This patch adds a helper function to deliver restart irqs. To be able to be used
by kvm, the psw load/store methods have to perform special cc-code handling only
when running with tcg.

This patch adds handling code for the following SIGP orders:
- SIGP SET ARCHITECTURE
- SIGP SET PREFIX
- SIGP STOP
- SIGP STOP AND STORE STATUS
- SIGP STORE STATUS AT ADDRESS
SIGP STOP (AND STORE STATUS) are the only orders that can stay pending forever
(and may only be interrupted by resets), so special care has to be taken about
them. Their status also has to be tracked within QEMU. This patch takes
care of migrating this status (e.g. if migration happens during a SIGP STOP).
Due to the BQL, only one VCPU is currently able to execute SIGP handlers at a
time. According to the PoP, BUSY should be returned if another SIGP order is
currently being executed on a VCPU. This can only be implemented when the BQL
does not protect all handlers. For now, all SIGP orders on all VCPUs will be
serialized, which will be okay for the first shot.

Whenever we set the SIGP status in the status register, we have to wipe out
the lower 4 bytes and keep the higher 4 bytes. Also the condition code will
always be set to STATUS_STORED.
Let's introduce the wrapper for SigpInfo, as this will avoid most duplicate
code in the future.

Whenever a sigp order is to be executed by a target vcpu, we use run_on_cpu().
As we have only one pointer to pass all data to these sigp handlers, let's
introduce the struct sigp_info and use it as a transport container.
All orders targeting a single vcpu are now dispatched from a separate
handler. The destination vcpu is only valid for these orders and must not be
checked for SIGP SET ARCHITECTURE.
The sigp_info is filled with life in this new handler and used to pass the
information about the sigp order to the existing handlers. The cc is set
within these handlers.
Rename sigp_cpu_start() and sigp_cpu_restart() on the way to match the SIGP
order names (in order to avoid touching affected lines several times).

Change the TSCH handler to use the new logical memory access functions.
Since the channel should not be updated in case of a protection or access
exception while writing to the guest memory, the css_do_tsch() has to be
split up into two parts, one for retrieving the IRB and one for the update.

According to the POP specification, the parameter blocks of various
functions like the IO instructions are accessed with logical addresses.
Thus we need a function that can read or write a buffer from/to the
guest's logical address space.
This patch now provides a function that can be used to access virtual
guest memory by using the mmu_translate function of QEMU to convert
the virtual addresses to physical.

Program access exceptions are defined to deliver a translation exception
code in the low-core. Add a function trigger_access_exception() that
generates the proper program interrupt on both KVM and non-KVM systems
and switch the existing code to use it.

An Address Space Control Element (ASCE) is only the very first unit of
an s390 address translation (normally residing in one of the control
registers). The entries in the page tables are called differently.
So let's call the relevant variable pt_entry instead of asce in
mmu_translate_pte() to avoid future confusion (thus there is no
functional change in this patch, just renaming).

The address space bits in the translation exception code were wrong.
In fact, we can simply copy the bits from the PSW, so there's no need
for the trans_bits() function anymore.
Additionally, we now also set the fetch/store bits in the translation
exception code, so a guest can determine whether the exception occured
during a write or during a read.

When a fault occurs during the MMU lookup in s390_cpu_get_phys_page_debug(),
the trigger_page_fault() function writes the translation exception code
into the lowcore - something you would not expect during a memory access
by the debugger. Ease this problem by adding an additional parameter to
mmu_translate() which can be used to specify whether a program check and
the translation exception code should be injected or not.

The current code used a wrong and very confusing way of dealing with
the table levels by introducing a "fake level above current". However,
the real problem was simply that the checks for the region/segment
invalid bit and for the matching region/segment level was done at the
wrong spot in the code - it has to be done after the first table entry
has been looked up instead (e.g. there is also no "invalid" bit in the
ASCE itself and the current "level" has to be the same as the level in
the entry that we just looked up).
Also the entries for the segment table are quite a bit different compared
to the region table entries. So this patch moves the related code into the
function mmu_translate_segment() to make it clear at which table level we
currently are and to get rid of the ugly switch-statement in the function
mmu_translate_region().

Whenever a reboot initiated by the guest is done, the reipl parameters should
remain valid. The disk configured by the guest is to be used for
ipl'ing. External reboot/reset request (e.g. via virsh reset guest) should
completely reset the guest to the initial state, and therefore also reset the
reipl parameters, resulting in an ipl behaviour of the initially configured
guest. This could be an external kernel or a disk.

To support dynamically updating the IPL device from inside the KVM
guest on the s390 platform, DIAG 308 instruction is intercepted
in QEMU to handle the request.
Subcode 5 allows to specify a new boot device, which is saved for
later in the s390_ipl device. This also allows to switch from an
external kernel to a boot device.
Subcode 6 retrieves boot device configuration that has been previously
set.

This patch fixes the bug with borrow_in being set incorrectly, but it
also simplifies the logic to be much more plain, improving speed. It
fixes both the 32-bit SLB* and 64-bit SLBG*.
The SLBG* change has been well-tested. I haven't tested the SLB* change
explicitly, but the code was copy-pasted from the tested code.
The error of these functions' current implementations would not likely
be triggered by compiler-generated code, since the only error was in the
state of the carry/borrow flag. Compilers rarely generate an
instruction sequence such as carry-set -> carry-set-and-use ->
carry-use.
(With Paolo's fix and mine, there are still a couple of failures from
GMP's testsuite, but they are almost surely due to incorrect code
generation from gcc 4.9. But since this gcc is running under qemu, it
might be qemu bugs. I intend to investigate this.)

As described in CP programming services an unimplemented DIAGNOSE
function should return a specification exception. Today we give the
guest an operation exception.
As both exception types are suppressing and Linux as a guest does not
care about the type of program check in its exception table handler
as long as both types have the same kind of error handling (nullifying,
terminating, suppressing etc.) this was unnoticed.

The TARGET_HAS_ICE #define is intended to indicate whether a target-*
guest CPU implementation supports the breakpoint handling. However,
all our guest CPUs have that support (the only two which do not
define TARGET_HAS_ICE are unicore32 and openrisc, and in both those
cases the bp support is present and the lack of the #define is just
a bug). So remove the #define entirely: all new guest CPU support
should include breakpoint handling as part of the basic implementation.

on s390 MSI-X irqs are presented as thin or adapter interrupts
for this we have to reorganize the routing entry to contain
valid information for the adapter interrupt code on s390.
To minimize impact on existing code we introduce an architecture
function to fixup the routing entry.

This patch implements the s390 pci instructions in qemu. It allows
to access and drive pci devices attached to the s390 pci bus.
Because of platform constrains devices using IO BARs are not
supported. Also a device has to support MSI/MSI-X to run on s390.

This patch implements a pci bus for s390x together with infrastructure
to generate and handle hotplug events, to configure/unconfigure via
sclp instruction, to do iommu translations and provide s390 support for

We can avoid loads of syscalls when dropping to user space by storing the values
of more registers directly within kvm_run.
Support is added for:
- ARCH0: CPU timer, clock comparator, TOD programmable register,

The css functions are only used from ioinst.c and other files that are
only built for CONFIG_SOFTMMU. So we do not need the dummy wrappers for

Compute kvm_irqfds_allowed by checking the KVM_CAP_IRQFD extension.
Remove direct settings in architecture specific files.
Add a new kvm_resamplefds_allowed variable, initialized by
checking the KVM_CAP_IRQFD_RESAMPLE extension. Add a corresponding
kvm_resamplefds_enabled() function.
A special notice for s390 where KVM_CAP_IRQFD was not immediatly
advirtised when irqfd capability was introduced in the kernel.
KVM_CAP_IRQ_ROUTING was advertised instead.
This was fixed in "KVM: s390: announce irqfd capability",
ebc3226202d5956a5963185222982d435378b899 whereas irqfd support
was brought in 84223598778ba08041f4297fda485df83414d57e,
"KVM: s390: irq routing for adapter interrupts". Both commits
first appear in 3.15 so there should not be any kernel
version impacted by this QEMU modification.

Array index starts at 0, so the valid index of ext_queue array,
io_queue array, mchk_queue array should be MAX_EXT_QUEUE - 1,
MAX_IO_QUEUE - 1, MAX_MCHK_QUEUE - 1.
The original checks missed the invalid bound value, which will lead
possible out of bounds read in the follow codes.

The SAM instructions simply change 2 bits in PSW.MASK to advertise
the current memory mode. While we can't fully guarantee that 31 bit
mode (or even remotely 24 bit mode) actually work correctly, we don't
check whether lpswe modifies these bits, so we shouldn't keep the
guest from executing SAM instructions either.
This patch implements all SAM instrutions with their actual PSW changing
semantics, making more recent Linux kernels boot properly which do issue
a SAM31 call during early boot.

When injecting an sclp console interrupt into the guest, we increase
the PC by 4 for some reason. I have no idea why I put that code there,
but it's clearly wrong. Remove the increment.
This patch fixes sclp serial input for the ccw machine.

We need to synchronize registers after a reset has been performed. The
current code does that in qemu_system_reset(), load_normal_reset() and
modified_clear_reset() for all vcpus. After SIGP (INITIAL) CPU RESET,
this needs to be done for the targeted vcpu as well, so let's call
cpu_synchronize_post_reset() in the respective handlers.

This patch reuses kvm_s390_reset_vcpu() to get rid of some CONFIG_KVM and
CONFIG_USER_ONLY ifdefs in cpu.c.
In order to get rid of CONFIG_USER_ONLY, kvm_s390_reset_vcpu() has to provide a
dummy implementation - the two definitions are moved to the proper section in
cpu.h.

Let QEMU propagate the cpu state to kvm. If kvm doesn't yet support it, it is
silently ignored as kvm will still handle the cpu state itself in that case.
The state is not synced back, thus kvm won't have a chance to actively modify
the cpu state. To do so, control has to be given back to QEMU (which is already
done so in all relevant cases).
Setting of the cpu state can fail either because kvm doesn't support the
interface yet, or because the state is invalid/not supported. Failed attempts
will be traced

This patch makes sure that halting a cpu and stopping a cpu are two different
things. Stopping a cpu will also set the cpu halted - this is needed for common
infrastructure to work (note that the stop and stopped flag cannot be used for
our purpose because they are already used by other mechanisms).
A cpu can be halted ("waiting") when it is operating. If interrupts are
disabled, this is called a "disabled wait", as it can't be woken up anymore. A
stopped cpu is treated like a "disabled wait" cpu, but in order to prepare for a
proper cpu state synchronization with the kvm part, we need to track the real
logical state of a cpu.

Until now, when a s390 cpu was stopped or halted, the number of running
CPUs was tracked in a global variable. This was problematic for migration,
so Jason came up with a per-cpu running state.
As it turns out, we want to track the full logical state of a target vcpu,
so we need real s390 cpu states.
This patch is based on an initial patch by Jason Herne, but was heavily
rewritten when adding the cpu states STOPPED and OPERATING. On the way we
move add_del_running to cpu.c (the declaration is already in cpu.h) and
modify the users where appropriate.
Please note that the cpu is still set to be stopped when it is
halted, which is wrong. This will be fixed in the next patch. The LOAD and
CHECK-STOP state will not be used in the first step.

This patch reduces the core registers to the psw and the general purpose
registers. The fpc and ac registers are handled as coprocessors registers by gdb.
This allows to reuse the feature xml files taken from gdb without further
modification and is what other architectures do.
The target.xml is now generated and provided to the gdb client. Therefore, the
client doesn't have to guess which registers are available at which logical
register number.

Currently, load_normal_reset() and modified_clear_reset() as triggered
by a guest vcpu will initiate cpu resets on the current vcpu thread for
all cpus. The reset should happen on the individual vcpu thread
instead, so let's use run_on_cpu() for this.
This avoids calls to synchronize_rcu() in the kernel.

to inject interrupts with larger payloads (when a kvm backend becomes
available).
Moreover, kvm_s390_floating_interrupt() does no longer have the bogus

The kernel only drops to userspace if an endless program interrupt loop
has been detected. Let's print an error message in this case to inform
the user about the crash and stop the affected CPU with a panic event,
just like it is already done for the external interruption loop detection.

Interception code 0x14 only drops to userspace when an unmanageable
external interruption interception occured (e.g. if the External New
PSW does not disable external interruptions). Instead of bailing out
via the default handler, it is better to inform the user with a
proper error message that also includes the bad PSW, and to stop
the affected CPU with a panic event instead.

Exploit the new api for userspace-controlled cmma. If supported, enable
cmma during kvm initialization and register a reset handler for cmma,
which is also called directly from the load IPL code.
The reset functionality is needed to reset the cmma state of the guest
pages, e.g. if a system reset is triggered via qemu monitor; otherwise
this could result in data corruption.
A guest triggered reboot may now lead to multiple cmma resets; this is
OK, however, as this is slowpath anyway and the simplest way to achieve
the intended effects.

Rather than include helper.h with N values of GEN_HELPER, include a
secondary file that sets up the macros to include helper.h. This
minimizes the files that must be rebuilt when changing the macros
for file N.

This patch allows to insert and remove sw breakpoints using the QEMU gdbserver
on s390 as well as to interrupt execution on a breakpoint hit when running
with KVM enabled.
Whenever a software breakpoint is inserted, common code calls kvm ioctl
KVM_UPDATE_GUEST_DEBUG. As this method's default on s390 is to return an error
if not implement, the insertion will fail. Therefore, KVM also has to be
updated in order to make use of software breakpoints.

When restoring the previously saved instruction in
kvm_arch_remove_sw_breakpoint(), we only restored one byte. Let's use
the sizeof() operator to make sure we restore the entire instruction.
While we are at it, let's remove the duplicate definitions of DIAG 501
and replace its size (used when reading/writing the instruction) with
a sizeof() operator to make the code self explaining and less error-prone.

Make use of the new s390 adapter irq routing support to enable real
in-kernel irqfds for virtio-ccw with adapter interrupts.
Note that s390 doesn't provide the common KVM_CAP_IRQCHIP capability, but
rather needs KVM_CAP_S390_IRQCHIP to be enabled. This is to ensure backward
compatibility.

Now that we have a CPU object with a reset method, it is better to
keep the KVM reset close to the CPU reset. Using qemu_register_reset
as we do now keeps them far apart.
With this patch, PPC no longer calls the kvm_arch_ function, so
it can get removed there. Other arches call it from their CPU
reset handler, and the function gets an ARMCPU/X86CPU/S390CPU.
Note that ARM- and s390-specific functions are called kvm_arm_*
and kvm_s390_*, while x86-specific functions are called kvm_arch_*.
That follows the convention used by the different architectures.
Changing that is the topic of a separate patch.

s390x introduced helper functions for getting/setting one_regs with
commit 860643bc. However, nothing about these is s390-specific.
Alexey Kardashevskiy had already posted a general version, so let's

With the EDAT-1 facility, the MMU translation can stop at the
segment table already, pointing to a 1 MB block. And while we're
at it, move the page table entry handling to a separate function,
too, as suggested by Alexander Graf.

The real-to-absolute address translation in mmu_translate() was
missing the second part for translating the page at the prefix
address back to the 0 page. And while we're at it, also moved the
code into a separate helper function since this might come in
handy for other parts of the code, too.

Some ONE_REGS on s390 are not protected by a capability. Older kernels
might not provide those and return an error. Fortunately these registers
are only critical for the migration path. There is no need to error out
on reset and normal runtime. Furthermore, these kernels don't provide
a proper dirty bitmap anyway, so let's use tracing for those errors.
Also provide generic one reg helper to simplify the code.

This patch creates empty function stubs (used by the gdbserver) in preparation
for the hw debugging support by kvm on s390, which will enable the
__KVM_HAVE_GUEST_DEBUG define in the linux headers and require these methods on

Most targets were using offsetof(CPUFooState, breakpoints) to determine
how much of CPUFooState to clear on reset. Use the next field after
CPU_COMMON instead, if any, or sizeof(CPUFooState) otherwise.

Handle the new CCW_CMD_SET_IND_ADAPTER command enabling adapter interrupts
on guest request. When active, host->guest notifications will be handled
via global_indicator -> queue indicators instead of queue indicators +
subchannel I/O interrupt. Indicators for virtqueues may be present at an
offset.

The current implementation uses the second byte of the instruction
to identify the instruction handler. This is not sufficient to
support instructions not starting with 0xb2. This patch
adds separate handlers for 0xb2, 0xb9 and 0xeb to be able to
support the full instruction set.

The s390_cpu_initial_reset() function had two deficiencies: First, it
used an ioctl for the destination CPU, and this ioctl could block
nearly forever, as long as the destination CPU was running in the SIE
loop. Second, it also cleared the general purpose registers - something
it should not do according to the Principles of Operations.
Since we've already got another function for the initial CPU reset in
cpu.c, we can also use that function instead. And by using run_on_cpu()
for executing this code, we make sure that the destination CPU is
correctly kicked out of kernel mode now.

Some of the state in the kernel can not be reset from QEMU yet.
For this we've got to use the KVM_S390_INITIAL_RESET ioctl to make
sure that the state in the kernel is set to the right values during
initial CPU reset, too.

If the 51 most significant bits of the SCCB address are zero or equal to
the prefix, we should throw an specification exception, too.
Also moved the check for privileged mode to sclp_service_call() to have
all program checks in one place now.

So far, the DIAG 500 hypervisor call was only setting -EINVAL in
R2 when a guest tried to call this function with an illegal subcode.
This patch now changes the behavior so that a specification exception
is thrown instead, since this is the common behavior of other DIAG
functions (and other CPU instructions) when being called with illegal
parameters.

The SET-ARCHITECTURE handler in QEMU caused a program interruption.
This is wrong according to the "Principles of Operations" specification
(since SIGP should never cause a program interrupt) and was likely only
introduced for debugging purposes. Since we handle SET-ARCHITECTURE in
the kernel already and only dropped to user space in case of bad mode
parameters, we should just report INVALID PARAMETER in QEMU instead.

The partial register handling (introduced with commits
420840e58b and
3474b67948 ) aimed to improve intercept
handling performance.
It made the code more complicated though. During development for life
migration/init/reset etc it turned out that this might cause several
hard to debug programming errors. With the introduction of ioeventfd
(and future irqfd patches) the qemu intercept handlers are no longer
hot-path. And therefore the partial register handling can be
removed to simplify the code.
