Swap 'buf' and 'bytes' around for consistency with
blk_co_{pread,pwrite}(), and in preparation to implement these functions
using generated_co_wrapper.

Some typedefs and macros are defined after the type check macros.
This makes it difficult to automatically replace their
definitions with OBJECT_DECLARE_TYPE.

Since commit 61f20b9dc5 ("spapr_nvram: Pre-initialize the NVRAM to
support the -prom-env parameter"), pseries machines can pre-initialize
the "system" partition in the NVRAM with the data passed to all -prom-env
parameters on the QEMU command line.
In this case it is assumed that all the data fits in 64 KiB, but the user
can easily pass more and crash QEMU:

This happens because we don't check if all the prom-env data fits in
the NVRAM and chrp_nvram_set_var() happily memcpy() it passed the
buffer.
This crash affects basically all ppc/ppc64 machine types that use -prom-env:
- pseries (all versions)
- g3beige
- mac99
and also sparc/sparc64 machine types:
- LX
- SPARCClassic
- SPARCbook
- SS-10
- SS-20
- SS-4
- SS-5
- SS-600MP
- Voyager
- sun4u
- sun4v
Add a max_len argument to chrp_nvram_create_system_partition() so that
it can check the available size before writing to memory.
Since NVRAM is populated at machine init, it seems reasonable to consider
this error as fatal. So, instead of reporting an error when we detect that
the NVRAM is too small and adapt all machine types to handle it, we simply
exit QEMU in all cases. This is still better than crashing. If someone
wants another behavior, I guess this can be reworked later.

Without the patch, affected machine types cause QEMU to report some
memory corruption and crash:
malloc(): corrupted top size
free(): invalid size
*** stack smashing detected ***: terminated
With the patch, QEMU prints the following message and exits:
NVRAM is too small. Try to pass less data to -prom-env
It seems that the conditions for the crash have always existed, but it
affects pseries, the machine type I care for, since commit 61f20b9dc5
only.

sysemu/sysemu.h is a rather unfocused dumping ground for stuff related
to the system-emulator. Evidence:
* It's included widely: in my "build everything" tree, changing
sysemu/sysemu.h still triggers a recompile of some 1100 out of 6600
objects (not counting tests and objects that don't depend on
qemu/osdep.h, down from 5400 due to the previous two commits).
* It pulls in more than a dozen additional headers.
Split stuff related to run state management into its own header
sysemu/runstate.h.
Touching sysemu/sysemu.h now recompiles some 850 objects. qemu/uuid.h
also drops from 1100 to 850, and qapi/qapi-types-run-state.h from 4400
to 4200. Touching new sysemu/runstate.h recompiles some 500 objects.
Since I'm touching MAINTAINERS to add sysemu/runstate.h anyway, also
add qemu/main-loop.h.

In my "build everything" tree, changing hw/qdev-properties.h triggers
a recompile of some 2700 out of 6600 objects (not counting tests and
objects that don't depend on qemu/osdep.h).
Many places including hw/qdev-properties.h (directly or via hw/qdev.h)
actually need only hw/qdev-core.h. Include hw/qdev-core.h there
instead.
hw/qdev.h is actually pointless: all it does is include hw/qdev-core.h
and hw/qdev-properties.h, which in turn includes hw/qdev-core.h.
Replace the remaining uses of hw/qdev.h by hw/qdev-properties.h.
While there, delete a few superfluous inclusions of hw/qdev-core.h.
Touching hw/qdev-properties.h now recompiles some 1200 objects.

In my "build everything" tree, changing migration/vmstate.h triggers a
recompile of some 2700 out of 6600 objects (not counting tests and
objects that don't depend on qemu/osdep.h).
hw/hw.h supposedly includes it for convenience. Several other headers
include it just to get VMStateDescription. The previous commit made
that unnecessary.
Include migration/vmstate.h only where it's still needed. Touching it
now recompiles only some 1600 objects.

The qemu coding standard is to use CamelCase for type and structure names,
and the pseries code follows that... sort of. There are quite a lot of
places where we bend the rules in order to preserve the capitalization of
internal acronyms like "PHB", "TCE", "DIMM" and most commonly "sPAPR".
That was a bad idea - it frequently leads to names ending up with hard to
read clusters of capital letters, and means they don't catch the eye as
type identifiers, which is kind of the point of the CamelCase convention in
the first place.
In short, keeping type identifiers look like CamelCase is more important
than preserving standard capitalization of internal "words". So, this
patch renames a heap of spapr internal type names to a more standard
CamelCase.
In addition to case changes, we also make some other identifier renames:
VIOsPAPR* -> SpaprVio*
The reverse word ordering was only ever used to mitigate the capital
cluster, so revert to the natural ordering.
VIOsPAPRVTYDevice -> SpaprVioVty
VIOsPAPRVLANDevice -> SpaprVioVlan
Brevity, since the "Device" didn't add useful information
sPAPRDRConnector -> SpaprDrc
sPAPRDRConnectorClass -> SpaprDrcClass
Brevity, and makes it clearer this is the same thing as a "DRC"
mentioned in many other places in the code
This is 100% a mechanical search-and-replace patch. It will, however,
conflict with essentially any and all outstanding patches touching the
spapr code.

Trying to add a spapr-nvram device currently aborts QEMU like this:

This NVRAM device registers RTAS calls during its realize function
and thus can only be used once - and that's internally from spapr.c.
So let's mark the device with user_creatable = false to avoid that
the users can crash their QEMU this way.

The blk_getlength() function can return an error value if the
image size cannot be determined. Check for this rather than
ploughing on and trying to g_malloc0() a negative number.

The member VMStateField.start is used for two things, partial data
migration for VBUFFER data (basically provide migration for a
sub-buffer) and for locating next in QTAILQ.
The implementation of the VBUFFER feature is broken when VMSTATE_ALLOC
is used. This however goes unnoticed because actually partial migration
for VBUFFER is not used at all.
Let's consolidate the usage of VMStateField.start by removing support
for partial migration for VBUFFER.

In case we do not load the NVRAM contents from a file and the user
specified the "-prom-env" parameter, use the new CHRP NVRAM helper
functions to pre-initialize the NVRAM partitions, so that the SLOF
firmware now can pick up the environment variables from the -prom-env
parameter, too.

When spapr-nvram is backed by a file using pflash interface,
migration fails on the destination guest with assert:
bdrv_co_pwritev: Assertion `!(bs->open_flags & 0x0800)' failed.
This avoids the problem by delaying the pflash update until after
the device loads complete.
This fix is similar to the one for the pflash_cfi01 migration:

We have several block drivers that understand BDRV_REQ_FUA,
and emulate it in the block layer for the rest by a full flush.
But without a way to actually request BDRV_REQ_FUA during a
pass-through blk_pwrite(), FUA-aware block drivers like NBD are
forced to repeat the emulation logic of a full flush regardless
of whether the backend they are writing to could do it more
efficiently.
This patch just wires up a flags argument; followup patches
will actually make use of it in the NBD driver and in qemu-io.

Commit 57cb38b included qapi/error.h into qemu/osdep.h to get the
Error typedef. Since then, we've moved to include qemu/osdep.h
everywhere. Its file comment explains: "To avoid getting into
possible circular include dependencies, this file should not include
any other QEMU headers, with the exceptions of config-host.h,
compiler.h, os-posix.h and os-win32.h, all of which are doing a
similar job to this file and are under similar constraints."
qapi/error.h doesn't do a similar job, and it doesn't adhere to
similar constraints: it includes qapi-types.h. That's in excess of
100KiB of crap most .c files don't actually need.
Add the typedef to qemu/typedefs.h, and include that instead of
qapi/error.h. Include qapi/error.h in .c files that need it and don't
get it now. Include qapi-types.h in qom/object.h for uint16List.
Update scripts/clean-includes accordingly. Update it further to match
reality: replace config.h by config-target.h, add sysemu/os-posix.h,
sysemu/os-win32.h. Update the list of includes in the qemu/osdep.h
comment quoted above similarly.
This reduces the number of objects depending on qapi/error.h from "all
of them" to less than a third. Unfortunately, the number depending on
qapi-types.h shrinks only a little. More work is needed for that one.

The code for -machine pseries maintains a global sPAPREnvironment structure
which keeps track of general state information about the guest platform.
This predates the existence of the MachineState structure, but performs
basically the same function.
Now that we have the generic MachineState, fold sPAPREnvironment into
sPAPRMachineState, the pseries specific subclass of MachineState.
This is mostly a matter of search and replace, although a few places which
relied on the global spapr variable are changed to find the structure via
qdev_get_machine().

The only case when sPAPR NVRAM migrates now is if is backed by a file and
copy-storage migration is performed. In other cases NVRAM does not
migrate regardless whether it is backed by a file or not.
This enables shadow copy of NVRAM in RAM which is read from a file
(if used) and used for reads. Writes to NVRAM are mirrored to the file.
This defines a VMSTATE descriptor for NVRAM device so the memory copy
of NVRAM can migrate and be flushed to a backing file on the destination
if one is specified.

Device models should access their block backends only through the
block-backend.h API. Convert them, and drop direct includes of
inappropriate headers.
Just four uses of BlockDriverState are left:
* The Xen paravirtual block device backend (xen_disk.c) opens images
itself when set up via xenbus, bypassing blockdev.c. I figure it
should go through qmp_blockdev_add() instead.
* Device model "usb-storage" prompts for keys. No other device model
does, and this one probably shouldn't do it, either.
* ide_issue_trim_cb() uses bdrv_aio_discard() instead of
blk_aio_discard() because it fishes its backend out of a BlockAIOCB,
which has only the BlockDriverState.
* PC87312State has an unused BlockDriverState[] member.
The next two commits take care of the latter two.

At the moment spapr_rtas_register() allocates a new token number for every
new RTAS callback so numbers are not fixed and depend on the number of
supported RTAS handlers and the exact order of spapr_rtas_register() calls.
These tokens are copied into the device tree and remain the same during
the guest lifetime.
When we start another guest to receive a migration, it calls
spapr_rtas_register() as well. If the number of RTAS handlers or their
order is different in QEMU on source and destination sides, the "/rtas"
node in the device tree will differ. Since migration overwrites the device
tree (as it overwrites the entire RAM), the actual RTAS config on
the destination side gets broken.
This defines global contant values for every RTAS token which QEMU
is using today.
This changes spapr_rtas_register() to accept a token number instead of
allocating one. This changes all users of spapr_rtas_register().
This changes XICS-KVM not to cache tokens registered with KVM as they
constant now.
This makes TOKEN_BASE global as RTAS_XXX use TOKEN_BASE as
a base. TOKEN_MAX is moved and renamed too and its value is changed
to the last token + 1. Boundary checks for token values are adjusted.
This reserves token numbers for "os-term" handlers and PCI hotplug
which we are working on.

RTAS is a hypervisor provided binary blob that a guest loads and
calls into to execute certain functions. It's similar to the
vsyscall page in Linux or the short lived VMCI paravirt interface
from VMware.
The QEMU implementation of the RTAS blob is simply a passthrough
that proxies all RTAS calls to the hypervisor via an hypercall.
While we pass a CPU argument for hypercall handling in QEMU, we
don't pass it for RTAS calls. Since some RTAs calls require
making hypercalls (normally RTAS is implemented as guest code) we
have nasty hacks to allow that.
Add a CPU argument to RTAS call handling so we can more easily
invoke hypercalls just as guest code would.

Curerntly the pseries VIO device code contains quite a few explicit
uses of DO_UPCAST and plain C casts. This is (obviously) type unsafe,
and not the conventional way of doing things in the QOM model. This
patch converts the code to use the QOM convention of per-type macros
to do verified casts with OBJECT_CHECK().
