This adds GRAPH_RDLOCK annotations to declare that callers of
bdrv_co_block_status() need to hold a reader lock for the graph.
For some places, we know that they will hold the lock, but we don't have
the GRAPH_RDLOCK annotations yet. In this case, add assume_graph_lock()
with a FIXME comment. These places will be removed once everything is
properly annotated.

The generated coroutine wrappers already take care to take the lock in
the non-coroutine path, and assume that the lock is already taken in the
coroutine path.
The only thing we need to do for the wrapped function is adding the
GRAPH_RDLOCK annotation. Doing so also allows us to mark the
corresponding callbacks in BlockDriver as GRAPH_RDLOCK_PTR.

Take the rdlock already, before we add the assertions.
All these functions either read the graph recursively, or call
BlockDriver callbacks that will eventually need to be protected by the
graph rdlock.
Do it now to all functions together, because many of these recursively
call each other.
For example, bdrv_co_truncate calls BlockDriver->bdrv_co_truncate, and
some driver callbacks implement their own .bdrv_co_truncate by calling
bdrv_flush inside. So if bdrv_flush asserts but bdrv_truncate does not
take the rdlock yet, the assertion will always fail.

For consistency with other I/O functions, and in preparation to
implement bdrv_{pread,pwrite}() using generated_co_wrapper.
unsigned int fits in int64_t, so all callers remain correct.
bdrv_check_request32() is called further down the stack and causes -EIO
to be returned if 'bytes' is negative or greater than
BDRV_REQUEST_MAX_BYTES, which in turns never exceeds SIZE_MAX.

Header guard symbols should match their file name to make guard
collisions less likely.
Cleaned up with scripts/clean-header-guards.pl, followed by some
renaming of new guard symbols picked by the script to better ones.

Elevate s->in_flight early so that other incoming requests will wait
on the CoQueue in nbd_co_send_request; restart them after getting back
from nbd_reconnect_attempt. This could be after the reconnect timer or
nbd_cancel_in_flight have cancelled the attempt, so there is no
need anymore to cancel the requests there.
nbd_co_send_request now handles both stopping and restarting pending
requests after a successful connection, and there is no need to
hold send_mutex in nbd_co_do_establish_connection. The current setup
is confusing because nbd_co_do_establish_connection is called both with
send_mutex taken and without it. Before the patch it uses free_sema which
(at least in theory...) is protected by send_mutex, after the patch it
does not anymore.

block coroutines functions run in different aiocontext, and are
not protected by the BQL. Therefore are I/O.
On the other side, generated_co_wrapper functions use BDRV_POLL_WHILE,
meaning the caller can either be the main loop or a specific iothread.

Let's drop hand-made coroutine wrappers and use coroutine wrapper
generation like in block/io.c.
Now, blk_foo() functions are written in same way as blk_co_foo() ones,
but wrap blk_do_foo() instead of blk_co_do_foo().

When checking for allocation across a chain, it's already easy to
count the depth within the chain at which the allocation is found.
Instead of throwing that information away, return it to the caller.
Existing callers only cared about allocated/non-allocated, but having
a depth available will be used by NBD in the next patch.

Now that we are not maintaining boilerplate code for coroutine
wrappers, there is no more sense in keeping the extra indirection layer
of bdrv_prwv(). Let's drop it and instead generate pure bdrv_preadv()
and bdrv_pwritev().
Currently, bdrv_pwritev() and bdrv_preadv() are returning bytes on
success, auto generated functions will instead return zero, as their
_co_ prototype. Still, it's simple to make the conversion safe: the
only external user of bdrv_pwritev() is test-bdrv-drain, and it is
comfortable enough with bdrv_co_pwritev() instead. So prototypes are
moved to local block/coroutines.h. Next, the only internal use is
bdrv_pread() and bdrv_pwrite(), which are modified to return bytes on
success.
Of course, it would be great to convert bdrv_pread() and bdrv_pwrite()
to return 0 on success. But this requires audit (and probably
conversion) of all their users, let's leave it for another day
refactoring.

We are going to keep coroutine-wrappers code (structure-packing
parameters, BDRV_POLL wrapper functions) in separate auto-generated
files. So, we'll need a header with declaration of original _co_
functions, for those which are static now. As well, we'll need
declarations for wrapper functions. Do these declarations now, as a
preparation step.
