QEMU side has already imported pvpanic.h from linux, remove bit
definitions from include/hw/misc/pvpanic.h, and use
include/standard-headers/linux/pvpanic.h instead.
Also minor changes for PVPANIC_CRASHLOADED -> PVPANIC_CRASH_LOADED.

To ease the PCI device addition in next patches, split the code as follows:
- generic code (read/write/setup) is being kept in pvpanic.c
- ISA dependent code moved to pvpanic-isa.c
Also, rename:
- ISA_PVPANIC_DEVICE -> PVPANIC_ISA_DEVICE.
- TYPE_PVPANIC -> TYPE_PVPANIC_ISA.
- MemoryRegion io -> mr.
- pvpanic_ioport_* in pvpanic_*.
Update the build system with the new files and config structure.

Advertise both types of events as supported when the guest OS
queries the pvpanic device. Currently only PVPANIC_PANICKED is
exposed; PVPANIC_CRASHLOADED must also be advertised, but only on
new machine types.

Some typedefs and macros are defined after the type check macros.
This makes it difficult to automatically replace their
definitions with OBJECT_DECLARE_TYPE.
Patch generated using:
$ ./scripts/codeconverter/converter.py -i \
--pattern=QOMStructTypedefSplit $(git grep -l '' -- '*.[ch]')
which will split "typdef struct { ... } TypedefName"
declarations.
Followed by:
$ ./scripts/codeconverter/converter.py -i --pattern=MoveSymbols \
$(git grep -l '' -- '*.[ch]')
which will:
- move the typedefs and #defines above the type check macros
- add missing #include "qom/object.h" lines if necessary

Handle bit 1 write, then post event to monitor.
Suggested by Paolo, declear a new event, using GUEST_PANICKED could
cause upper layers to react by shutting down or rebooting the guest.

sysemu/sysemu.h is a rather unfocused dumping ground for stuff related
to the system-emulator. Evidence:
* It's included widely: in my "build everything" tree, changing
sysemu/sysemu.h still triggers a recompile of some 1100 out of 6600
objects (not counting tests and objects that don't depend on
qemu/osdep.h, down from 5400 due to the previous two commits).
* It pulls in more than a dozen additional headers.
Split stuff related to run state management into its own header
sysemu/runstate.h.
Touching sysemu/sysemu.h now recompiles some 850 objects. qemu/uuid.h
also drops from 1100 to 850, and qapi/qapi-types-run-state.h from 4400
to 4200. Touching new sysemu/runstate.h recompiles some 500 objects.
Since I'm touching MAINTAINERS to add sysemu/runstate.h anyway, also
add qemu/main-loop.h.

In my "build everything" tree, changing hw/qdev-properties.h triggers
a recompile of some 2700 out of 6600 objects (not counting tests and
objects that don't depend on qemu/osdep.h).
Many places including hw/qdev-properties.h (directly or via hw/qdev.h)
actually need only hw/qdev-core.h. Include hw/qdev-core.h there
instead.
hw/qdev.h is actually pointless: all it does is include hw/qdev-core.h
and hw/qdev-properties.h, which in turn includes hw/qdev-core.h.
Replace the remaining uses of hw/qdev.h by hw/qdev-properties.h.
While there, delete a few superfluous inclusions of hw/qdev-core.h.
Touching hw/qdev-properties.h now recompiles some 1200 objects.

This causes two slight backwards-incompatibilities between "-M pc-1.5"
and 1.5's "-M pc":
(1) a fw_cfg file is removed with this patch. This is only a problem
if migration stops the virtual machine exactly during fw_cfg enumeration.
(2) after migration, a VM created without an explicit "-device pvpanic"
will stop reporting panics to management.
The first problem only occurs if migration is done at a very, very
early point (and I'm not sure it can happen in practice for reasonable-size
VMs, since it will likely take more time to send the RAM to destination,
than it will take for BIOS to scan fw_cfg).
The second problem only occurs if the guest panics _and_ has a guest
driver _and_ management knows to look at the crash event, so it is
mostly theoretical at this point in time.
Thus keep the code simple, and pretend it was never broken.

Avoid use of static variables: PC systems
initialize pvpanic device through pvpanic_init,
so we can simply create the fw_cfg file at that point.
This also makes it possible to skip device
creation completely if fw_cfg is not there, e.g. for xen -
so the ports it reserves are not discoverable by guests.
Also, make pvpanic_init void since callers ignore return
status anyway.

pvpanic device is used to send guest panic event from guest to qemu.
When guest panic happens, pvpanic device driver will write a event
number to IO port 0x505(which is the IO port occupied by pvpanic device,
by default). On receiving the event, pvpanic device will pause guest
cpu(s), and send a qmp event QEVENT_GUEST_PANICKED.
