If a guest sets up bad descriptors, it could force QEMU to access
non-existing memory regions. Thus we should check the return value
of dma_memory_read/write() to make sure that these errors don't go

When a SCSI command is received from the guest, the CDB length implied
by the first byte might exceed the number of bytes the guest sent. In
this case scsi_req_new() will read uninitialized data, causing
unpredictable behavior.
Adds the buf_len parameter to scsi_req_new() and plumbs it through the
call stack.

The loop condition in xhci_ring_chain_length() is under control of
the guest, and additionally the code does not check for failed DMA
transfers (e.g. if reaching the end of the RAM), so the loop there
could run for a very long time or even forever. Fix it by checking
the return value of dma_memory_read() and by introducing a maximum
loop length.

Canokey core currently using 16 bytes as maximum packet size for
control endpoint, but to run the device in high-speed a 64 bytes
maximum packet size is required according to USB 2.0 specification.
Since we don't acutally need to run the device in high-speed, simply
don't assign high member in USBDesc.
When canokey-qemu is used with xhci, xhci would drive canokey
in high speed mode, since the bcdUSB in canokey-core is 2.1,
yet canokey-core set bMaxPacketSize0 to be 16, this is out
of the spec as the spec said that ``The allowable maximum
control transfer data payload sizes...for high-speed devices,
it is 64 bytes''.
In this case, usb device validation in Windows 10 LTSC 2021
as the guest would fail. It would complain
USB\DEVICE_DESCRIPTOR_VALIDATION_FAILURE.
Note that bcdUSB only identifies the spec version the device
complies, but it has no indication of its speed. So it is
allowed for the device to run in FS but comply the 2.1 spec.
To solve the issue we decided to just drop the high
speed support. This only affects usb-ehci as usb-ehci would
complain speed mismatch when FS device is attached to a HS port.
That's why the .high member was initialized in the first place.
Meanwhile, xhci is not affected as it works well with FS device.
Since everyone is now using xhci, it does no harm to most users.

The 'active' bit passes control over a qTD between the guest and the
controller: set to 1 by guest to enable execution by the controller,
and the controller sets it to '0' to hand back control to the guest.
ehci_state_writeback write two dwords to main memory using DMA:
the third dword of the qTD (containing dt, total bytes to transfer,
cpage, cerr and status) and the fourth dword of the qTD (containing
the offset).
This commit makes sure the fourth dword is written before the third,
avoiding a race condition where a new offset written into the qTD
by the guest after it observed the status going to go to '0' gets
overwritten by a 'late' DMA writeback of the previous offset.
This race condition could lead to 'cpage out of range (5)' errors,
and reproduced by:
./qemu-system-x86_64 -enable-kvm -bios $SEABIOS/bios.bin -m 4096 -device usb-ehci -blockdev driver=file,read-only=on,filename=/home/aengelen/Downloads/openSUSE-Tumbleweed-DVD-i586-Snapshot20220428-Media.iso,node-name=iso -device usb-storage,drive=iso,bootindex=0 -chardev pipe,id=shell,path=/tmp/pipe -device virtio-serial -device virtconsole,chardev=shell -device virtio-rng-pci -serial mon:stdio -nographic
(press a key, select 'Installation' (2), and accept the default
values. On my machine the 'cpage out of range' is reproduced while

qemu_oom_check() is a function which essentially says "if you pass me
a NULL pointer then print a message then abort()". On POSIX systems
the message includes strerror(errno); on Windows it includes the
GetLastError() error value printed as an integer.
Other than in the implementation of qemu_memalign(), we use this
function only in hw/usb/redirect.c, for three checks:
* on a call to usbredirparser_create()
* on a call to usberedirparser_serialize()
* on a call to malloc()
The usbredir library API functions make no guarantees that they will
set errno on errors, let alone that they might set the
Windows-specific GetLastError string. malloc() is documented as
setting errno, not GetLastError -- and in any case the only thing it
might set errno to is ENOMEM. So qemu_oom_check() isn't the right
thing for any of these. Replace them with straightforward
error-checking code. This will allow us to get rid of
qemu_oom_check().

returns error in that case) we don't need to remember the packet.
Avoid using the usb_packet field in OHCIState (as that can be a
waiting async packet on another endpoint) and allocate and use a local
USBPacket for the iso transfer instead. After this we don't have to
care if we're called from a completion callback or not so we can drop

The function scsi_bus_new() creates a new SCSI bus; callers can
either pass in a name argument to specify the name of the new bus, or
they can pass in NULL to allow the bus to be given an automatically
generated unique name. Almost all callers want to use the
autogenerated name; the only exception is the virtio-scsi device.
Taking a name argument that should almost always be NULL is an
easy-to-misuse API design -- it encourages callers to think perhaps
they should pass in some standard name like "scsi" or "scsi-bus". We
don't do this anywhere for SCSI, but we do (incorrectly) do it for
other bus types such as i2c.
The function name also implies that it will return a newly allocated
object, when it in fact does in-place allocation. We more commonly
name such functions foo_init(), with foo_new() being the
allocate-and-return variant.
Replace all the scsi_bus_new() callsites with either:
* scsi_bus_init() for the usual case where the caller wants
an autogenerated bus name
* scsi_bus_init_named() for the rare case where the caller
needs to specify the bus name
and document that for the _named() version it's then the caller's
responsibility to think about uniqueness of bus names.

Per xHCI spec v1.2 chapter 4.17.5 page 296:
If MSI or MSI-X interrupts are enabled, Interrupt Pending (IP)
shall be cleared automatically when the PCI dword write generated
by the interrupt assertion is complete.
Currently QEMU does not clear the IP flag in the MSI / MSI-X mode.
This causes subsequent spurious interrupt to be delivered to guests.
To solve this, we change the xhci intr_raise() hook routine to have
a bool return value that is passed to its caller (the xhci core),
with true indicating that IP should be self-cleared.

When trying to remove the -usbdevice option, there were complaints that
"-usbdevice braille" is still a very useful shortcut for some people.
Thus we never remove this option. Since it's not such a big burden to
keep it around, and it's also convenient in the sense that you don't
have to worry to enable a host controller explicitly with this option,
we should remove it from he deprecation list again.
However, there is one exception: "-usbdevice audio" should go away, since
audio devices without "audiodev=..." parameter are also on the deprecation
list and you cannot use "-usbdevice audio" with "audiodev".

In order to keep track of the alternate setting that should be used for
a given interface, the USBDevice struct keeps an array of alternate
setting values, which is indexed by the interface number. In
usb_host_set_interface, when this array is updated, usb_host_ep_update
is called as a result. However, when usb_host_ep_update accesses the
active libusb_config_descriptor, it indexes udev->altsetting with the
loop variable, rather than the interface number.
With the simple trace backend enable, this behavior can be seen:
[...]
usb_xhci_xfer_start 0.440 pid=1215 xfer=0x5596a4b85930 slotid=0x1 epid=0x1 streamid=0x0
usb_packet_state_change 1.703 pid=1215 bus=0x1 port=b'1' ep=0x0 p=0x5596a4b85938 o=b'undef' n=b'setup'
usb_host_req_control 2.269 pid=1215 bus=0x1 addr=0x5 p=0x5596a4b85938 req=0x10b value=0x1 index=0xd
usb_host_set_interface 0.449 pid=1215 bus=0x1 addr=0x5 interface=0xd alt=0x1
usb_host_parse_config 2542.648 pid=1215 bus=0x1 addr=0x5 value=0x2 active=0x1
usb_host_parse_interface 1.804 pid=1215 bus=0x1 addr=0x5 num=0xc alt=0x0 active=0x1
usb_host_parse_endpoint 2.012 pid=1215 bus=0x1 addr=0x5 ep=0x2 dir=b'in' type=b'int' active=0x1
usb_host_parse_interface 1.598 pid=1215 bus=0x1 addr=0x5 num=0xd alt=0x0 active=0x1
usb_host_req_emulated 3.593 pid=1215 bus=0x1 addr=0x5 p=0x5596a4b85938 status=0x0
usb_packet_state_change 2.550 pid=1215 bus=0x1 port=b'1' ep=0x0 p=0x5596a4b85938 o=b'setup' n=b'complete'
usb_xhci_xfer_success 4.298 pid=1215 xfer=0x5596a4b85930 bytes=0x0
[...]
In particular, it is seen that although usb_host_set_interface sets the
alternate setting of interface 0xd to 0x1, usb_host_ep_update uses 0x0
as the alternate setting due to using the incorrect index to
udev->altsetting.
Fix this problem by getting the interface number from the active
libusb_config_descriptor, and then using that as the index to
udev->altsetting.

Currently, blk_is_read_only() tells whether a given BlockBackend can
only be used in read-only mode because its root node is read-only. Some
callers actually try to answer a slightly different question: Is the
BlockBackend configured to be writable, by taking write permissions on
the root node?
This can differ, for example, for CD-ROM devices which don't take write
permissions, but may be backed by a writable image file. scsi-cd allows
write requests to the drive if blk_is_read_only() returns false.
However, the write request will immediately run into an assertion
failure because the write permission is missing.
This patch introduces separate functions for both questions.
blk_supports_write_perm() answers the question whether the block
node/image file can support writable devices, whereas blk_is_writable()
tells whether the BlockBackend is currently configured to be writable.
All calls of blk_is_read_only() are converted to one of the two new
functions.
