Greg KH 02eca3f5f5 tg3: add device id of Apple Thunderbolt Ethernet device
The Apple Thunderbolt ethernet device is already listed in the driver,
but not hooked up in the MODULE_DEVICE_TABLE().  This fixes that and
allows it to work properly.

Michael Chan c1f5163de4 bnx2: Fix bug in bnx2_free_tx_skbs().
In rare cases, bnx2x_free_tx_skbs() can unmap the wrong DMA address
when it gets to the last entry of the tx ring.  We were not using
the proper macro to skip the last entry when advancing the tx index.

Julia Lawall 022f09784b drivers/net/ethernet/broadcom/cnic.c: remove invalid reference to list iterator variable
If list_for_each_entry, etc complete a traversal of the list, the iterator
variable ends up pointing to an address at an offset from the list head,
and not a meaningful structure.  Thus this value should not be used after
the end of the iterator.  There does not seem to be a meaningful value to
provide to netdev_warn.  Replace with pr_warn, since pr_err is used
elsewhere.

This problem was found using Coccinelle (http://coccinelle.lip6.fr/).

Eric Dumazet a334b5fb19 bnx2x: populate skb->l4_rxhash
l4_rxhash is set on skb when rxhash is obtained from canonical 4-tuple
over transport ports/addresses.

We can set skb->l4_rxhash for all incoming TCP packets on bnx2x for
free, as cqe status contains a hash type information.

Eric Dumazet acfa9e94e2 net: dont use __netdev_alloc_skb for bounce buffer
commit a1c7fff7e1 (net: netdev_alloc_skb() use build_skb()) broke b44 on
some 64bit machines.

It appears b44 and b43 use __netdev_alloc_skb() instead of alloc_skb()
for their bounce buffers.

There is no need to add an extra NET_SKB_PAD reservation for bounce
buffers :

- In TX path, NET_SKB_PAD is useless

- In RX path in b44, we force a copy of incoming frames if
  GFP_DMA allocations were needed.

Michael Chan ae0eef6608 cnic: Fix mmap regression.

    cnic: Remove uio mem[0].

introduced a regression as older versions of userspace app still rely
on this mmap.  Restore the mmap functionality and get the base address
from pci_resource_start() as the nedev->base_addr has been deprecated for
PCI devices.

Update version to 2.5.12.

Michael Chan a2028b2376 cnic: Fix occasional NULL pointer dereference during reboot.
We register with bnx2x before we allocate ctx_tbl structure, so it is
possible for bnx2x to call cnic_ctl before the structure is allocated.
This can sometimes cause NULL pointer dereference of cp->ctx_tbl.  We
fix this by adding simple checking for valid state before proceeding.
The cnic_ctl call is RCU protected so we don't have to deal with race
conditions.

Because of the additional checking, we need to finish the shutdown
before clearing the CNIC_UP flag.

Merav Sicron 37ae41a965 bnx2x: Move the CNIC L2 CIDs to be right after the RSS CIDs
Currently the CNIC-related L2 CIDs (for sending control FCoE / iSCSI packets)
were at fixed position, according to the maximal number of RSS queues multiplied
by the number of traffic-classes. This change makes the CIDs dynamic, as they
are defined to be right after the highest RSS CID. This decreases the memory
allocated for the context.

Merav Sicron 65565884fb bnx2x: Make the transmission queues adjacent
In the current scheme the transmission queues of traffic-class 0 were 0-15, the
transmission queues of traffic-class 1 were 16-31 and so on. If the number of
RSS queues was smaller than 16, there were gaps in transmission queues
numbering, as well as in CIDs numbering. This is both a waste (especially when
16 is increased to 64), and may causes problems with flushing queues when
reducing the number of RSS queues (using ethtool -L). The new scheme eliminates
the gaps.

Merav Sicron a052997ea3 bnx2x: Allow more than 64 L2 CIDs
With increased number of RSS queues, each multiplied by the number of traffic-
classes, we may have up to 64*3=192 CIDs. The current driver scheme with regard
to context allocation supports only 64 CIDs. The new scheme enables scatter-
gatehr list of pages for the context.

1. In multi-function device, show only the online tests in self-test results as
   only these test are performed (offline tests cannot be performed as they may
   corrupt the traffic of other functions on the same physical port). Note that
   multi-function mode cannot change while the driver is up.
2. Check result code in NIC load and act accordingly.

Yuval Mintz 79504d708d bnx2x: correct LPI pass-through configuration
Commit c8c60d88c5 contained
an incorrect logic which enabled a buffer overflow when accessing
an array during LPI pass-through configuration.
This patch fixes this issue by removing that logic altogether.

Eric Dumazet bc14786a10 bnx2x: fix panic when TX ring is full
There is a off by one error in the minimal number of BD in
bnx2x_start_xmit() and bnx2x_tx_int() before stopping/resuming tx queue.

A full size GSO packet, with data included in skb->head really needs
(MAX_SKB_FRAGS + 4) BDs, because of bnx2x_tx_split()

This error triggers if BQL is disabled and heavy TCP transmit traffic
occurs.

bnx2x_tx_split() definitely can be called, remove a wrong comment.

Eric Dumazet d6cb3e4138 bnx2x: fix checksum validation
bnx2x driver incorrectly sets ip_summed to CHECKSUM_UNNECESSARY on
encapsulated segments. TCP stack happily accepts frames with bad
checksums, if they are inside a GRE or IPIP encapsulation.

Our understanding is that if no IP or L4 csum validation was done by the
hardware, we should leave ip_summed as is (CHECKSUM_NONE), since
hardware doesn't provide CHECKSUM_COMPLETE support in its cqe.

Then, if IP/L4 checksumming was done by the hardware, set

Joe Perches 6469933605 ethernet: Remove casts to same type
Adding casts of objects to the same type is unnecessary
and confusing for a human reader.

For example, this cast:

I used the coccinelle script below to find and remove these
unnecessary casts.  I manually removed the conversions this
script produces of casts with __force, __iomem and __user.

Resolved the iwlwifi conflict with mainline using 3-way diff posted
by John Linville and Stephen Rothwell.  In 'net' we added a bug
fix to make iwlwifi report a more accurate skb->truesize but this
conflicted with RX path changes that happened meanwhile in net-next.

In e1000e a conflict arose in the validation code for settings of
adapter->itr.  'net-next' had more sophisticated logic so that
logic was used.

Ariel Elior 24f06716cd bnx2x: bug fix when loading after SAN boot
This is a bug fix for an "interface fails to load" issue.
The issue occurs when bnx2x driver loads after UNDI driver was previously
loaded over the chip. In such a scenario the UNDI driver is loaded and operates
in the pre-boot kernel, within its own specific host memory address range.
When the pre-boot stage is complete, the real kernel is loaded, in a new and
distinct host memory address range. The transition from pre-boot stage to boot
is asynchronous from UNDI point of view.

A race condition occurs when UNDI driver triggers a DMAE transaction to valid
host addresses in the pre-boot stage, when control is diverted to the real
kernel. This results in access to illegal addresses by our HW as the addresses
which were valid in the preboot stage are no longer considered valid.
Specifically, the 'was_error' bit in the pci glue of our device is set. This
causes all following pci transactions from chip to host to timeout (in
accordance to the pci spec).

Eric Dumazet 1191cb8348 bnx2x: remove some bloat

was inlined in bnx2x/bnx2x_cmn.h for no good reason and made my work not
very easy.

Eric Dumazet 8d4057a938 tg3: provide frags as skb head
This patch converts tg3 driver, one of our reference drivers, to use new
build_skb() api in frag mode.

Instead of using kmalloc() to allocate the memory block that will be
used by build_skb() as skb->head, we use a page fragment.

This is a followup of patch "net: allow skb->head to be a page fragment"

This allows GRO, TCP coalescing, and splice() to be more efficient.

Incidentally, this also removes SLUB slow path contention in kfree()

Eric Dumazet d3836f21b0 net: allow skb->head to be a page fragment
skb->head is currently allocated from kmalloc(). This is convenient but
has the drawback the data cannot be converted to a page fragment if
needed.

We have three spots were it hurts :

1) GRO aggregation

 When a linear skb must be appended to another skb, GRO uses the
frag_list fallback, very inefficient since we keep all struct sk_buff
around. So drivers enabling GRO but delivering linear skbs to network
stack aren't enabling full GRO power.

2) splice(socket -> pipe).

 We must copy the linear part to a page fragment.
 This kind of defeats splice() purpose (zero copy claim)

3) TCP coalescing.

 Recently introduced, this permits to group several contiguous segments
into a single skb. This shortens queue lengths and save kernel memory,
and greatly reduce probabilities of TCP collapses. This coalescing
doesnt work on linear skbs (or we would need to copy data, this would be
too slow)

Given all these issues, the following patch introduces the possibility
of having skb->head be a fragment in itself. We use a new skb flag,
skb->head_frag to carry this information.

build_skb() is changed to accept a frag_size argument. Drivers willing
to provide a page fragment instead of kmalloc() data will set a non zero
value, set to the fragment size.

Then, on situations we need to convert the skb head to a frag in itself,
we can check if skb->head_frag is set and avoid the copies or various
fallbacks we have.

This means drivers currently using frags could be updated to avoid the
current skb->head allocation and reduce their memory footprint (aka skb
truesize). (thats 512 or 1024 bytes saved per skb). This also makes
bpf/netfilter faster since the 'first frag' will be part of skb linear
part, no need to copy data.

Matt Carlson f891ea1634 tg3: Avoid panic from reserved statblk field access
When RSS is enabled, interrupt vector 0 does not receive any rx traffic.
The rx producer index fields for vector 0's status block should be
considered reserved in this case.  This patch changes the code to
respect these reserved fields, which avoids a kernel panic when these
fields take on non-zero values.

Following patch adds afex multifunction support to the driver (afex
multifunction is based on vntag header) and updates FW version used to 7.2.51.

Support includes the following:

1. Configure vif parameters in firmware (default vlan, vif id, default
   priority, allowed priorities) according to values received from NIC.
2. Configure FW to strip/add default vlan according to afex vlan mode.
3. Notify link up to OS only after vif is fully initialized.
4. Support vif list set/get requests and configure FW accordingly.
5. Supply afex statistics upon request from NIC.
6. Special handling to L2 interface in case of FCoE vif.

Dan Carpenter 7e5998aa74 bnx2x: off by one in bnx2x_ets_e3b0_sp_pri_to_cos_set()
The sp_pri_to_cos[] array size depends on the config but lets say it is
BX_E3B0_MAX_NUM_COS_PORT0 and max_num_of_cos is also
DCBX_E3B0_MAX_NUM_COS_PORT0.  In the original code
"pri == max_num_of_cos" was accepted but it is one past the end of the
array.

Also we used "pri" before capping it.  It's a harmless read past the end
of the array, but it would affect which error message gets printed.

Yaniv Rosner 59a2e53b82 bnx2x: Fix link issue for BCM8727 boards.
This patch fixes a link problem on BCM57712 + BCM8727 designs in which the TX
laser is controller by GPIO, after 1.60.xx drivers were previously loaded.
On these designs the TX_LASER is enabled by logic AND between the PHY
(through MDIO), and the GPIO. When an old driver is used, it disables the
MDIO part, hence the GPIO control had no affect de facto.

Dmitry Kravkov 30a5de7723 bnx2x: added support for working with one msix irq.
Until now, the bnx2x driver needed at least 2 available msix interrupt

Yuval Mintz 452427b015 bnx2x: previous driver unload revised
The flow in which the bnx2x driver starts after a previous driver
has been terminated in an 'unclean' manner has several bugs and
FW risks, which makes it possible for the driver to fail after
boot-from-SAN or kdump.

Michael Chan 7ae5289017 tg3: Fix RSS ring refill race condition
The RSS feature in tg3 hardware has only one rx producer ring for all
RSS rings.  NAPI vector 1 is special and handles the refilling of the
rx producer ring on behalf of all RSS rings.  There is a race condition
between these RSS NAPIs and the NAPI[1].  If NAPI[1] finishes checking
for refill and then another RSS ring empties the rx producer ring
before NAPI[1] exits NAPI, the chip will be completely out of SKBs in
the rx producer ring.

We fix this by adding a flag tp->rx_refill and rely on napi_schedule()/
napi_complete() to help synchronize it to close the race condition.

Update driver version to 3.123.

When cycling the interface down and up, bnx2x_init_firmware() knows that
the firmware is already loaded, but nevertheless it allocates certain
arrays anew (init_data, init_ops, init_ops_offsets, iro_arr). The old
arrays are leaked.

Fix the leaks by returning early if the firmware was already loaded.
Because if the firmware is loaded, so are the arrays.

John Fastabend 2b88f2de30 net: dcb: getnumtcs()/setnumtcs() should return an int
{g|s}etnumtcs() today returns a u8 that is only used by the DCB code
to verify no error occurred. Today the driver implementations return
negative error codes which end up being non-zero so the logic works
out but triggers some sparse warnings.

To fix the sparse warnings convert the return value to an int.

Matt Carlson 65ec698d13 tg3: Fix tg3_get_stats64 for 5700 / 5701 devs
tg3_get_stats64() takes tp->lock when dealing with non-serdes bcm5700
and bcm5701 devices.  However, functions that call tg3_halt() have
already acquired tp->lock.  When tg3_get_stats64() is called in
tg3_halt(), deadlock will occur.

This patch fixes the problem by separating the stat gathering code into
a new tg3_get_nstats() function.  tg3_get_stats64() is recoded to call
this function and take tp->lock.  The code that takes tp->lock in
tg3_calc_crc_errors() has been removed.  Function signatures have been
cleaned up too.

Matt Carlson 21f7638e6f tg3: Create timer helper functions
This patch seeks to clean up the timer related code.  It begins by
moving one-time timer setup code from tg3_open() to tg3_init_one().
It then creates a function that encapsulates the code needed to start
the timer.  A tg3_timer_stop() function was added for parity.  Finally,
this patch moves all the timer functions to a more suitable location.

Matt Carlson c71013597d tg3: Clear RECOVERY_PENDING with reset_task_cancel
If an error happens in the tx completion thread, tg3_reset_task will be
scheduled and TX_RECOVERY_PENDING will be set.  The TX_RECOVERY_PENDING
flag causes tg3_poll[_msix] to return early before doing much of its
work.  Tg3_reset_task() gets canceled when the configuration of the
device is changing, which always results in a chip reset.  When this
happens, the TX_RECOVERY_PENDING flag may be left set, which would
unnecessarily hinder tg3_poll from doing work.  This patch fixes the
problem.

Matt Carlson 34655ad638 tg3: Fix link check in tg3_adjust_link
The tg3 driver tried to detect link changes by comparing the tg3 local
active_speed member with SPEED_UNKNOWN (or formerly SPEED_INVALID).
This check is not correct, since phylib will never set its speed member
to either of these two values.  The code only appeared to work because
tg3 initializes active_speed to SPEED_INVALID during tg3_init_one.  This
patch introduces a new "old_link" tg3 member and then compares the
phy_device's link member against it to detect link state changes.

Dmitry Kravkov fe603b4d68 bnx2x: add gro_check
The patch provides workaround for BUG in FW 7.2.16,
which in GRO mode may miscalculate buffer and
place on SGE one frag less than it could.
It may happen only for some MTUs, we mark these MTUs
with gro_check flag during device initialization or
MTU change.

Next FW should include fix for the issue and the
patch could be reverted.

Dmitry Kravkov 621b4d66b2 use FW 7.2.16
The patch integrates FW 7.2.16 HSI and implements driver
part of GRO flow.

FW 7.2.16 adds the ability to aggregate packets for GRO
(and not just LRO) and also fixes some bugs.

1. Added new aggregation mode: GRO. In this mode packets are aggregated
   such that the original packets can be reconstructed by the OS.
2. 57712 HW bug workaround - initialized all CAM TM registers to 0x32.
3. Adding the FCoE statistics structures to the BNX2X HSI.
4. Wrong configuration of TX HW input buffer size may cause theoretical
   performance effect. Performed configuration fix.
5. FCOE - Arrival of packets beyond task IO size can lead to crash.
   Fix firmware data-in flow.
6. iSCSI - In rare cases of on-chip termination the graceful termination
   timer hangs, and the termination doesn't complete. Firmware fix to MSL
   timer tolerance.
7. iSCSI - Chip hangs when target sends FIN out-of-order or with isles
   open at the initiator side. Firmware implementation corrected to drop
   FIN received out-of-order or with isles still open.
8. iSCSI - Chip hangs when in case of retransmission not aligned to 4-bytes
   from the beginning of iSCSI PDU. Firmware implementation corrected
   to support arbitrary aligned retransmissions.
9. iSCSI - Arrival of target-initiated NOP-IN during intense ISCSI traffic
   might lead to crash. Firmware fix to relevant flow.

Matt Carlson c6700ce224 tg3: Remove unneeded link_config.orig_... members
This patch removes the unneeded link_config.orig_* members.  When the
phy is in a low power state, the TG3_PHYFLG_IS_LOW_POWER flag solely
determines how the link is configured.  When the phy is powered back up,
it can resume using the original settings.

For the phylib case, the link configuration still needs to be saved, but
since the phylib maintains its own configuration, we can repurpose the
(unused in this case) tg3 link configuration members.

would wait for the previous event to complete, then proceed to gather
data through a series of phy accesses.  Since phy accesses are
relatively slow, it is possible for another thread to attempt to submit
its own event while the UMP code is still construction its message.

This patch seeks to minimize the collision window as much as possible by
preloading the phy data.

Jesper Juhl 3f61cd879c bnx2x: Fix mem leak in bnx2x_tpa_stop() if build_skb() fails.
We allocate memory for 'new_data' with kmalloc(). If we get the memory
we then try to build_skb() and if that should fail (which it can) we
do not enter 'if (likely(skb)) {' and actually use 'new_data' but
instead fall through to the 'drop:' label and end up returning from
the function without ever assigning 'new'data' to anything or freeing
it. That leaks the memory allocated to 'new_data'.

This patch fixes the memory leak by doing a kfree(new_data) in the
case where build_skb() fails (or where allocation of 'new_data' itself
fails, but in taht case it's just a harmless kfree(NULL)).

1. Sample mcp pulse and mcp sequence in nic load instead of in init_one
as they may change by the time we want to use them.

2. Allow cnic to access device during nic load (by adding a new "LOADING" state
to recovery flow). This prevents the unnecessary cnic timeout which resulted
by cnic attempting to access because nic is loading, but being blocked because
of the Recovery state.

3. Issue 'fake' driver load command to mcp when last driver unloads to prevent
mcp from taking ownership. When recovery is complete unload fake driver to
allow mcp to initialize the hardware before first driver loads.

Ariel Elior 889b9af34f bnx2x: Track active PFs with bitmap
The recovery register (to which a hardware lock has been added in previous
patch) is used amongst other things to track the active PFs. The old
implementation which used a per path counter is not viable in a virtualized
environment where a pf may increment the counter and then have the kernel
crash around it preventing the counter from ever reaching zero.
In the new implementation the scenario described will result in the PF timing
out against the mcp, which will clear the PF's bit in the bitmask allowing
recovery process to proceed.

Use hardware locks to protect resources common to several Physical Functions. In
a virtualized environment the RTNL lock only protects a PF's driver against
the PFs sharing it's VMs with regard to device resources. Other PFs may reside
in other VMs under other OSs, and are not subject to the lock. Such resources
which were previously protected implicitly by the RTNL lock must now be
protected explicitly with dedicated HW locks.

In a virtualized environment it is possible for a loading driver to discover
that Firmware is already loaded to the device, and that this FW does not match
its own. This can happen for example if different Physical Functions are
Assigned to different VMs in which different driver versions are loaded. The
code in this patch ensures that only drivers with matching FW are loaded over
the device, and that in the case described above where the Firmware version
doesn't match the driver load is aborted.

Ariel Elior 127a425e8f bnx2x: Removing indirect register access
In virtualized environments indirect access to the device may not be supported
(depending on the Hypervisor type). Indirect device access was used since in
some harware contexts (i.e. certain chipset and BIOS) every access the driver
makes across the pci is followed by a BIOS initiated Zero Length Read to the
same address. When accessing widebus registers this zero length read corrupts
the serialization of the read/write sequence resulting with errors. To avoid
this problem widebus registers are always accessed via the DMAE or the indirect
interface. However, the 57712x and 578xx devices intercept the zero length read
and so using the indirect interface with these devices is not necessary. Since
PDA is only supported for 57712x and 578xx the indirect access to device was
restricted to 57710 and 57711x.

that there are enough credits to support additional vlan-macs,

Matt Carlson c3b5003b62 tg3: Fix single-vector MSI-X code
Kdump kernels leave MSI-X interrupts (as setup by the crashed kernel)
enabled.  However, kdump only enables one CPU in the new environment,
thus causing tg3 to abort MSI-X setup.  When the driver attempts to
enable INTA or MSI interrupt modes on a kdump kernel, interrupt
delivery fails.

This patch attempts to workaround the problem by forcing the driver
to enable a single MSI-X interrupt.  In such a configuration, the
device's multivector interrupt mode must be disabled.

Super-Isolate mode.

Michael Chan 23021c2105 cnic: Improve error recovery on bnx2x devices
When a bnx2x device encounters parity errors, it will not respond to all
SPQ messages.  As a result, the shutdown sequence before reset can take
a long time as the ulp drivers (bnx2i/bnx2fc) have to wait for timeout
of all such messages.

To improve this scenario, when bnx2x returns error on the SPQ, we'll send
an immediate response to the ulp drivers to avoid such lengthy timeouts.

Adjust the return code of relevant functions to return error only if
the message cannot be sent on the SPQ so that we'll generate an error
completion to the ulp drivers.

Matt Carlson 90415477bf tg3: Make the RSS indir tbl admin configurable
This patch adds the ethtool callbacks necessary to change the rss
indirection table from userspace.  Should the number of interrupts
change (e.g.  across a close / open call, or through a reset) and
any one of the indirection table values fall out-of-range, the driver
will reset the indirection table to a default layout.

[Integrated many suggestions made by Ben Hutchings.]

* Removed TG3_FLAG_SUPPORT_MSIX checks at the start of
  tg3_get_rxfh_indir() and tg3_set_rxfh_indir().

Michael Chan c2c20ef43d bnx2: Update driver to use new mips firmware.
bnx2-mips-06-6.2.3 and bnx2-mips-09-6.2.1.b

New firmware fixes iSCSI problems with some LeftHand targets that don't
set TTT=0xffffffff for Data-In according to spec.  Firmware generates
exception warnings for this condition and becomes very slow.  This is
fixed by suppressing these warnings when using default error mask.

Ben Hutchings 278bc4296b ethtool: Define and apply a default policy for RX flow hash indirection
All drivers that support modification of the RX flow hash indirection
table initialise it in the same way: RX rings are assigned to table
entries in rotation.  Make that default policy explicit by having them
call a ethtool_rxfh_indir_default() function.

In the ethtool core, add support for a zero size value for
ETHTOOL_SRXFHINDIR, which resets the table to this default.

Ben Hutchings 7850f63f16 ethtool: Centralise validation of ETHTOOL_{G, S}RXFHINDIR parameters
Add a new ethtool operation (get_rxfh_indir_size) to get the
indirectional table size.  Use this to validate the user buffer size
before calling get_rxfh_indir or set_rxfh_indir.  Use get_rxnfc to get
the number of RX rings, and validate the contents of the new
indirection table before calling set_rxfh_indir.  Remove this
validation from drivers.

Dmitry Kravkov 7185bb335a bnx2x: properly initialize L5 features
The code is missing initialization of NO_FCOE_FLAG and NO_ISCSI*FLAGS
when CONFIG_CNIC is not selected.
This causes panic during driver load since commit
1d187b34da where NO_FCOE tested
unconditionally (outside #ifdef BCM_CNIC  structure) and
accessed fp[FCOE_IDX] which is not allocated.

Dmitry Kravkov 73dbb5e162 bnx2x: fix crash while ethtool -t
commit 2df1a70aaf  "bnx2x: Support
for byte queue limits" has introduced an asymmetry in usage of
netdev_tx_completed_queue and netdev_tx_sent_queue. Missing
call to netdev_tx_sent_queue causes the crash during ethtool -t.

The patch adds the missing call.
