
'params' is NULL and then asserting that both driver and filename are
non-NULL in the normal path.

The MCF5208EVB supports 2 MiB of flash at address 0. Add support
for this memory region and some code to load the file that can
be specified with the "-bios" command line option.
This can be used for example to load U-Boot images for the
MCF5208EVB (we still lack some features in the CPU emulation for
this firmware, though, so it can not be run successfully yet).

At the moment the handling of init-svtor and cpuwait initial
values is split between armsse.c and iotkit-sysctl.c:
the code in armsse.c sets the initial state of the CPU
object by setting the init-svtor and start-powered-off
properties, but the iotkit-sysctl.c code has its own
code setting the reset values of its registers (which are
then used when updating the CPU when the guest makes
runtime changes).
Clean this up by making the armsse.c code set properties on the
iotkit-sysctl object to define the initial values of the
registers, so they always match the initial CPU state,
and update the comments in armsse.c accordingly.

The CPUWAIT register acts as a sort of power-control: if a bit
in it is 1 then the CPU will have been forced into waiting
when the system was reset (which in QEMU we model as the
CPU starting powered off). Writing a 0 to the register will
allow the CPU to boot (for QEMU, we model this as powering
it on). Note that writing 0 to the register does not power
off a CPU.
For this to work correctly we need to also honour the
INITSVTOR* registers, which let the guest control where the
CPU will load its SP and PC from when it comes out of reset.
