Clean up includes so that osdep.h is included first and headers
which it implies are not included manually.

commit 01c22f2cdd ("main-loop: Suppress
"I/O thread spun" warnings for qtest") doesn't actually disable the
warning for everyone since some tests don't run under the qtest
accelerator.
Check qtest_driver instead.

Warning from the Sparse static analysis tool:
stubs/qtest.c:14:6:
warning: symbol 'qtest_allowed' was not declared. Should it be static?
Add the missing include statement which declares qtest_allowed.

When running under qtest we don't actually have any vcpu threads
to be starved, so the warning about the I/O thread spinning isn't
relevant, and the way qtest manipulates the simulated clock means
the warning is produced a lot as a false positive. Suppress it if
qtest_enabled(), so 'make check' output is less noisy.
