constify ui_out_impl
...
This patch constifies ui_out_impl in struct ui_out, and various
instances of ui_out_impl.
This removes a couple of FIXME comments (near cli_ui_out_impl and
mi_ui_out_impl) that did not make sense to me.
Tested by rebuilding.

destructor if present.
(clear_table): Local function which clears the table data of a
ui_out object.
(ui_out_destroy): Public function which frees a ui_out object.

* ui-out.h: Provide prototype for make_cleanup_ui_out_list_end.
* stack.c (print_frame) [UI_OUT]: Call
make_cleanup_ui_out_list_end to make sure we mark the end of the
list if we do a non-local exit. At the end of the function,
instead of calling ui_out_list_end directly, let do_cleanups
handle it.
