Support write on Parallels images. The code is almost the same as one

Main approach is taken from qcow2_co_readv.
The patch drops coroutine lock for the duration of IO operation and
peforms normal scatter-gather IO using standard QEMU backend.
The patch also adds comment about locking considerations in the driver.

Switch the .bdrv_read method implementation from using bdrv_pread() to
bdrv_read() on the underlying file, since the latter is subject to i/o
throttling while the former is not.
Besides, since bdrv_read() operates in sectors rather than bytes, adjust
the helper functions to do so too.

The problem is that s->catalog_bitmap is allocated/filled as

g_new(T, n) is safer than g_malloc(sizeof(*v) * n) for two reasons.
One, it catches multiplication overflowing size_t. Two, it returns
T * rather than void *, which lets the compiler catch more type
errors.
Perhaps a conversion to g_malloc_n() would be neater in places, but
that's merely four years old, and we can't use such newfangled stuff.
This commit only touches allocations with size arguments of the form
sizeof(T), plus two that use 4 instead of sizeof(uint32_t). We can
make the others safe by converting to g_malloc_n() when it becomes
available to us in a couple of years.

Parallels has released in the recent updates of Parallels Server 5/6
new addition to his image format. Images with signature WithouFreSpacExt
have offsets in the catalog coded not as offsets in sectors (multiple
of 512 bytes) but offsets coded in blocks (i.e. header->tracks * 512)
In this case all 64 bits of header->nb_sectors are used for image size.
This patch implements support of this for qemu-img and also adds specific
check for an incorrect image. Images with block size greater than
INT_MAX/513 are not supported. The biggest available Parallels image
cluster size in the field is 1 Mb. Thus this limit will not hurt
anyone.

Parallels image format has several additional fields inside:
- nb_sectors is actually 64 bit wide. Upper 32bits are not used for
images with signature "WithoutFreeSpace" and must be explicitly
zeroed according to Parallels. They will be used for images with
signature "WithouFreSpacExt"
- inuse is magic which means that the image is currently opened for
read/write or was not closed correctly, the magic is 0x746f6e59
- data_off is the location of the first data block. It can be zero
and in this case data starts just beyond the header aligned to
512 bytes. Though this field does not matter for read-only driver
This patch adds these values to struct parallels_header and adds
proper handling of nb_sectors for currently supported WithoutFreeSpace
images.
WithouFreSpacExt will be covered in next patches.

Some code in the block layer makes potentially huge allocations. Failure
is not completely unexpected there, so avoid aborting qemu and handle
out-of-memory situations gracefully.
This patch addresses the allocations in the parallels block driver.

This avoids a possible division by zero.
Convert s->tracks to unsigned as well because it feels better than
surviving just because the results of calculations with s->tracks are
converted to unsigned anyway.

The first test case would cause a huge memory allocation, leading to a
qemu abort; the second one to a too small malloc() for the catalog
(smaller than s->catalog_size), which causes a read-only out-of-bounds
array access and on big endian hosts an endianess conversion for an
undefined memory area.
The sample image used here is not an original Parallels image. It was
created using an hexeditor on the basis of the struct that qemu uses.
Good enough for trying to crash the driver, but not for ensuring
compatibility.

This does the first part of the conversion to coroutines, by
wrapping bdrv_read implementations to take the mutex.
Drivers that implement bdrv_read rather than bdrv_co_readv can
then benefit from asynchronous operation (at least if the underlying
protocol supports it, which is not the case for raw-win32), even
though they still operate with a bounce buffer.
raw-win32 does not need the lock, because it cannot yield.
nbd also doesn't probably, but better be safe.

The big conversion of bdrv_read/write to coroutines caused the two
homonymous callbacks in BlockDriver to become reentrant. It goes
like this:
1) bdrv_read is now called in a coroutine, and calls bdrv_read or
bdrv_pread.
2) the nested bdrv_read goes through the fast path in bdrv_rw_co_entry;
3) in the common case when the protocol is file, bdrv_co_do_readv calls
bdrv_co_readv_em (and from here goes to bdrv_co_io_em), which yields
until the AIO operation is complete;
4) if bdrv_read had been called from a bottom half, the main loop
is free to iterate again: a device model or another bottom half
can then come and call bdrv_read again.
This applies to all four of read/write/flush/discard. It would also
apply to is_allocated, but it is not used from within coroutines:
besides qemu-img.c and qemu-io.c, which operate synchronously, the
only user is the monitor. Copy-on-read will introduce a use in the
block layer, and will require converting it.
The solution is "simply" to convert all drivers to coroutines! We
just need to add a CoMutex that is taken around affected operations.

Format drivers shouldn't need to bother with things like file names, but rather
just get an open BlockDriverState for the underlying protocol. This patch
introduces this behaviour for bdrv_open implementation. For protocols which
need to access the filename to open their file/device/connection/... a new
callback bdrv_file_open is introduced which doesn't get an underlying file
opened.
For now, also some of the more obscure formats use bdrv_file_open because they
open() the file themselves instead of using the block.c functions. They need to
be fixed in later patches.
