While using indexed colors, the last color was never added to the palette.
Triggered with ubuntu livecd.

vnc-auth-sals.c. Because it's tooooo long.

Move sdl, vnc, curses and cocoa UI into ui/ to cleanup
the root directory. Also remove some unnecessary explicit
targets from Makefile.
