db895a1e6a isa: Use realizefn for ISADevice 
Drop ISADeviceClass::init and the resulting no-op initfn and let
children implement their own realizefn. Adapt error handling.
Split off an instance_init where sensible.

29bb5317cb i8259: QOM cleanups 
Eliminate DO_UPCAST() for PICCommonState. Prepares for ISA realizefn.

Also give the i8259_common type registration functions unique names
while at it.

0d09e41a51 hw: move headers to include/ 
Many of these should be cleaned up with proper qdev-/QOM-ification.
Right now there are many catch-all headers in include/hw/ARCH depending
on cpu.h, and this makes it necessary to compile these files per-target.
However, fixing this does not belong in these patches.
