e34e47eb28 event_notifier: handle initialization failure better
Add 'initialized' field and use it to avoid touching event notifiers which are
either not initialized or if their initialization failed.

This is somewhat a hack, but it seems the less intrusive way to make
virtio code deal with event notifiers that failed initialization.

f87d72f5c5 qemu: add a cleanup callback function to EventNotifier
Adding a cleanup callback function to the EventNotifier struct
which allows users to execute event_notifier_cleanup in a
different context.

d6da1e9eca event_notifier: cleanups around event_notifier_set_handler
Remove the useless is_external argument.  Since the iohandler
AioContext is never used for block devices, aio_disable_external
is never called on it.  This lets us remove stubs/iohandler.c.

54e18d35e4 event-notifier: Add "is_external" parameter
All callers pass "false" keeping the old semantics. The windows
implementation doesn't distinguish the flag yet. On posix, it is passed
down to the underlying aio context.
