bdrv_is_allocated() returns a boolean which indicates if the offset is
allocated, not 0 on success and everything else is an error.

Fix two stupid bugs that I forgot to push out:
- qiov->size already is in bytes, no need to shift it
- actually use the supplied patter in the writev command
